The effects of one-lung ventilation mode on lung function in elderly patients undergoing esophageal cancer surgery.

The objective of the present study was to explore the effects of different one-lung ventilation (OLV) modes on lung function in elderly patients undergoing esophageal cancer surgery. A total of 180 consecutive elderly patients (ASA Grades I-II, with OLV indications) undergoing elective surgery were recruited in the study. Patients were randomly divided into 4 groups (n = 45). In Group A, patients received low tidal volume (VT < 8 mL/kg) + pressure controlled ventilation (PCV), low tidal volume (VT < 8 mL/kg) + volume-controlled ventilation (VCV) in Group B, high tidal volume (VT ≥ 8 mL/kg) + P

Post-Acute Ventilator Weaning and Discharge Outcomes in Individuals With Cervical Spinal Cord Injury.

BACKGROUND: Cervical spinal cord injury (CSCI) often results in respiratory muscle weakness, requiring prolonged mechanical ventilation and tracheostomy. However, the frequency and timing of ventilator-weaning milestones after acute hospitalization are not well described. This study assessed the rates and timing of daytime ventilator weaning, liberation from invasive ventilator support, and decannulation among adults with CSCI managed under a standardized weaning protocol at a rehabilitation center, and the associations of weaning milestones with discharge disposition. METHODS: We retrospecti

[The assistance of the ventilated infant: role of the nurse in the management of the endotracheal tube].

One of the principal duties of a staff nurse working in a NICU is the assistance of the ventilated infants. The management of the endotracheal tube is a key moment in the neonatal care and it is fundamental to prevent the ventilator induced lung injury and the change in the perfusion (especially in the brain) that may determine an impairment of gas exchange. Therefore, the role of the staff nurse is to guarantee that the tube remains correctly placed and firmly fixed in order to avoid tubes displacement and accidental extubations. High production of bronchial secretion may lead to a tube obstr

[Anaesthesiological aspects of paediatric medical care in "non-specialised departments"].

Paediatric perioperative care represents specific challenges related to the distinct developmental, anatomic and physiological characteristics of children, requiring specialised expertise, including pharmacology. A specially trained anaesthesia team, an appropriate environment and appropriate paediatric-sized equipment (endotracheal tubes, cannulas) represent key factors in determining the perioperative outcome for this population. Other important equipment that must be adapted to the paediatric population include: Non-invasive anaesthesia monitoring equipment (ECG, blood pressure cuff, pulse

Estimating occupant satisfaction of HVAC system noise using quality assessment index.

Noise may be defined as any unwanted sound. Sound becomes noise when it is too loud, unexpected, uncontrolled, happens at the wrong time, contains unwanted pure tones or unpleasant. In addition to being annoying, loud noise can cause hearing loss, and, depending on other factors, can affect stress level, sleep patterns and heart rate. The primary object for determining subjective estimations of loudness is to present sounds to a sample of listeners under controlled conditions. In heating, ventilation and air conditioning (HVAC) systems only the ventilation fan industry (e.g., bathroom exhaust

[Assessment of influence of breath holding and hyperventilation on human postural stability with spectral analysis of stabilographic signal].

The influence of breath holding and voluntary hyperventilation on the classic stabilometric parameters and the frequency characteristic of stabilographic signal were studied. We measured the stabilometric parameters on the force platform ("Ritm", Russia) on the healthy volunteers (n = 107) during quiet breath, voluntary hyperventilation (20 seconds) and maximal inspiratory breath holding (20 seconds). Respiratory frequency, respiratory amplitude and ventilation were estimated with strain gauge. We found that antero-posterior and medio-lateral sway amplitude and velocity as well as sway surface

Cross-sectional changes in lung volume measured by electrical impedance tomography are representative for the whole lung in ventilated preterm infants.

OBJECTIVE: Electrical impedance tomography measures lung volume in a cross-sectional slice of the lung. Whether these cross-sectional volume changes are representative of the whole lung has only been investigated in adults, showing conflicting results. This study aimed to compare cross-sectional and whole lung volume changes using electrical impedance tomography and respiratory inductive plethysmography. DESIGN: A prospective, single-center, observational, nonrandomized study. SETTING: The study was conducted in a neonatal ICU in the Netherlands. PATIENTS: High-frequency ventilated preterm

Chest-MRI under pulsatile flow ventilation: A new promising technique.

OBJECTIVES: Magnetic resonance imaging (MRI) of the chest has long suffered from its sensitivity to respiratory and cardiac motion with an intrinsically low signal to noise ratio and a limited spatial resolution. The purpose of this study was to perform chest MRI under an adapted non invasive pulsatile flow ventilation system (high frequency percussive ventilation, HFPV®) allowing breath hold durations 10 to 15 times longer than other existing systems. METHODS: One volunteer and one patient known for a thymic lesion underwent a chest MRI under ventilation percussion technique (VP-MR). Routine

Propofol and barbiturates for the anesthesia of refractory convulsive status epilepticus: pros and cons.

OBJECTIVE: To discuss mainly the use of propofol and barbiturates in the anesthesia of refractory status epilepticus (RSE). METHODS: Review of literature. RESULTS: There are no prospective, randomized works comparing the effects of anesthetics in the treatment of RSE. Recently, the use of propofol has increased in the treatment of RSE. Propofol terminates both clinical and electric seizures quickly, but the maintenance of burst-suppression EEG pattern requires repetitive titration of doses. Relapses of seizures have occurred in 19-33% of patients, especially when tapering of dose. The advant

Risk factors for mortality in neonatal ARDS: a multicenter retrospective cohort study in China.

As a life-threatening respiratory syndrome, epidemiological data from China has shown that the mortality rate of neonatal acute respiratory distress syndrome (ARDS) is as high as 12.5%. Nevertheless, studies on the influencing factors of this mortality remain limited. This research enrolled newborns with ARDS who initiated invasive mechanical ventilation (IMV) within 72 hours after birth. A Cox regression model with hazard ratio (HR) was constructed using the least absolute shrinkage and selection operator analysis with the lambda.1se screening criterion. Four characteristic variables were ide

The care of critically ill children after hematopoietic SCT: a North American survey.

Although potentially curative, hematopoietic SCT (HSCT) is associated with significant morbidity. To improve outcomes, multicenter studies of critical illness in this patient population appear needed. To assist in the design of such studies, a survey was conducted to identify variations in care provided to critically ill pediatric HSCT patients. A survey was conducted of the highest volume pediatric HSCT centers in the United States (n=30) and Canada (n=4). One pediatric critical care medicine (PCCM) physician and one pediatric HSCT physician were surveyed at each institution. Analysis consist

[Clinical and pathophysiologic problems associated with smoke inhalation injury].

Smoke inhalation injury is one of the primary determinants of survival following major burn injury. The primary site of injury in smoke inhalation appears to be the small airway rather than the alveoli, and thus small airway occlusion caused by edema and pseudomembrane formation are the primary mechanisms of progressive hypoxia. Ventilation-perfusion (VA/Q) alterations after smoke inhalation are characterized by increased blood flow to low VA/Q compartments, although an increase in true shunt (VA/Q = 0) was not a consistent finding. This differs considerably from most adult respiratory distres

Hypocapnia and measurement of airflow resistance and dynamic compliance as functions of respiratory frequency.

Measurements of lung mechanical behaviour as a function of respiratory frequency may be associated with significantly increased ventilation at high breathing rates. It follows that these measurements may be influenced by hypocapnia which has been shown to increase airflow resistance and to decrease dynamic compliance. To examine this possibility we continuously monitored the end-tidal CO2 tension during the determination of pulmonary resistance and dynamic compliance as functions of frequency and we compared measurements obtained by the standard method and by a technique designed to prevent th

Respiratory muscle failure.

The diseases which are commonly complicated by hypercapnic respiratory failure also compromise the respiratory muscles in several ways. Increased work of breathing, mechanical disadvantage, neuromuscular disease, impaired nutritional status, shock, hypoxemia, acidosis, and deficiency of potassium, magnesium, and inorganic phosphorus are the major non-neurologic factors which contribute to respiratory muscle fatigue and failure. Respiratory muscle fatigue has two components. High frequency fatigue occurs rapidly with intense contractile efforts but is usually not severe. It also recovers rapidl

[Present status of extracorporeal membrane oxygenation for adult respiratory failure].

In 1970s, survival rate in patients undergoing extracorporeal membrane oxygenation (ECMO) for acute respiratory failure was some around 10% even in sophisticated institutions. Most of them were treated by veno-arterial bypass along with mechanical ventilation with high air way pressure. Problems seen in this treatment modality were; difficulty in controlling bleeding and superimposed infection, mechanical problems of equipment (membrane lung, pumps, bypass circuit etc.), inadequate understanding of pathophysiology of respiratory failure. Lung injuries were also caused by high air way pressure
