Neurovascular Injury in Hip Arthroplasty.

Neurological and vascular complications following hip arthroplasty are uncommon, and their impact ranges from transient and trivial to permanent and devastating. The proximity of neural and vascular structures makes any operation on the hip potentially hazardous. Direct or indirect injuries of these structures may occur during operative exposure and subsequent procedures. Thus, complete awareness of the anatomy of the pelvis and proximal femur is required. Peripheral nerve injuries can involve either distant sites or nerves in the immediate vicinity of the hip joint. Sciatic nerve injury is th…

Hospital readmission after hip fracture.

INTRODUCTION: Readmission to the hospital following a hip fracture is common, often involves an adverse event, and strains an already overburdened health care system. OBJECTIVES: To assess the rate of 30-day readmission to the hospital after discharge for care of hip fracture. A secondary objective was measurement of the 30-day mortality rate for those patients readmitted versus those patients not readmitted to the hospital after discharge. MATERIALS AND METHODS: Study design was a retrospective review of registry data comparing readmitted patients to those not readmitted after hip fracture.…

Hip arthroscopy: less invasive technique.

The popularity of hip arthroscopy has increased significantly over the past decade. It is technically demanding and may be associated with complications such as iatrogenic damage to the articular cartilage or acetabular labrum, compression injuries to the structures in the perineum, and distraction injuries to the leg. Some of these complications can be avoided by paying meticulous attention to the technique and reducing the traction/surgical time. We describe a technique by which the risk of injury to the articular cartilage may be minimized by introduction of the surgical instruments under d…

Dislocation following total hip replacement.

BACKGROUND: Hip replacement ranks among the more successful operations on the musculoskeletal system, but it can have serious complications. A common one is dislocation of the total hip endoprosthesis, an event that arises in about 2% of patients within 1 year of the operation. Physicians should be aware of how this problem can be prevented and, if necessary, treated, so that the degree of trauma due to hip dislocation after hip replacement surgery can be kept to a minimum. METHODS: The authors searched Medline selectively for pertinent publications and analyzed the annual reports of internat…

Complex primary total hip arthroplasty.

Although total hip arthroplasty is now a classic procedure that is well controlled by orthopedic surgeons, some cases remain complex. Difficulties may be due to co-morbidities: obesity, skin problems, muscular problems, a history of neurological disease or associated morphological bone deformities. Obese patients must be informed of their specific risks and a surgical approach must be used that obtains maximum exposure. Healing of incisions is not a particular problem, but adhesions must be assessed. Neurological diseases may require tenotomy and the use of implants that limit instability. Spe…

Jogging after total hip arthroplasty.

BACKGROUND: Jogging has been classified as a high-impact sport, and jogging after total hip arthroplasty (THA) has not been well documented. PURPOSE: To investigate the participation rate for postoperative jogging as well as jogging parameters and the influence of jogging on implant stability and bearing wear. STUDY DESIGN: Case-control study; Level of evidence, 3. METHODS: Included in this study were 804 hips in 608 patients (85 men, 523 women) who underwent THA between 2005 and 2011 with follow-up longer than 1 year. The mean patient age was 62 years (range, 26-98 years), and mean follow-…

Pediatric hip: pearls and pitfalls.

Pediatric hip pathology varies depending on patient age. Newborns are often screened for developmental dysplasia of the hip, whereas toddlers and older children can present later with pain, leg length discrepancy, or limp. Young children may have hip pain related to transient synovitis, septic arthritis, or Legg-Calvé-Perthes disease. Older children are more likely to suffer from slipped capital femoral epiphysis or apophyseal avulsion fractures. Knowledge about how the growing skeleton differs from adults as well as the classic imaging findings in many of these diagnoses is paramount when tak…

Ankle fracture following hip arthroscopy.

This article describes a case of an ankle fracture following hip arthroscopy. A 58-year-old woman underwent hip arthroscopy for a labral tear. She was placed in a lateral decubitus position with her foot in a padded boot. Traction was maintained for approximately 30 minutes. She was instructed to bear weight as tolerated with crutches postoperatively. At 2-week follow-up, she reported ipsilateral ankle pain. Radiographs revealed a minimally displaced medial malleolus fracture. She was treated with a cast followed by a cam walker boot and successfully went on to complete union and resolution of…

Periacetabular osteotomy for hip preservation.

The Bernese PAO has now been performed for nearly 30 years. In that time, it has proved itself a technically complex procedure with the potential to considerably improve the natural history of the dysplastic hip. Significant refinements in the surgical technique combined with the recognition of FAI, improvements in the understanding of hip biomechanics and acetabular orientation, and more discerning patient selection have improved the outcomes related to this procedure. Although the recovery can be demanding and the potential for complications exists, the results are reproducible and the techn…

[Dislocation after total hip arthroplasty].

Dislocations after total hip arthroplasties are one of the most common complications of the procedure. According to registers, recurrent hip dislocations account for up to 30 % of the indications for a revision operation. The incidence of a dislocation is influenced by indication-associated, patient-dependent and operation-specific risk factors. 50 % of the dislocations occur within the first 3 months which confirms the high relevance of operation-specific influencing factors. The diagnosis is almost always made with the help of computed tomography, as this is the only method to determine the …

Evaluating hip pain in children.

Hip pathology in the pediatric patient can be very challenging to diagnose, as there are many diagnoses that must be considered by the clinician. Categorizing children by age can aid in the process of making an early diagnosis, which is especially important when it comes to serious pediatric hip conditions such as septic arthritis, Legg-Calvé-Perthes disease and slipped capital femoral epiphysis.

Traumatic hip dislocation--a review.

Hip dislocations are uncommon injuries that result from high-energy mechanisms. These patients require careful trauma evaluation to rule out concomitant injuries. Early closed or open reduction that is performed within 6 hours and close radiological follow-up is recommended to obtain the best possible results. It is also essential to educate the patient regarding the potential sequelae and follow them for evidence of osteonecrosis and posttraumatic arthritis.

Vascular complications in floating hip.

The so-called floating hip involves a pelvis or an acetabular fracture together with an ipsilateral femur bone fracture. Concurrent injuries are also common in these cases, with the evolution and outcome worse than expected when these fractures exist in isolation. We studied a series of 11 patients with floating hip lesions. They all presented within a general polytrauma diagnosis. Diaphyseal femur fracture together with ipsilateral acetabular fracture was the most common combination. Supracondylar fracture occurred in 3 cases; these patients also had a popliteal vascular injury, and underwent…

Hip dislocation: evaluation and management.

A simple hip dislocation is one without fracture of the proximal femur or acetabulum. Complex fracture-dislocations involve the acetabulum, femoral head, or femoral neck. The incidence of posttraumatic arthritis is much lower in simple dislocations than in fracture-dislocations. The most common mechanism of injury is a high-energy motor vehicle accident, which is usually associated with other systemic and musculoskeletal injuries. The hip should be reduced emergently in an atraumatic fashion. For acetabular fracture, intraoperative stress views may be necessary to evaluate for instability and …

Hip arthroplasty after previous arthrodesis.

UNLABELLED: Total hip arthroplasty after previous arthrodesis has been associated with increased complications and decreased survivorship of the prosthesis. We evaluated pain, function, and the factors influencing survivorship of total hip arthroplasties after previous arthrodesis between 1985 and 2000 and compared these results with those obtained in prior years with the same procedure and in the same institution. We retrospectively reviewed 30 patients who had previous spontaneous or surgical arthrodesis. The minimum followup was 2 years (mean, 10.4 years; range 2-20.5 years). Seven failures…

Ultrasound hip evaluation in achondroplasia.

BACKGROUND: During the period from 1985 to 2006, 22 children (44 hips) affected by achondroplasia were ultrasonographically evaluated. METHODS: The patients' age at examination ranged from 7 days to 29 months. The hip ultrasound (US) examination was performed, according to Graf's method, using a Siemens Sonoline sonogram with linear 5.0- and 7.5-MHz probes. In all the hips, the alpha angle was impossible to be measured because the medial margin of the ilium was not ultrasonographically detectable. The ultrasonographic findings included the following: configuration of the acetabular bony rim, …

[Arterial trauma in hip arthroplasty].

INTRODUCTION: Arterial injuries due to total hip arthroplasty can be a serious complication. The risk of vascular injury is related to proximity of the structures, tractions, and the aggressive use of instruments. OBJECTIVES: To analyze the frequency, injury mechanism, clinical presentation and therapeutic options in this kind of injury. PATIENTS AND METHOD: A group of 4162 patients subjected to total hip arthroplasty between 1986 and 2006 (reinterventions 7.2%). We describe 8 cases (5 women and 3 men) with a mean age of 61 years (reinterventions 50%). The vascular surgery was performed at l…

Obesity in total hip replacement.

A prospective, multi-centre study was carried out on 1421 total hip replacements between January 1999 and July 2007 to examine if obesity has an effect on clinical outcomes. The patients were categorised into three groups: non-obese (body mass index (BMI) < 30 kg/m(2)), obese (BMI 30 to 40 kg/m(2)) and morbidly obese (BMI > 40 kg/m(2)). The primary outcome measure was the change in Oxford hip score at five years. Secondary outcome measures included dislocation and revision rates, increased haemorrhage, deep infection, deep-vein thrombosis and pulmonary embolism, mean operating time and length …

Hip displacement in cerebral palsy.

BACKGROUND: Hip displacement is considered to be common in children with cerebral palsy but the reported incidence and the proposed risk factors vary widely. Knowledge regarding its overall incidence and associated risk factors can facilitate treatment of these children. METHODS: An inception cohort was generated from the Victorian Cerebral Palsy Register for the birth years 1990 through 1992, inclusive, and multiple data sources pertaining to the cohort were reviewed during 2004. Gross motor function was assessed for each child and was graded according to the Gross Motor Function Classificat…
