A lifestyle-driven inflammatory load index and its circulating metabolomic signature identify susceptibility to hospital-recorded acute respiratory infections: a population-based cohort study of 295,679 adults.

BACKGROUND: Whether a population-scale lifestyle-anchored inflammatory load index and its circulating metabolomic risk signature identify susceptibility to hospital-recorded acute respiratory infections (ARIs) has not been well established. This study examined these questions in the UK Biobank using the Circadian Imbalance Index (CII) and a nuclear magnetic resonance (NMR)-derived metabolomic risk signature (MRS). METHODS: This prospective cohort study included 295,679 UK Biobank participants after prespecified exclusions. The CII was derived from five baseline components: evening chronotype,

Development and validation of a delirium risk prediction model using a modified version of the Delirium Elderly at Risk (mDEAR) screen in hospitalized patients aged 65 and older: A medical record review.

Most delirium risk prediction models are cumbersome to use, time consuming to complete, and require education ensure accuracy. The purpose of this study was to develop and validate a risk prediction model using routinely assessed risk factors predictive of delirium including: cognitive impairment, ≥80-years old, functional dependence, sensory impairment, and chronic substance use. This retrospective study included 7999 encounters of hospitalized patients aged 65-years or older admitted from 1/1/2019 to 12/31/2019. Various models were compared, with the best tested for validation. A model, wher

The Influence of an Electronic Medical Record Embedded Best Practice Alert on Rate of Hospital Acquired Catheter Associated Urinary Tract Infections: Do Best Practice Alerts Reduce CAUTIs?

OBJECTIVE: To understand if an electronic medical record embedded best practice alert decreased our hospital's Catheter-associated urinary tract infections (CAUTIs) and catheter utilization (CU) rates. METHODS: Data from our inpatient prospective CAUTI database, spanning 2011 to 2016, were utilized for our analysis with the Best Practice Alert (BPA) starting in 2013. Using generalized linear models we compared the CU and CAUTI rates between pre- and post-BPA periods in different patient subpopulations. RESULTS: We identified no decrease in the CU rate and no effect on the CAUTI rates as a re

Australian Aboriginal children have higher hospitalization rates for otitis media but lower surgical procedures than non-Aboriginal children: A record linkage population-based cohort study.

INTRODUCTION: Otitis media (OM) is one of the most common infectious diseases affecting children globally and the most common reason for antibiotic prescription and paediatric surgery. Australian Aboriginal children have higher rates of OM than non-Aboriginal children; however, there are no data comparing OM hospitalization rates between them at the population level. We report temporal trends for OM hospitalizations and in-hospital tympanostomy tube insertion (TTI) in a cohort of 469,589 Western Australian children born between 1996 and 2012. MATERIALS AND METHODS: We used the International C

Preoperative Frailty Assessed by the Record-Based Multidimensional Prognostic Index Predicts 90-Day Days Alive and out of Hospital Following Radical Cystectomy for Bladder Cancer: A Retrospective Cohort Study.

Background/Objectives: Radical cystectomy (RC) for muscle-invasive bladder cancer (MIBC) is associated with high morbidity. Frailty is an important determinant of surgical outcomes; however, its association with the composite outcome Days Alive and Out of Hospital (DAOH) has not been examined following RC. We assessed the impact of preoperative frailty on 90-day DAOH in older patients undergoing RC for MIBC. Methods: We conducted a retrospective cohort study including 408 consecutive patients aged ≥65 years undergoing RC at a tertiary referral center between 2018 and 2023. Frailty was assessed

Associations of recorded in-hospital linezolid and fluoroquinolone exposure with mortality and documented drug-induced liver injury in non-HIV tuberculous meningitis: a propensity score-weighted real-world cohort study.

BACKGROUND: Tuberculous meningitis (TBM) remains the most severe form of tuberculosis. Linezolid- and fluoroquinolone-containing regimens are used in clinical practice, but their associations with mortality and specific toxicities in non-HIV TBM remain uncertain. METHODS: We conducted a single-center retrospective cohort study of non-HIV patients with TBM hospitalized between October 2013 and January 2024. The exposures were recorded linezolid use, fluoroquinolone-class use, and combined linezolid-fluoroquinolone use during the index hospitalization. Each indexed exposure was compared with no

Screening for Life-Threatening Arrhythmia in Asymptomatic Patients After Paediatric Cardiac Surgery: A Single-Centre Retrospective Analysis of 790 Pre-hospital-discharge 24-h Holter Electocardiogram Recordings.

Severe arrhythmias may occur early after open heart surgery. Because younger patients do not usually show any specific symptoms, presently Holter monitoring is routinely performed for 24 h predischarge at our centre to prevent adverse outcomes. It is unknown whether this test is truly justified in this patient population. Retrospective single-centre analysis of all consecutive patients younger than 19 years old after open heart surgery 2013-2019 who underwent routine Holter monitoring before hospital discharge. Patients with permanent pacemakers and patients who died during this hospital stay

Medicare program; hospital inpatient prospective payment systems for acute care hospitals and the long-term care hospital prospective payment system and fiscal year 2015 rates; quality reporting requirements for specific providers; reasonable compensation equivalents for physician services in excluded hospitals and certain teaching hospitals; provider administrative appeals and judicial review; enforcement provisions for organ transplant centers; and electronic health record (EHR) incentive program. Final rule.

We are revising the Medicare hospital inpatient prospective payment systems (IPPS) for operating and capital-related costs of acute care hospitals to implement changes arising from our continuing experience with these systems. Some of these changes implement certain statutory provisions contained in the Patient Protection and Affordable Care Act and the Health Care and Education Reconciliation Act of 2010 (collectively known as the Affordable Care Act), the Protecting Access to Medicare Act of 2014, and other legislation. These changes are applicable to discharges occurring on or after October
