A 30-year-old man presented with massive left hydronephrosis caused by congenital stricture of the ureteropelvic junction. Glomerular filtration rate of the post-released kidney (inulin clearance and creatinine clearance) was preserved completely. INTRODUCTION: Giant hydronephrosis is defined as having 1.5 L or 1.6% of body weight of fluid in the renal pelvis. In the exception of a few, the preponderance of giant hydronephrosis is attributable to congenital anomalies, mostly pelvi-ureteric junction obstruction (PUJO). However, giant hydronephrosis could also be attributed to ureteric calculi, ureteral tumors or trauma. A 58-year old female was referred to our hospital due to left renal cyst that was pointed out at her health check-up. Abdominal CT scan showed left hydronephrosis with a 20 x 12 x 11cm tumor. The serum CA19-9 level elevated to 4,400 U/ml.
