The stability of total talar prosthesis-How stable to dislocation? Cadaveric study.

The aim of this study was to characterize ankle stability of total talar prosthesis (TTP) and to determine the effect of implant sizes on stability as well as the resistance to TTP dislocation. Twelve below-knee cadaveric specimens were divided into two groups. Group 1 received a size matched implant and Group 2 received downsized implant by 5%. The stability assessment under fluoroscopy was performed for each cadaver in its native state. Following TTP insertion process, each then underwent evaluation of the TTP ankle stability. The stability of pre- and post-TTP was compared. (1) Anterior dra

Patella incarceration with associated patella fracture: a rare case of a paediatric patient with no associated femoral fracture.

We describe the case of a 15-year-old girl who presented to the Accident and Emergency Department with right knee pain and a tense effusion following a twist and fall directly onto her right knee. An MRI scan demonstrated that she had an incarcerated dislocated patella with an associated patella avulsion fracture. This required open reduction. Open reduction and fixation was performed using suture anchors. We feel that this case is particularly pertinent since nearly all previous case reports describe an incarcerated patella with an associated femur fracture. Furthermore, no previous case repo

When does the patella dislocate? A systematic review of biomechanical & kinematic studies.

BACKGROUND: Patellar dislocations are a significant injury with the potential for long term problems. Little work has been done on establishing the mechanism by which this injury occurs. OBJECTIVES: To determine the mechanism of injury of a patella dislocation based on the available published literature and compare them to already proposed theories. METHODS: A systematic review of the literature was conducted following searches performed on MEDLINE, EMBASE and ProQuest from the earliest year of indexing using the following search terms in any combination: "patella", "dislocation", "mechanism

Value of CT scan-assessed tibial tuberosity-trochlear groove distance in identification of patellar instability.

PURPOSE: Tibial tuberosity-trochlear groove (TTTG) distance is currently considered as one of the main factors in decision-making for surgical intervention in patients with patellar instability. The current study aimed to provide a valid comparison of TTTG distance in the healthy knees with those with patellar instability. MATERIALS AND METHODS: Patients with a history of two or more episodes of patellar dislocation in the same knee who were referred to our department for the assessment of TTTG distance were included. Asymptomatic knees of patients as well as both knees of 21 healthy individu

Short-term outcomes after hybrid unicompartmental knee arthroplasty: A retrospective cohort study with minimum 3-year follow-up.

This study aims to evaluate the survival rate and short-term clinical outcomes of hybrid unicompartmental knee arthroplasty (UKA). We retrospectively analyzed 155 Oxford phase-3 hybrid UKAs in 155 patients who were followed for more than 3 years. Kaplan-Meier survival curves were generated using revision as an endpoint. Oxford knee score (OKS) and range of motion (ROM) were evaluated for clinical assessment, and radiographs were used to assess implant-related complications. At an average follow-up of 3.6 years (range, 3-6 years), 2 knees required revision. The reasons for revision were bearing

Patient-specific gait pattern in individuals with patellofemoral instability reduces knee joint loads.

Patellofemoral instability is influenced by morphological factors and associated with compensational alterations in gait pattern. Recent simulation studies investigated the impact of knee morphology on the stability and loading of the patellofemoral joint but neglected the patient-specific gait pattern. The aim of this study was to investigate the impact of patient-specific gait pattern on muscle forces and joint loading in individuals with patellofemoral instability. Musculoskeletal simulations with a model including a twelve degrees of freedom knee joint were performed based on three-dimensi

Intra-Articular Dislocation of the Patella With Associated Hoffa Fracture in a Skeletally Immature Patient.

Since 1887, approximately 50 cases of an intra-articular patellar dislocation have been reported in the worldwide literature. The vast majority of patients required an open reduction of the patella or closed reduction under general anesthesia. This injury has never been reported in association with a coronal shear fracture of the femoral condyle. A 14-year-old boy presented to our institution with his left knee locked in flexion after a direct blow. Radiographs showed the patella rotated on its horizontal axis and lying in a transverse position within the knee joint, as well as a concomitant f

Floating knee in children.

The outcomes of 18 ipsilateral displaced femoral and tibial fractures in 17 children are assessed and a new classification system is proposed. Average age was 8.75 years, and follow-up averaged 3.2 years. In the modified Bohn and Durbin classification used, eight cases were type I, four were type II, three were type IIIa, one was type IIIb, and two were type IV. In tibial fractures there was angulation in three cases, and in femoral fractures there were dislocation and angulation in four cases, refracture in one case, leg length discrepancy in four cases, and asymptomatic knee ligament injury

The Twin Peg Oxford partial knee replacement: the first 100 cases.

We present the clinical results of the first 100 patients who received the Twin Peg Oxford partial knee replacement which has a 15° extra femoral surface for contact in deep flexion, and has two pegs for more secure fixation. We measured the clinical outcome 2 years after the medial unicompartmental arthroplasty using patient and surgeon derived outcome measures. The results showed a mean Oxford Knee Score of 41, a mean American Knee Society Objective Score of 93 and a Functional Score of 84, a mean range of motion of 130° and a high satisfaction rate. Results were significantly better in male

The Management of Orthopaedic Emergencies for the Community Surgeon.

Orthopaedic emergencies demand rapid recognition and decisive action, particularly in resource-constrained or nontertiary settings where the community surgeon often serves as the first and only line of defense. It is important to provide a practical, evidence-informed guide for the management of musculoskeletal injuries, including irreducible joint dislocations, ultra-low-energy knee dislocations, open fractures with vascular compromise, compartment syndrome, and femoral neck fractures in young patients. Emphasis is placed on early diagnosis, stabilization strategies, soft-tissue management, a…

Management of Pipkin Type-III Femoral Head Fracture and Ipsilateral Open Floating Knee with Extensor Apparatus Injury: A Case Report.

CASE: A twenty-eight-year-old man presented with a Pipkin type-III femoral head fracture and open floating knee injury. Initially debridement was performed, during which the extensor apparatus could not be salvaged. The top priority in management of the floating knee was achievement of adequate soft-tissue coverage followed by skeletal stabilization as staged procedures. Because the femoral head was split, dislocated, comminuted, and deemed nonreconstructible, arthroplasty was considered the best option. The extensor mechanism damage was addressed by reconstruction using an iliotibial tract fl

Osteomyelitis variolosa: a case report.

Osteomyelitis variolosa is an infection of bone and joints by smallpox virus variola major, most commonly in the elbows, wrists, ankles, hands, and feet. We report one such case in a 70-year-old woman who presented with deformities of the right knee, both elbows and ankles, and the left hand, and a history of childhood fever with rashes. Her lateral femoral condyle of the right knee was hypoplastic with patella baja. Her right elbow was ankylosed and her left elbow was dislocated with multidirectional instability. Her third and fourth metacarpals on the left hand were shortened. Both ankles we

Effect of surgical approach for contralateral side hip arthroplasty in below knee amputees: a retrospective cohort study.

BACKGROUND: The gluteus medius muscle plays a very important role in the stability of the gait, especially in patients with amputation of the lower limbs. Therefore, choosing the appropriate type of approach for hip arthroplasty is very important. Hence, this study aimed to compare the outcomes and complications between the anterolateral approach (ALA) and posterior approach (PA) for hip arthroplasty in patients with contralateral below knee amputation. METHODS: From January 1999 to November 2014, 67 patients who underwent hip arthroplasty with contralateral below knee amputation were retrosp

Patellar Morphology in Recurrent Lateral Patellofemoral Instability: Associations With Risk Factors and Postoperative Outcomes.

BACKGROUND: Patellar morphology, most commonly described by the Wiberg classification, is a potential risk factor for recurrent lateral patellofemoral instability. However, the Wiberg classification has not been widely used in this population, and therefore its clinical utility remains unclear. PURPOSE: (1) To determine the incidence of Wiberg types and to explore relationships between the Wiberg classification and risk factors in a population of patients with recurrent lateral patellofemoral instability. (2) To evaluate the relationship between the Wiberg classification and disease-specific
