BACKGROUND: Primary pancreatic leiomyosarcoma is a rare pancreatic malignancy; the clinical presentation and treatment is not well-characterized. Further, the molecular mechanisms underlying its pathogenesis are not known. We report a patient with pancreatic stromal tumor that progressed to primary pancreatic leiomyosarcoma with hepatic and peritoneal metastases.

BACKGROUND: Leiomyosarcoma (LMS) of the gastrointestinal tract is an extremely rare high-grade neoplasm with poor prognosis. For advanced LMS with distant metastasis, the decision as to the choice of the most appropriate therapeutic strategy, including chemotherapy and surgery, is difficult. Leiomyosarcomas of the broad ligament of the uterus are extremely rare tumors. Most of the time they are diagnosed in older women and despite radical surgical treatment prognosis is dismal. We present a rare case of a 38-year-old nulligravida presenting with a large, firm mass, fixed in the pelvis. After enucleation of the tumor, histological examination revealed the presence of a broad ligament leiomyosarcoma, leading to total abdominal hysterectomy and bilateral salpingo-ophorectomy three weeks later. A 59-year-old male was referred to our hospital with the chief complaint of a painless scrotal mass. A 6 X 4.5 X 4 cm elastic hard mass with irregular surface was palpable in the right scrotum. We report an unusual case of a leiomyosarcoma of the urinary bladder after chemotherapy for retinoblastoma and the results of a review of the published reports of bladder sarcomas.
