Direction-fixed paroxysmal nystagmus lateral canal benign paroxysmal positioning vertigo (BPPV): another form of lateral canalolithiasis.

Benign paroxysmal positioning vertigo (BPPV) is the most frequent vertiginous syndrome. It is caused either by free-floating otoliths in the semicircular canals (canalolithiasis) or by otoconial debris adhering to a canal cupula (cupulolithiasis). The posterior canal is the most frequently involved (80%), while the lateral canal is involved less frequently (15%), and the rarest conditions are anterior canalolithiasis and apogeotropic posterior canalolithiasis (5%). The main diagnostic sign of lateral canal BPPV is paroxysmal horizontal bidirectional positioning nystagmus evoked through Pagnini…

Testicular microlithiasis in boys and young men with congenital or acquired undescended (ascending) testis.

PURPOSE: We assessed the prevalence of testicular microlithiasis by ultrasound in boys and young men with congenital or acquired undescended (ascending) testis. MATERIALS AND METHODS: During followup for testicular growth patients with congenital or acquired undescended (ascending) testis were also screened by ultrasound for testicular microlithiasis, which was defined as echogenic foci without shadowing within the testis parenchyma. Classic microlithiasis was defined as 5 or more echogenic foci in either or both testes and limited microlithiasis as fewer than 5 foci. RESULTS: We performed 1…

Rhinolithiasis: the importance of flat panel detector-based cone beam computed tomography in diagnosis and treatment.

Rhinolithiasis is a disease caused by deposition of organic and inorganic compounds in the nasal cavity. A nidus may be endogenous or exogenous. Rhinoliths are uncommon and often an asymptomatic condition diagnosed accidentally during a routine examination; but sometimes they may cause complications such as unilateral nasal obstruction, fetid rhinorrhea, or epistaxis. This article describes a case of rhinolith involving a 21-year-old female. The flat panel detector-based cone beam computed tomography (FPD-CBCT) findings, differential diagnosis, and treatment in a case of rhinolith are presente…

A case of tooth-filling antral rhinolithiasis presenting with blockage of the osteomeatal complex.

Tooth-filling antral rhinolithiasis is the phenomenon by which dental fillings are extruded into the maxillary sinus and are a source of infection. These fillings eventually become coated by salts of calcium and therefore form a stone. We describe a rare case in which the dental amalgam was eventually seen to cause osteomeatal complex obstruction after entering the sinus.

Pulmonary alveolar microlithiasis in a patient with rheumatic valvular heart disease.

Pulmonary alveolar microlithiasis is a rare disease characterized by widespread intra-alveolar calcification of both lungs that is asymptomatic in the early stages. The disease typically follows a protracted course, and death can occur in 5 to 41 years after the initial diagnosis. Rheumatic fever is a multisystemic inflammatory disease that afflicts the child and juvenile population, and it is still very common in developing countries. Valve failure is the condition most linked to increased morbidity and mortality rates in this population and is the most severe complication of rheumatic fever,…

Broncholithiasis in an immune compromised boy with disseminated Mycobacterium kansasii.

A case of broncholithiasis in a child is reported. To our knowledge, it has not been reported in children. Broncholithiasis is a condition in which a peribronchial calcified lymph node erodes into or distorts an adjacent bronchus. Symptoms of broncholithiasis include cough, recurrent episodes of fever, haemoptysis, and purulent sputum. The most common cause of broncholithiasis is Mycobacterium tuberculosis (M. tuberculosis). Here we describe a 14-year-old boy known to have disseminated Mycobacterium kansasii (M. kansasii) infection associated with hypoplastic myelodysplastic syndrome (MDS). He…

A newly recognised cause of vertigo: horizontal canal variant of benign positional vertigo.

AIMS: To describe the presentation, causes, treatment, and outcomes of patients presenting with the newly recognised horizontal canal varian … ositioning is in the horizontal plane, with the direction depending on whether the mechanism is canalithiasis or cupulolithiasis. CONCLUSIONS: Horizontal canal BPV explains nearly all variations on 'classical' (posterior canal) BPV. It accounts for at least 10% of BPV and has been frequently misdiagnosed. Repositioning is usually curative if the symptomati …

On-table cholangioscopy for hepatolithiasis in a patient with gallstone ileus post hepaticojejunostomy.

Roux-en-Y hepaticojejunostomies (RYHJs) can be associated with biliary complications. We describe a very rare case of gallstone ileus causing a Roux limb obstruction in a patient who had recurrent hepatolithiasis and cholangitis secondary to residual stones despite repeated cholangioscopy. The obstruction was managed with enterotomy and removal of gallstone via an open surgical approach, with complete clearance of hepatolithiasis through on-table trans-enteric cholangioscopy.

Gynura segetum-induced liver injury leading to acute liver failure: a case report and literature review.

BACKGROUND: Gynura segetum (GS) is widely used in medical care and in community settings in China as the herbal remedy. It is widely thought to have antiphlogistic properties and pain relief in traditional Chinese medicine. It has been reported that GS can cause chronic drug-induced liver injury (DILI), manifested as hepatic sinusoid obstruction syndrome (HOSO). But case reports of acute DILI developing acute liver failure (ALF) due to GS are extremely rare. CASE PRESENTATION: We report a case of a 63-year-old female patient with hepatolithiasis for more than 6 years. There were no deteriorat…

Biliary tract exploration via left hepatic duct stump versus the common bile duct incision in left-sided hepatolithiasis: a meta-analysis.

BACKGROUND: Left lateral hepatic resection is the preferred surgical approach for treating left hepatolithiasis. However, it is not clear whether cholangioscopy via left hepatic duct (LHD) orifice can replace conventional common bile duct (CBD) approach during laparoscopic procedures. METHODS: We performed a comprehensive literature search by screening medical databases, then compared perioperative outcomes and occurrence of recurrent stones between LHD and CBD approaches. RESULTS: A total of five studies, comprising 345 patients, were included in this meta-analysis. The reported operative t…

Lithopedion Causing Intestinal Obstruction in a 71-Year-Old Woman: A Case Report Litopedia causando obstrução intestinal em idosa de 71 anos: relato de caso.

Ectopic pregnancy is the leading cause of pregnancy-related death during the first trimester, and it occurs in 1 to 2% of pregnancies. Over 90% of ectopic pregnancies are located in the fallopian tube. Abdominal pregnancy refers to an ectopic pregnancy that has implanted in the peritoneal cavity, external to the uterine cavity and fallopian tubes. The estimated incidence is 1 per 10,000 births and 1.4% of ectopic pregnancies. Lithopedion is a rare type of ectopic pregnancy, and it occurs when the fetus from an unrecognized abdominal pregnancy may die and calcify. The resulting "stone baby" may

Lingual tonsillolith: prevalence and imaging characteristics evaluated on 2244 pairs of panoramic radiographs and CT images.

OBJECTIVES: Lingual tonsilloliths are not as well-known to radiologists than palatine tonsilloliths, although they might be common in clinical practice. The aim of this investigation was to clarify the prevalence and imaging characteristics of lingual tonsilloliths using panoramic radiographs and CT images. METHODS: This study included 2244 patients without pathology at the base of tongue who had undergone panoramic radiography and CT of the maxillofacial region. The size, number and position of lingual tonsilloliths relative to the mandible and tongue were evaluated. RESULTS: Lingual tonsil

Management and follow-up of pediatric asymptomatic testicular microlithiasis: are we doing it well?

PURPOSE: To define timing and methods for a balanced follow-up of testicular microlithiasis (TM) in pediatric age. MATERIALS AND METHODS: We retrospectively reviewed medical records of 21 pediatric asymptomatic patients (42 testicular units) diagnosed with TM and without associated risk factors. Microliths were found bilaterally on ultrasonography in all the patients. Distribution of microliths (focal or diffuse) inside the parenchyma was evaluated as well as its eventual variation over time. Every six months, each patient underwent clinical and ultrasonography evaluation, as well as serum ch…

"Secondary signs of lateralization" in apogeotropic lateral canalolithiasis.

The diagnosis of lateral canalolithiasis is based on the typical finding of the horizontal paroxysmal positional nystagmus induced by the Pagnini-Mc Clure manoeuvre. This technique usually identifies also the affected side, namely, the side where the paroxysmal nystagmus is more intense in geotropic forms and the side where the paroxysmal nystagmus is less intense in apogeotropic forms. However, this method is not always applicable since, especially in apogeotropic forms, the intensity of the nystagmus is not so distinctly different between the two sides. Further useful signs to identify the a…
