Alcohol-associated liver disease.

Alcohol-associated liver disease (ALD) is the leading cause of liver-related mortality in Western countries. The assessment of histological severity in ALD now relies predominantly on non-invasive methods, particularly elastography and blood-based tests, commercially available or laboratory-developed. These non-invasive approaches have largely replaced the need for liver biopsy. Management of ALD is based primarily on the treatment of alcohol use disorder, and access to pharmacological therapies and addiction specialists should be improved. At present, there is no specific treatment for ALD, w…

AASLD EXPO: Liver biopsy.

BACKGROUND: Liver biopsy is a foundational tool in hepatology, providing direct histologic assessment of hepatic parenchyma. Although noninvasive approaches have substantially reduced the need for biopsy in practice, it continues to play a critical role in diagnosis, staging, and therapeutic decision-making across a broad spectrum of liver disorders. METHODS: This Expert Practice Opinion (EXPO) document was created by a writing group of subject matter experts appointed by the AASLD Practice Guidelines Committee. Writing group members performed a targeted literature review and created expert o…

Advanced robotic liver surgery.

BACKGROUND: Robotic surgery represents the most advanced evolution of minimally invasive liver surgery, progressively extending its application from minor resections to highly complex hepatobiliary procedures. Beyond the traditional definition of major hepatectomy, surgical complexity is increasingly determined by anatomical location, proximity to major vascular and biliary structures, patient-related factors, and integration within multimodal treatment strategies. METHODS: A narrative and critical review of the literature was conducted using PubMed/MEDLINE, focusing on robotic liver resectio…

Liver Metabolism and Function.

The liver is an extraordinary organ defined by its extensive roles that maintain a healthy state. The liver metabolizes carbohydrate, protein, and lipid for energy, transforms these to other nutrient classes, and can package nutrients to export or store them within the liver. In addition to its role in nutrient homeostasis, the liver has an excretory function, detoxifies and eliminates countless endogenous and exogenous substances, is a site of vitamin and mineral storage. The liver has capacity to maintain most of its functions in the face of disease until about 70% of the hepatic mass is aff…

Radiotherapy for liver tumours⭑.

Radiotherapy for hepatic tumours has evolved from palliative care to a potentially curative approach, thanks to advancements like stereotactic body radiotherapy. This technique allows for precise targeting while sparing healthy liver tissue, making it an option for hepatocellular carcinoma and liver metastases when surgery or ablation are contraindicated, achieving high response rates. Additionally, combined approaches such as stereotactic body radiotherapy following transarterial chemoembolization improve tumour response. Emerging modalities like magnetic resonance imaging-guided adaptive rad…

Xenobiotics Induced Liver Toxicity.

Technological advancement and industrialization have significantly increased human exposure to xenobiotics, a diverse group of foreign chemical substances with potential toxic effects. Xenobiotics include pharmaceuticals, pesticides, personal care products, industrial chemicals, and environmental pollutants. The liver, as the primary organ responsible for biotransformation, plays a central role in the detoxification and activation of these compounds. However, metabolic processes may generate reactive intermediates that contribute to hepatocellular injury. This review provides a comprehensive o…

Semaglutide-Induced Liver Injury.

Semaglutide, a glucagon-like peptide-1 receptor agonist widely used for managing type 2 diabetes mellitus and obesity. Reports of semaglutide-associated hepatotoxicity are exceedingly rare. We detail a case of a 44-year-old woman who developed liver injury with elevated liver enzymes after semaglutide initiation for weight management. Liver enzyme levels normalized after discontinuing the drug and worsened upon rechallenge, confirming semaglutide's potential to cause liver injury. In addition, our case study encompasses a literature review of all reported semaglutide-related drug-induced liver…

CD36 in liver diseases.

Cluster of differentiation 36 (CD36) is a transmembrane glycoprotein with the ability to bind to multiple ligands and perform diverse functions. Through the recognition of long-chain fatty acids, proteins containing thrombospondin structural homology repeat domains such as thrombospondin-1, and molecules with molecular structures consistent with danger- or pathogen-associated molecular patterns, CD36 participates in various physiological and pathological processes of the body. CD36 is widely expressed in various cell types, including hepatocytes and KCs in the liver, where it plays a pivotal r…

Fontan-Associated Liver Disease.

Fontan-associated liver disease (FALD) occurs in all patients who have undergone Fontan palliation for functional single ventricle congenital heart defects. While liver fibrosis is universal in patients who have undergone Fontan palliation, FALD may lead to more serious consequences including portal hypertension, cirrhosis, and hepatocellular carcinoma. Scientific studies of the pathophysiology and clinical management of FALD have been limited to date by the heterogeneous nature of the disease, relatively small population of patients with Fontan physiology, and inaccuracy of noninvasive stagin…

[Small liver vessels disorders].

Small liver vessels disorders are a heterogeneous group of rare diseases that can affect the portal venules, the hepatic sinusoids, the centri-lobular veins, or the hepatic arteries. The most frequent is the porto-sinusoidal vascular disorder (PSVD), which is characterised by damage of the portal venules or sinusoids and may be associated with portal hypertension, in the absence of cirrhosis. Diagnosis is therefore based on a liver biopsy. PSVD is often associated with an extrahepatic condition, most commonly immune-mediated, haematological, or a toxic. Its two main complications are variceal …

[The infectious liver tumour].

Alveolar echinococcosis remains a rare parasitic disease, but the number of cases in Western Europe is increasing. Diagnosis is difficult due to a long incubation period and non-specific symptoms. Curative treatment can only be achieved through R0 resection. Suppressive therapy with albendazole can now achieve good long-term survival even in cases where surgery is not possible.

[Infections and liver cirrhosis].

End-stage liver disease is a life-threatening clinical syndrome combined with a state of immune dysfunction. In this constellation patients are prone to bacterial, fungal and viral infections associated with markedly increased morbidity and mortality rates. Bacterial infections are the most prevalent kind of infection in patients with end-stage liver disease accounting for nearly 30%. The evolving rates of multidrug resistant organisms present enormous challenges in treatment strategies. Therefore, the urgent needs for prevention, early detection strategies and widespread treatment options are…

Nontraumatic intrathoracic liver incarceration.

Diaphragmatic hernias are a rare finding in adult population, especially in urgent settings. Nevertheless, the acute symptoms can be life-threatening and may require urgent surgical intervention. We report a rare case of a nontraumatic diaphragmatic hernia in a young adult with a history of laparotomy at the age of 1, suggesting a pre-existing malformation. The clinical presentation was dominated by respiratory symptoms and biological signs of hepatic ischemia, with no abdominal pain. Computed tomography demonstrated protrusion of the entire liver into the thoracic cavity. In this context, an …

"No-donor" liver transplantation.

Liver transplantation is hindered by organ shortage. The potential way to relieve this issue is to expand the donor pool via extending the donor criteria and make full use of all available grafts. The concept of "no-donor" liver transplantation allows grafts to be recovered from other liver recipients. This review summarizes the current clinical practice of "no-donor" liver transplantation, focusing on the experiences of Chinese transplant teams. Domino liver transplantation was introduced by Furtado in 1995 and implemented later in 2013 in China, and novel donor indications including some ess…

Dyslipidaemia in Liver Diseases.

The liver is the central organ in lipid metabolism and plays a key role in a variety of biochemical processes. It is involved in lipoprotein synthesis, fatty acid beta oxidation, ketone body production, cholesterol synthesis, bile production, and storage and mobilization of lipids. Metabolic diseases such as obesity, type 2 diabetes mellitus and certain dyslipidaemias can lead to chronic liver conditions, especially non-alcoholic fatty liver disease. Conversely, chronic liver diseases such as liver cirrhosis and chronic cholestasis can induce dyslipidaemias. This review provides a comprehensiv…
