Inflammatory triggers of lymphangiogenesis.

Inflammation is the common denominator to the postnatal events that overlap with lymphatic vessel growth, or lymphangiogenesis. Undoubtedly, inflammation and accompanying fluid overload are cardinal factors in wound healing, lymphedema, the pathogenesis of some forms of lymphangiomatosis, and solid tumor lymphangiogenesis. The assertion that inflammation actually triggers lymphangiogenesis lies in the evidence set forth below that inflammation is the usual precursor to tissue repair and regeneration. Moreover, the panel of pro-inflammatory and anti-inflammatory molecules that orchestrates the

Magnetic resonance lymphangiography: with or without contrast?

Lymphedema is an important medical issue around the world, caused by an anomalous collection of fluid in soft tissue due to congenital malformations or stenosis or obstruction of lymphatic vessels. Magnetic resonance lymphangiography (MRL) is an emerging technique focused on noninvasive or minimally invasive imaging of lymphatics with the goal to diagnose and treat lymphedema. This review will briefly discuss lymphatic imaging starting with lymphography and radionuclide lymphoscintigraphy up to the newest methods, focusing on MRL, a rising technique, and highlighting the technical aspects fund…

[Bacterial infections of skin and soft tissues in filariasis].

Adenolymphangitis is a common occurrence in filarial lymphedema. Damage to the lymphatics and lymph nodes by F. bancrofti is followed by obliteration of lymph vessels and lymph stasis. Obstruction of lymphatics prevents the bacteria penetrating skin to be evacuated with lymph stream to regional lymph nodes. Colonization of dermis, subcutis and lymphatics evokes clinical symptoms of adenolymphangitis. The question arises which strains of bacteria are responsible for the acute and chronic types of adenolymphangitis. The most probable strains responsible for this condition belong to the cocci and

Transplantation of lymph node fragments in a rabbit ear lymphedema model: a new method for restoring the lymphatic pathway.

The treatment of lymphedema by medical or surgical means remains a difficult task, and no technique is presently satisfactory. We establishe … the end of the experiment (p < 0.001). Indirect lymphographic examination revealed that the distal lymphatic vessels of the control group were increased in number, dilated, and did not allow proximal passage of dye. Light microscopy revealed the vascular pedicle to be surrounded by dense scar tissue in the "bridge." The transplantation group showed free passage of …

[Isolated splenic lymphangioma].

Splenic lymphangioma is a rare malformation of splenic lymphatic channels characterized by cysts resulting from increased number of enlarged thin-walled lymphatic vessels. In our case, there were no clinical manifestations. Lymphangioma was congenital and diagnosed by ultrasound as an accidental finding. Surgery is the only method of radical treatment of splenic lymphangioma. We describe an extremely rare case of pediatric isolated splenic lymphangioma and laparoscopic resection of spleen as the most advantageous variant of surgical treatment.

TIE1-dependent lymphatic vascular remodeling is mediated by its second tyrosine kinase domain.

Mutations in ANG2 and TIE1 are associated with primary lymphedema in humans, but the mechanisms of ANG/TIE signaling in the lymphatic vasculature remain incompletely understood. We document that TIE2 is not detected in lymphatic endothelial cells (LECs) before E14.5 but is expressed in collecting vessels from later embryonic stages, in contrast to robust TIE1 expression in all LECs from early stages. Accordingly, only LEC-specific deletion of Tie1 but not Tie2 resulted in defective lymphatic development and abnormal valve function. We discovered that defects of Tie1 lymphatic knockout mice wer

Endogenous Calcitonin Gene-Related Peptide Deficiency Exacerbates Postoperative Lymphedema by Suppressing Lymphatic Capillary Formation and M2 Macrophage Accumulation.

Lymphedema is a chronic condition caused by disruption of lymphatic vessels, which often occurs after invasive surgery. Calcitonin gene-related peptide (CGRP) is a 37-amino acid peptide produced by alternative splicing of the primary transcript of the calcitonin/CGRP gene (Calca). CGRP was initially identified as a neuropeptide released primarily from sensory nerves and involved in regulating pathophysiological nociceptive pain. However, recent studies have shown CGRP is also released from a variety of other cells and possesses multiple functions. In this study, CGRP knockout (-/-) mice were u…

Lymphatic System Transfer (LYST) with Pedicled SCIP for Patients with Lymphedema and Concomitant Chronic Venous Disease.

Lymphatics System Transfer (LYST) is a recently described technique that combines the benefits of the established Vascularized Lymph Node Transfer (VLNT) with the advantages of Vascularized Lymph Vessel Transfer (VLVT). This procedure involves harvesting vascularized tissue that includes superficial lymphatic vessels and their corresponding draining nodes and transferring the tissue flap to the upper or lower extremity by microsurgical anastomosis. We present a novel pedicled LYST technique utilizing a Superficial Circumflex Iliac Perforator (SCIP) flap to treat patients with lower extremity l…

A case of kaposiform hemangioendothelioma at the sigmoid colon.

We report a case of kaposiform hemangioendothelioma (KHE) occurring at the sigmoid colon in an 8-year-old male. He had experienced lower abdominal pain and fever for several days. Contrast-enhanced CT images revealed a well-enhanced mass involving several large and small cystic parts with thin, enhanced walls at the sigmoid colon. The diagnosis was made histopathologically after surgical resection; the cystic parts corresponded to dilated lymphatic vessels. KHE is a rare, locally aggressive, vascular tumor that usually occurs in the soft tissues of the extremities. However, a case occurring in…

Therapeutic targeting of local metabolic dysfunction in secondary lymphedema: mechanisms and opportunities.

Secondary lymphedema is a chronic and progressive disorder resulting from impaired lymphatic drainage after surgery, radiotherapy, infection, trauma, or other lymphatic insults. Its pathological features include chronic inflammation, adipose deposition, fibrosis, and structural and functional damage to lymphatic vessels. Previous studies have mainly explained disease progression from the perspectives of lymphatic injury and tissue remodeling, while local metabolic dysfunction and the associated immune alterations remain insufficiently defined. Recent evidence indicates that lymphedematous tiss…

Vascular endothelial growth factor-a promotes peritumoral lymphangiogenesis and lymphatic metastasis.

Metastases are commonly found in the lymphatic system. The molecular mechanism of lymphatic metastasis is, however, poorly understood. Here we report that vascular endothelial growth factor (VEGF)-A stimulated lymphangiogenesis in vivo and that overexpression of VEGF-A in murine T241 fibrosarcomas induced the growth of peritumoral lymphatic vessels, which occasionally penetrated into the tumor tissue. As a result of peritumoral lymphangiogenesis, metastases in lymph nodes of mice were detected. VEGF-A-overexpressing tumors contained high numbers of infiltrating inflammatory cells such as macro…

Lymphatic and blood vasculature in primary cutaneous melanomas of the scalp and neck.

BACKGROUND: Scalp/neck melanomas have a poor prognosis, possibly because of a rich vascular supply that prompts tumor cells' dissemination. METHODS: We compared the accuracy of immunohistochemical (IHC) staining with morphology for the identification of lymphovascular invasion in 156 scalp/neck melanomas. We then analyzed the association of vessel invasion and density with pathological features and survival. RESULTS: IHC-detected lymphatic vessel invasion (LVI) and blood vessel invasion (BVI) were identified in 34.6% and 13.5% of cases, respectively. IHC increased the LVI/BVI detection compa

Angiomatosis of the breast: a clinicopathological and immunophenotypical characterisation of seven cases.

AIMS: Mammary angiomatosis is a rare, benign vascular lesion that morphologically mimics low-grade angiosarcoma (LGAS). To date, only occasional reports of this entity have been published, none of which included analysis by immunohistochemistry. The purpose of this study was to further characterise mammary angiomatosis by clinical, histological, and immunohistochemical means while emphasising distinguishing features from LGAS. METHODS: Seven cases of primary mammary angiomatosis were evaluated. For one patient, a subsequent recurrence was also evaluated. RESULTS: All patients were female wit
