Evaluation of accuracy and sensory outcomes of mandibular reconstruction using computer-assisted surgical simulation.

PURPOSE: To introduce a modified protocol for mandibular reconstruction and evaluate the protocol using a standardized assessment method. METHOD: This retrospective study involved a case series of nine patients who underwent mandibular reconstruction between 2015 and 2017. The modular protocol comprised three novel modifications in terms of computer-assisted surgical simulation (CASS); surgical template (ST), and surgical procedure. The standardized postoperative evaluation consisted of operation time, part comparison analysis (PCA), facial symmetry, and mechanical quantitative sensory testin

Assessment of perineural infiltration and spread of oral squamous cell carcinoma: a clinicohistopathologic study.

BACKGROUND: As the tumor spreads through the pathway of least resistance, the present study was carried out to evaluate the presence of perineural infiltration and spread of oral squamous cell carcinoma (OSCC) along the perineural spaces in gingivobuccal sulcus tumors infiltrating into the mandible. AIMS AND OBJECTIVES: (1) To investigate the incidence of perineural invasion of OSCC along the inferior alveolar nerve and (2) to investigate the neurovascular bundle as a potential route of spread of OSCC. MATERIALS AND METHODS: Twenty-six patients with histopathologically proven OSCC of the gin

Phosphaturic Mesenchymal Tumor: 2 New Oral Cases and Review of 53 Cases in the Head and Neck.

Phosphaturic mesenchymal tumor (PMT) is a rare neoplasm that secretes fibroblast growth factor-23 (FGF-23) and causes oncogenic osteomalacia. It occurs in adults with equal gender distribution and the most common location is the lower extremities, followed by the head and neck. Besides osteomalacia, the clinical presentation includes bone pain and multiple bone fractures. Microscopic features consist of spindle cells, multinucleated giant cells, and calcifications embedded in a chondromyxoid matrix. Laboratory findings indicate normal calcium and parathyroid levels, hypophosphatemia, and incre

Comparison between patient-specific implants and hand-bent plates in mandibular reconstruction.

OBJECTIVES: Alloplastic reconstruction is often required following segmental mandibular resection (SMR). Hardware-associated complications, especially hardware fractures, almost invariably necessitate explantation. Recent advances in manufacturing techniques promise potential advantages over traditional methods, including increased fracture resistance. However, to date, there is no scientific evidence confirming whether these benefits translate to real-life clinical conditions. MATERIALS AND METHODS: A total of 210 patients underwent SMR followed by alloplastic reconstruction between 2007 and

Hyperbaric Oxygen Effects on Surgical Outcomes in Mandibular Osteoradionecrosis Requiring Segmental Resection and Free Tissue Transfer.

BACKGROUND: Treatment of head and neck cancer with radiation can cause osteoradionecrosis of the mandible (MORN), requiring removal of nonviable bone and free flap reconstruction. OBJECTIVE: To compare surgical outcomes in patients with MORN treated with free tissue transfer who received hyperbaric oxygen therapy (HBO) to those who did not, measured by flap failure, nonunion, plate exposure, and postoperative infections. METHODS: This retrospective cohort study evaluated 311 patients with advanced MORN treated at a single tertiary care academic center with segmental mandibulectomy and free f

Osteoma of mandibular condyle as cause of acute pain and limited-mouth-opening: case report.

In benign tumors in the mandibular condyle such as osteoma and osteochondroma, symptoms such as pain and limited-mouth-opening are rarely observed. Therefore, these tumors are often detected after the development of changes in occlusion and mandibular midline deviation. We encountered a very rare patient with mandibular condyle osteoma who showed acute pain and markedly limited-mouth-opening.

A new concept for mandible reconstruction after oncological resection: Multisegment virtual surgical planning.

PURPOSE: Virtual surgical planning (VSP) is good for three dimensional reconstructions in maxillofacial surgery, but it is not problem-free completely especially when the resection margins cannot be affirmed in preoperative period. We aimed to obtain an ideal reconstruction with elaborating VSP to be prepared for adverse conditions during surgery and to proceed the oncological resections step- by- step with A, B, and C resection planes. PATIENTS AND METHODS: Four patients undergoing multisegment VSP for the primary mandible malignancies were included in the study. The first resection margin w

Radicular cyst masquerading as a multilocular radiolucency.

Radicular cysts are the most common cystic lesions of the jaw. Most of these lesions involve the apex of the offending tooth and appear as well-defined periapical radiolucencies. This case presents an unusually large multilocular radicular cyst crossing the midline and involving almost the entire body of the mandible. The clinical and radiographic appearance mimicked an aggressive cyst or benign tumor. The lesion was surgically excised, and the teeth were endodontically treated without any postoperative complications.

Post-surgical unilateral temporomandibular joint dislocation treated by open reduction followed by orthodontic treatment.

A case of prolonged unilateral temporomandibular joint (TMJ) dislocation, which was treated by open surgical reduction and post-surgical orthodontic therapy, is presented. A 58-year-old woman presented complaining of facial asymmetry and malocclusion. She had received surgery for a malignant tumour in the right retromolar region 7 years previously. It was considered that contraction of the pterygoid muscle by surgical injury caused anterior meniscal displacement and TMJ dislocation. Since manual manipulation failed, direct open reduction was performed after separation of the lateral pterygoid

Mucus escape reaction that involves the mandible: a case report and diagnostic imaging considerations.

We present a case of mucus escape reaction in which we encountered difficulty in interpreting the images acquired by plain radiography, computed tomography, and magnetic resonance imaging. The 48-year-old male Japanese patient was referred for evaluation of a gradual swelling subjacent to the inferior border of the left mandible. At the early imaging examinations, magnetic resonance imaging provided information crucial to resolving the issue of whether the lesion consisted of a central malignant disease process or a malignant disease in the submandibular space or both of these two separate dis

Dynamic Navigation with Real-Time Instrument Visualization for Intraoral Resection of Condylar Osteoma Using a Piezoelectric Osteotome.

The objective of this protocol is to describe a dynamic navigation technique that enables direct real-time visualization of a piezoelectric osteotome during intraoral resection of condylar osteoma. Conventional navigation workflows typically rely on a handheld probe for anatomical localization, followed by a switch to the operative instrument, which may introduce spatial error in confined intraoral surgical fields. To address this limitation, the present method integrates the piezoelectric osteotome directly into the navigation coordinate system through reverse modeling, tool calibration, and

Exploring ameloblastic carcinoma with extensive squamous differentiation: Two case reports and literature insights into clinicopathological significance.

Ameloblastic carcinoma (AC) is a rare malignant odontogenic tumor that combines the architectural features of ameloblastoma with cytological signs of malignancy. Among its histological variations, squamous differentiation is occasionally observed. In this study, we report two mandibular cases of AC, both showing extensive areas of squamous differentiation, and review the literature to explore the potential significance of this feature. The first case involved a 44-year-old male with a multilocular lesion in the mandibular ramus, and the second, a 28-year-old female with bilateral mandibular in

[A Case of Primary Intraosseous Carcinoma Arising in the Odontogenic Cyst of the Mandible].

BACKGROUND: Primary intraosseous carcinoma(IOC)is a rare malignancy originating from the epithelial lining of odontogenic cysts. We present a case of IOC arising from an odontogenic cyst in the mandible. CASE: A 51-year-old man presented to a general hospital with limited mouth opening. Initial anti-inflammatory treatment was administered for a presumed secondary infected mandibular odontogenic cyst;however, symptoms persisted. Subsequent biopsy revealed squamous cell carcinoma, prompting referred to our department for multidisciplinary management. Various examinations were performed, and the

Large Complex Odontoma in the Angulus Mandibulae - Intraoral Enucleation as an Alternative to Mandibular Continuity Resection.

BACKGROUND: Odontomas are among the most common odontogenic tumors and are generally considered as hamartomatous odontogenic lesions. These tumors can be histopathologically divided into complex odontomas and compound odontomas based on their composition. Odontomas show a slow growing behavior and typically lack characteristic symptoms. The standard surgical treatment for large odontogenic tumors is a mandibular (continuity) resection followed by primary or secondary plastic reconstruction. CASE REPORT: A 22-year-old male presented to the Department of maxillofacial surgery with an increasing

Delayed Mandibular Hematoma Following Selective Androgen Receptor Modulator (SARM) Usage.

Selective androgen receptor modulators (SARMs) have been used for various conditions since the late 1990s. Functioning similarly to testosterone, they are used to improve sexual function, skeletal muscle mass, and bone mass, and exhibit other favorable physiological effects. While SARMs are associated with side effects varying from edema to polycythemia, the biggest concern is their lack of regulation, as they are not FDA approved. Recent findings suggest the use of SARMs for reducing breast cancer tumor growth and is being explored for its effects in AR+/HER2-/ER+ advanced breast cancer devel
