Two profoundly hard-of-hearing and deaf patients were examined by non-invasive extracranial magnetic stimulation (EMS) in an effort to determine whether EMS could evoke auditory sensations. The patients were fitted with standard earplugs and were stimulated at the auricle, the mastoid and the temporal lobe area. The threshold of auditory sensation (TAS) was determined at each stimulus position and found to be approximately 20-40% of the maximum EMS level (2.0 Tesla). The TAS was generally lowest in mastoid stimulation, but was variable, and dependent on the angle and position of the stimulatin…

In a series of 360 patients who underwent cochlear implantation at our center, four patients (five procedures) had cochlear implantation with obliteration of the mastoid cavity and management of cholesteatoma as a single-staged procedure. Three patients were bilaterally deaf secondary to CSOM and had bilateral mastoid cavities, and in one patient congenital cholesteatoma was identified during cochlear implantation. A mastoidectomy or revision mastoidectomy with obliteration of the mastoid cavity and cochlear implantation was performed as a single stage procedure. Cholesteatoma reoccurred in on…

Retrolabyrinthine vestibular neurectomy preserves hearing and relieves intractable vertigo emanating from the inner ear. However, this approach must be modified in patients with prior "canal-wall-down" procedures. Traversing an exteriorized mastoid cavity risks bacterial contamination of the subarachnoid space. Three patients seen at the Otologic Medical Group with prior canal-wall-down procedures required vestibular neurectomy for persistent vertigo. Using the retrosigmoid approach, the vestibular nerve was sectioned without sacrificing hearing and without traversing a potentially infected ma…

Simple and reliable repair techniques are described, selected for their simplicity, rapidity and the quality of the results obtained. For losses of substance of the helix, reduction of the pavillion (Antia) or a flat retro-auricular flap suffice for a limited loss, whilst miniature tubular flaps give a good result in cases of total loss. For the concha and the lobule, flat mastoid flaps (Rao-Preaux) are very simple to carry out. Following amputation, a broad, long Crikelair mastoid flap and Converse's tunnelisation procedure may be used to reconstruct the upper 2/3 and mastoid flaps (Preaux-Br…

Coccidioidomycosis involving the ear, mastoid bone, or both is uncommon. We describe 5 new cases from the United States and review 4 cases reported in the literature of otomycosis and mastoiditis caused by Coccidioides. Of the 9 cases, 8 were linked to residence in or travel to California. Two patients had poorly controlled diabetes mellitus, 7 had otomastoiditis, 1 had otitis externa without mastoid involvement, and 1 had mastoiditis without otic involvement. Four patients had concurrent or prior pulmonary coccidioidomycosis. Ipsilateral facial nerve palsies developed in 2 patients. All patie…

We describe the case of a 52-year-old man with a history of squamous cell carcinoma (SCC) of the tonsil who presented with right subacute otalgia and otorrhea. Dedicated computed tomography of the temporal bones showed opacification within the mastoid process with destruction of bony mastoid septations consistent with coalescent mastoiditis. Preoperative imaging showed no destruction or expansion of the bony eustachian tube that would indicate that a direct spread had occurred. An urgent cortical mastoidectomy was performed. Intraoperatively, a friable white mass surrounded with purulence and …

CONCLUSIONS: While infants under the age of 1 year exhibited a high rate of abnormal intensities in the middle ear and mastoid cavities, older infants showed no abnormal intensities in these regions. From the signal intensity on T1- and T2-weighted MRI, the abnormal intensities in the middle ear cavity were considered to represent liquid effusion. Taken together with the findings of temporal bone CT, the abnormal intensities in the mastoid cavity were considered to represent bone marrow.
OBJECTIVE: Histopathological studies of the temporal bone and tympanometry investigations have reported th…

OBJECTIVE: Various techniques have been described in the literature for prominent ear correction. These cartilage-preserving or cartilage-shaping techniques have their own advantages and disadvantages. We aim to achieve aesthetic and stable results with low complication rates using combinations of these methods. Herein, we present our results of prominent ear surgery with a modified bilateral fasciaperichondrial flap in combination with concha-mastoid and concha-scaphal sutures.
METHODS: Patients whose surgeries included a modified bilateral fasciaperichondrial flap for prominent ear deformit…

We describe the case of a child who, at her first episode of clinically evident acute otitis media, has developed a bilateral mastoiditis, though with unilateral simptomatology. The mastoiditis was complicated by the spontaneous drainage of the postauricular abscess in the subcutaneous tissue. According to the literature, we believe that the temporal bone computed tomography scan is the fundamental examination to properly define an anusual case of mastoiditis, plan adequate therapy (medical o surgical), and rule out other possible complications.

A retrospective review of 200 cases of aural polypectomy performed in Glasgow and London was assessed in previously unoperated ears and in those with mastoid cavities. In those with mastoid cavities 72% of the ears became dry whilst only 33% were dry in the previously unoperated group. Cholesteatoma was found in 33% of previously unoperated ears and in 71% with an attic or posterior marginal defect. It can be concluded from these results that aural polypectomy in the presence of a mastoid cavity is a satisfactory procedure but in a previously unoperated ear a more radical procedure should be c…

The use of a rotation pedicled flap, entitled the 'Singapore Swing', comprising temporalis fascia and mastoid periosteum, is presented as a new method of promoting healing in 'open' mastoid surgery for chronic ear discharge. Cadaver studies show that the main blood supply of the flap is derived from a branch of the postauricular artery which enters the pedicle near the mastoid tip. A review of the first 14 operations showed complete healing with dry ears and intact tympanic membranes in all instances, three cases requiring secondary grafting of residual perforations. We consider the results su…

In children, acute mastoiditis is a classic but rare complication of otitis media. Primary treatment of mastoiditis is mainly medical with intravenous antibiotics. Surgical procedures may be necessary for complicated forms of the disease. Though, the adequacy of the initial empiric antibiotic therapy with the susceptibility of the suspected bacteria germs is essential to avoid potentially serious complications. We perform a review of the literature to provide supportive arguments for the therapeutic approach of acute mastoiditis in children worked out by the French Group for Pædiatric Infectio…

OBJECTIVES: Tympanic membrane perforations are common, but there have been few studies of the factors determining the extent of the resulting conductive hearing loss. The aims of this study were to determine whether the size of tympanic membrane perforation, pneumatization of middle ear & mastoid cavity, and location of perforation were correlated with air-bone gap (ABG) of patients.
METHODS: Forty-two patients who underwent tympanoplasty type I or myringoplasty were included and preoperative audiometry were analyzed. Digital image processing was applied in computed tomography for the estimat…
