Endoscopic Transorbital Approach for the Management of Spheno-Orbital Meningiomas: Literature Review and Preliminary Experience.

OBJECTIVE: The endoscopic transorbital approach (ETOA) is a minimally invasive approach that could be particularly appropriate for managemen … for ophthalmoplegia. The most common postoperative complications were transient ophthalmoplegia and maxillary nerve hypoesthesia. Cerebrospinal fluid leak was reported in 2 patients. CONCLUSIONS: Our findings support the use of the ETOA for management of spheno-orbital meningiomas, particularly in at least 3 clinical scenarios: 1) when predominant hyperostotic b …

Role of Transarterial Embolization in the Treatment of Life-Threatening Hemorrhage in Patients With Maxillofacial Injury.

Life-threatening hemorrhage following maxillofacial injury (MFI) is rare but can be fatal. Conventional measures for hemostasis including nasal packing, balloon tamponade, and surgical ligation of bleeding points may not be effective or efficient in patients at risk of hypovolemic shock. Advantages of transarterial embolization (TAE) include rapid identification of the bleeding focus and its access, direct obstruction of the culprit vessels, ability to control multiple bleeding sites, and no requirement of general anesthesia. The internal maxillary artery is the most frequently targeted vessel

Evaluation of Stress and Strain Magnitude in Tapered and Cylindrical Implant Surrounding Bone, Used for Restoration of Mandibular Teeth: Finite Element Analysis in All-on-Four Concept.

The prevalence of the elderly population increased in the 20th century, as described in the World Health Organization 2004 Annual Report. The use of tilted implants parallel to the anterior wall of the maxillary sinus or the mental foramen/inferior alveolar nerve has been proposed for the treatment of the atrophic edentulous ridge. The aim of this study was to evaluate stress and strain magnitude in tapered and cylindrical surrounding bones. A 3D finite element model of an edentulous mandible was constructed. Two models of implants were used: cylindrical and tapered BEGO Semados RI Implants. F

Neuralgia-inducing cavitational osteonecrosis - Fact or myth, the debate persists.

Neuralgia-inducing cavitational osteonecrosis (NICO) is a debated condition characterized by cavitary lesions in the maxillary-mandibular region, often missed on conventional radiographs, and the golden standard for diagnostic measures is bone scintigraphy. It may arise secondary to trauma, such as dental extraction and endodontic treatment, and due to a low-grade chronic infection. NICO has been documented as a frequent cause of face pain involving the trigeminal nerve divisions. It may be severe, piercing pain, of short duration or even continuous pain of moderate intensity. It affects femal

The lymphatic vascular system of the mouse head.

Histological studies of the lymphatic vascular system in adult mice are hampered because bones cannot be sectioned properly. Here, we decalcified the heads of 14-day-old mice, embedded them in paraffin and stained resultant serial sections with the lymphendothelial-specific antibodies Lyve-1 and Podoplanin. We show that the tissues with the highest lymphatic vascular density are the dermis and the oral mucous membranes. In contrast, the nasal mucous membrane is devoid of lymphatics, except for its most basal parts below the vomeronasal organ. The inferior nasal turbinate contains numerous lymp

Endoscopic Transoral Resection of Parapharyngeal Osteoma: A Case Report.

Osteoma is a benign, mesenchymal, slow-growing, osteogenic tumor commonly occurring in the craniofacial bones and is characterized by the proliferation of compact or cancellous bone. Solitary osteomas can be classified as peripheral (parosteal, periosteal, or exophytic) when arising from the periosteum or central (endosteal) when arising from soft tissue. Peripheral osteoma occurs most frequently in the paranasal sinuses. Other locations include the orbital wall, temporal bone, pterygoid processes, external ear canal, and, rarely, the mandible. Osteomas in the oromandibular region often appear

Sites of attachment of Schneiderian papilloma: a retrospective analysis.

BACKGROUND: Inverted papilloma is a benign sinonasal tumor that arises from a localized attachment site. These attachment sites, however, have not been fully characterized in the literature. The objective of this research was to determine the specific distribution of attachment sites in patients with sinonasal papilloma. METHODS: Retrospective analysis of adult patients with sinonasal papilloma presenting to the University of Virginia between 1999 and 2010. These patients were reviewed and compared to historical controls with regard to histologic subtype and pedicle attachment site. RESULTS:

Thiel embalming technique: a valuable method for teaching oral surgery and implantology.

BACKGROUND: Because of its high requirements on surgical experience and the need of complete understanding of the anatomy, oral surgery and especially implantology belong to the most demanding procedures in dentistry. Therefore, hands-on courses for oral surgery and implantology are considered a prerequisite to prepare for clinical practice. To achieve teaching conditions as realistic as possible, we used a novel human cadaver embalming method enabling tissue dissection comparable with the living body. METHODS: Thirty cadavers which were offered by the Institute of Anatomy for the purpose of

Infratemporal hydatid cyst: a case presenting with blindness.

OBJECTIVE: We report a very rare case of a hydatid cyst in the infratemporal fossa, causing visual loss over a 10-day period, which disappeared with rapid surgical and medical treatment. CASE REPORT: A 14-year-old girl presented with right exophthalmos and visual loss. Over a 10-day period, her visual acuity had decreased to detection of hand motion only, due to pressure on the optic nerve caused by a parapharyngeal cyst pressing through a inferior orbital fissure on the right side. A craniotomy had previously been performed for a right frontoparietal hydatid cyst. The patient had been treate

Taste bud development in the channel catfish.

Taste bud formation in channel catfish is first seen to occur in stage 39 embryos, when taste bud primordia (stage 1), consisting of three to five cells, including a single calretinin-positive cell, can be recognized within the oropharyngeal cavity and maxillary barbels. Within a short time (stage 40), stage 2 taste bud primordia are apparent and include two or three calretinin-positive cells. The number of calretinin-positive cells continues to increase (stage 3), and the primordia begin to erupt as mature taste buds (stage 4) by embryonic stage 48. This same pattern of taste bud development

Endoscopically controlled surgical decompression of the orbit in malignant ophthalmopathy.

From May 1988, an endoscopically controlled endonasal orbital decompression was performed in 17 patients with malignant ophthalmopathy. Indications were exclusively acute loss of visual acuity or visual field defects, when medical and radiation therapy had failed. The new minimal invasive technique proceeds in three steps. First, an endonasal, endoscopically controlled ethmoidectomy with resection of the middle turbinate is done and the medial wall of the maxillary sinus is widely opened. Second, the medial and inferior orbital walls are removed, preserving the infraorbital nerve. In the last

[Midfacial degloving: an alternative approach to the frontobasal area, the nasal cavity and the paranasal sinuses].

Midfacial degloving can be characterized as an alternative surgical approach for exposing the bony structures of the midface. In combination with transient partial osteotomies the nasal cavities, the paranasal sinuses, the pterygopalatine fossa and the posterior parts of the anterior skull base are easily accessible. Using an intercartilaginous, a transseptal and a circumvestibular incision in the nose and a vestibular incision in the oral cavity the soft tissues of the upper face are mobilized and transposed cranially up to the infraorbital rim, the nasion and the lacrimal sac. Thus one can a

High-resolution CT analysis of facial struts in trauma: 2. Osseous and soft-tissue complications.

In six cadavers, high-resolution thin-section computed tomography (CT) was used to evaluate the sequelae of experimentally produced facial trauma. As confirmed by pluridirectional tomography, CT was an effective imaging method for the detection and classification of facial fractures. The ability of CT to simultaneously depict both osseous and soft-tissue structures expands the role that diagnostic radiology can play in the evaluation of the traumatized face. A method of evaluation is presented in which the face is geometrically conceptualized as a series of triplanar (horizontal, sagittal, and

Markerless augmented reality-guided mini-screw placement: A clinical case series.

INTRODUCTION: Placing mini-screws between teeth can risk contacting or damaging dental roots and injuring adjacent nerves, blood vessels or maxillary sinus when local anatomy is not visualised precisely. The latest digital advances in orthodontics have enabled three-dimensional (3D) virtual protocol designing, allowing orthodontists to simulate the orthodontic treatments and collaboratively map out the surgical intervention before procedures. Computed tomography (CT)-based surgical guides are strongly recommended for patients with delicate and complex anatomical conditions. CASE PRESENTATIONS

Neurovascular territories of the canine fossa: Analysis of a South African population.

BACKGROUND: The canine fossa, a depression on the surface of the maxillary bone, is important clinically due to the nexus of the neurovascular elements which occur in this region and supply the superficial and deep structures of the face. While it is known that there is much variation in the neurovascular structures of this region, little is known about sex differences. The aim of this study was to investigate and map the neurovascular branching within the region of the canine fossa of a South African population, with particular reference to any sex differences. METHODS: Sixty hemifaces (n=30

Lip Reconstruction by Double Layer and Double Flap New Combination Technique: A Case Series.

In the past, lip reconstruction after ablative surgery has been performed by primary closure and more recently by free flap transfer technique. Cheek's skin flap has been used to reconstruct the lower lip cutaneous portion. This study presents a reconstructive method for the vermillion and the lip's cutaneous portion using the Goldstein-Robotti techniques (for the vermillion) and the buccinator flap to reconstruct the cutaneous lip portion and the perioral muscles. This procedure allows a complete reconstruction with a double layer technique for defects of more than one-third of both lips, tog

Auto-stop Drilling Device for Implant Site Preparation: In Vitro Test of Eccentric Sensor Position.

PURPOSE: To quantify the reliability of a new drilling system for implant osteotomy characterized by an eccentric sensor that automatically stops the drill upon contact with soft tissue. This safety mechanism aims to minimize surgical trauma to nerves, vessels, and the maxillary sinus mucosa. The benefits of the eccentric sensor position on planar and angulated surfaces were tested in vitro. MATERIALS AND METHODS: Predicted drill protrusion after auto-stop was validated against experiments on four human cadaver mandibles (30 osteotomies with varying angles). Measurement of the drill's exit ho
