Clinical Outcomes and Medical Burdens of Neonatal Arrhythmias in Children's Hospitals in China: A Protocol for Multi-Center Retrospective Cohort Study.

Neonatal arrhythmias are significant contributors to infant mortality. Timely diagnosis and treatment are essential for neonates with non-benign arrhythmias to avoid severe complications, and ongoing treatment and follow-up are sometimes needed. The main objective of this study will be to understand the incidence and demographic characteristics of arrhythmias in hospitalized neonates in China and the related factors of outcomes. A secondary objective will be to establish the first follow-up system for neonatal arrhythmias in China. The medical burdens of neonatal arrhythmias in China will also

The Cost of Nurse-Midwifery Care: Use of Interventions, Resources, and Associated Costs in the Hospital Setting.

INTRODUCTION: Obstetrical care often involves multiple expensive, and often elective, interventions that may increase costs to patients, payers, and the health care system with little effect on patient outcomes. The objectives of this study were to examine the following hospital related outcomes: 1) use of labor and birth interventions, 2) inpatient duration of stay, and 3) total direct health care costs for patients attended by a certified nurse-midwife (CNM) compared with those attended by an obstetrician-gynecologist (OB-GYN), within an environment of safe and high-quality care. Patterns of inpatient care for newly diagnosed immune thrombocytopenia in US children's hospitals.

OBJECTIVE: Although recent evidence-based guidelines for the management of immune thrombocytopenia (ITP) recommend a conservative, observation-based approach for the majority of patients with newly diagnosed pediatric ITP, current practice patterns are unknown. This study used the Pediatric Health Information System database to examine patterns of inpatient care in newly diagnosed ITP in freestanding US children's hospitals and to examine geographic differences in care. METHODS: Data were extracted from Pediatric Health Information System for all newly diagnosed ITP admissions aged 1 to 18 ye

Medications associated with clinical deterioration in hospitalized children.

BACKGROUND: Medical emergency teams have been shown to reduce mortality in children's hospitals, but there are many potential barriers to their activation. Surveillance tools using electronic health record data help identify children at risk of deterioration. Existing early warning scores primarily include vital signs, but may benefit from the incorporation of medications. OBJECTIVE: We aimed to identify the therapeutic classes of medications temporally associated with clinical deterioration that could be incorporated with vital signs into surveillance tools. DESIGN: Case-crossover study. S

Measles-mumps-rubella vaccination and asthma-like disease in early childhood.

The authors evaluated the association between receipt of measles-mumps-rubella (MMR) vaccine and asthma-like disease in early childhood in a Danish nationwide cohort study (N = 871,234). Two outcomes were included: hospitalizations with asthma diagnoses and use of anti-asthma medications (for a subset of the cohort only). Poisson regression was used to estimate rate ratios according to vaccination status. MMR-vaccinated children were less often hospitalized with an asthma diagnosis (rate ratio (RR) = 0.75, 95% confidence interval (CI): 0.73, 0.78) and used fewer courses of anti-asthma medicati

LCA and economic study on the local oxygen supply in Central Europe during the COVID-19 pandemic.

Medical oxygen is the key to survival for COVID-19 patients. To meet the pandemic-driven oxygen demand spike, local hospitals began searching for a suitable medical oxygen delivery system. Among the studies published on the impact of COVID-19 on a range of aspects, including the global economy and the environment, no study has been conducted on the environmental impact of medical oxygen supply to hospitals under epidemic conditions. In this paper the authors perform a comparative Life Cycle Assessment (LCA) to evaluate the environmental and economic impact of three scenarios (oxygen cylinders,

A Mediation Analysis Examining High Risk, Anticholinergic Medication Use, Delirium, and Dementia After Major Surgery.

INTRODUCTION: Anticholinergic medications are known to cause adverse cognitive effects in community-dwelling older adults and medical inpatients, including dementia. The prevalence with which such medications are prescribed in older adults undergoing major surgery is not well described nor is their mediating relationship with delirium and dementia. We sought to determine the prevalence of high-risk medication use in major surgery patients and their relationship with the subsequent development of dementia. METHODS: This was a retrospective cohort study which used data between January 2013 and

Full subsidy and direct delivery of essential chronic disease medications for low-income populations: A pilot randomized controlled trial.

INTRODUCTION: Care transition interventions focused on medication therapy management have not proven effective among low-income populations. This pilot study assesses a novel intervention approach that directly addresses social determinants of health (SDoH) reported as major barriers to medication adherence post-hospitalization by low-income patients. METHODS: This pilot pragmatic randomized controlled trial enrolled hospitalized adult Medicaid and uninsured patients with ambulatory care sensitive conditions using ≥2 chronic disease medications from a large safety-net hospital in the Mid-Sout

Self-medication among operating room personnel during healthcare crises: a mixed-methods study of prevalence, predictors, and lived experiences.

BACKGROUND: Self-medication among healthcare workers poses serious risks to both personal health and patient safety. Operating room (OR) personnel, exposed to high-acuity care, irregular shifts, and systemic pressures, may be particularly vulnerable, yet comprehensive studies in this population remain limited. METHODS: This explanatory sequential mixed-methods study investigated self-medication among OR personnel during the COVID-19 pandemic. In the quantitative phase, 400 participants completed a validated 15-item questionnaire on self-medication behaviors, individual factors, and organizati

Drug safety in patients with mitochondrial disease: an observational cohort study.

BACKGROUND: Mitochondrial diseases are common inherited neurometabolic disorders and frequently involve the nervous system, yet their multisystem nature often necessitates complex pharmacological management. Many commonly prescribed medications have off-target effects on mitochondrial function, and patients with mitochondrial disease may be particularly vulnerable to such effects due to impaired energy metabolism. However, systematic data on medication safety in this patient group remain scarce. METHODS: In this retrospective, single-centre, cohort-based study at Turku University Hospital (Tu

Medical Response and Factors Leading to Violent Death in the United States Incarcerated Population.

BACKGROUND: This study aims to characterize trauma-associated deaths in the United States prison system. We hypothesize that incarcerated victims are less likely to receive appropriate medical care compared to the non-incarcerated. METHODS: We utilized 2015-2017 National Violent Death Reporting System data. Victims were classified by whether they were seen by emergency medical services, in the emergency room, or hospitalized prior to death, with the latter considered higher levels of care. Propensity score matching was used to compare highest level of care received by incarcerated versus non-
