Hereditary parkinsonism with dementia is caused by mutations in ATP13A2, encoding a lysosomal type 5 P-type ATPase.

Neurodegenerative disorders such as Parkinson and Alzheimer disease cause motor and cognitive dysfunction and belong to a heterogeneous group of common and disabling disorders. Although the complex molecular pathophysiology of neurodegeneration is largely unknown, major advances have been achieved by elucidating the genetic defects underlying mendelian forms of these diseases. This has led to the discovery of common pathophysiological pathways such as enhanced oxidative stress, protein misfolding and aggregation and dysfunction of the ubiquitin-proteasome system. Here, we describe loss-of-func

Megalencephalic leukoencephalopathy with subcortical cysts.

Megalencephalic leukoencephalopathy with subcortical cysts is one of the newly described white-matter disorders for which recognition has been brought about by advances in imaging technology. The essential diagnostic features include megalencephaly noted in infancy, motor disability in the form of spasticity, ataxia, occasional seizures, mild cognitive decline, and slow progression. Magnetic resonance imaging (MRI) shows bilateral extensive white-matter changes with cysts in the temporal regions. Based on the clinical and MRI features, megalencephalic leukoencephalopathy with subcortical cysts

Magnetic resonance imaging findings of Kernohan-Woltman notch in acute subdural hematoma.

OBJECTIVE AND IMPORTANCE: We report the case of a 73-year-old patient who presented a right motor deficit caused by an ipsilateral acute subdural hematoma. A magnetic resonance imaging (MRI) demonstration of Kernohan-Woltman notch phenomenon was obtained. CLINICAL PRESENTATION: The woman sustained a major head injury at home, followed by loss of consciousness. On admission to the emergency room, she was comatose, anisochoric (left > right), and showed a reaction to pain with decerebrating movements of left limbs (Glasgow Coma Scale (GCS) 4/15). A right severe hemiparesis was observed. Cerebra

Role of the pons in hypoxic respiratory depression in the neonatal rat.

The main purpose of this study was to evaluate the role of the pons in hypoxic respiratory depression (HRD) of the neonatal rat. Experiments were conducted using the isolated brainstem-spinal cord preparation of the neonatal rat (1-3 days old). The brainstem was transected at various levels. We found that ablation of the diencephalon decreased respiratory frequency (fR), and conversely, that ablation of the midbrain or pons increased fR. In the preparation with the pons intact (without the midbrain), hypoxia (superfusate PO2 = 56 mmHg) caused strong depression of respiratory activity, which wa

Surgical treatment of Parkinson's disease.

At present, there are three major surgical approaches to Parkinson's disease (PD): (1) Ablative surgery (i.e. pallidotomy, thalamotomy); (2) deep brain stimulation (DBS) of the thalamus, internal globus pallidus (GPi) and subthalamic nucleus (STN); and (3) grafting fetal mesencephalic cells into the striatum. As a result of increasing understanding of the pathophysiology of the basal ganglia and the demonstration of surgical alleviation of experimental parkinsonism, surgery has regained a paramount importance in the management of PD. The aim of pallidotomy and DBS is to reduce the excessive in

[Montirelin hydrate (NS-3), a TRH analog, improved disturbance of consciousness in cats: electroencephalographical studies].

Central effects of montirelin hydrate (NS-3) were electroencephalographically investigated in cats with experimentally induced disturbance of consciousness. All experiments were conducted under the gallamine-immobilized and artificially ventilated acute experimental condition. NS-3 and TRH produced EEG activation in cats with lesions in the midbrain reticular formation in a dose-dependent manner. Similar effects were observed in cats with bilateral lesions of the posterior hypothalamic area. These effects of NS-3 were 30 to 100 times more potent than those of TRH. NS-3 at doses higher than 0.0

[Bilateral intra-axial involvement of the common oculomotor nerve (III): 2 cases].

Bilateral palsy of the common ocular motornerve (III) was observed in two patients with an intra-axial lesion due ti hemorrhage in one and ischemia in the other. The lesions involved the cerebral peduncle in the periaqueductal region and the nuclear complex of the III in the first case. Bilateral infarct of the thalamus was seen in the second. Clinical manifestations were transitory except for the oculomotor impairment. In the first patient, oculomotricity was dissociated as intrinsic mortricity was spared. These exceptional cases demonstrate a syndrome with unique oculomotor expression result

[Claude syndrome caused by mesencephalic infarction: 2 cases].

Paramedian midbrain infarcts limited to the oculomotor nerve fibers are uncommon. We studied 2 cases where the clinical syndrome included a third cranial nerve palsy and a contralateral cerebellar ataxia. The CT scan disclosed a paramedian midbrain tegmental infarct, so that it is possible to term our two cases Claude's syndrome. The oculomotor nerve fascicular palsies were complete in one case and limited to the extraocular muscles in the second case. The selective involvement of oculomotor function suggests intraaxial fascicular organisation of the third cranial nerve in the brainstem.

Paralysis of downgaze in two patients with clinical-radiologic correlation.

Selective downgaze paralysis was correlated with discrete bilateral lesions at the mesencephalic-diencephalic junction in a 9-year-old girl following severe pneumococcal meningitis, and in a 64-year-old man who suffered an embolic infarction. Magnetic resonance imaging demonstrated bilateral lesions in the region of the rostral interstitial nucleus of the medial longitudinal fasciculus. Clinical-radiologic correlation allowed identification of the likely vascular cause in both patients.

Functional relationships between mesencephalic central gray and septum in regulating lordosis in female rats: effect of dual lesions.

In order to clarify the functional relationships between the lateral septum (LS) and the mesencephalic central gray (MCG) in regulating lordosis behavior, ovariectomized female rats received dual lesions in these two areas. In the first experiment, females with unilateral (right or left, R-MCGL or L-MCGL) or bilateral MCG (B-MCG) lesions were subjected to behavioral tests after the implantation of a Silastic tube containing estradiol. Lordosis was observed in only one B-MCGL female. In the R-MCGL and L-MCGL groups, most females displayed lordosis, but lordosis quotients (LQ) were significantly

[Functional state of the endogenous opioid peptide system in traumatic shock in rats].

The experiments have been performed on 120 Wistar rats and 546 CBWH mice. Met-enkephalin (ME) and beta-endorphin (BE) levels were determined to increase in blood and mid-brain 3 hours later after soft tissue compression of pelvic extremities. 48 hours later after six-hour compression BE and ME level in blood was increased, BE concentration in blood was also increased, and enkephalins' content in brain and blood was decreased as against the control. Opioid peptides influence on the host compression period has been studied. D-agonists were established to decrease, but mu-agonists were establishe

Indications for neurosurgical treatment of chronic pain.

Guidelines are presented for the neurosurgical treatment of chronic pain. In these guidelines a distinction is made between the pain of cancer and neurogenic pain. In cancer pain the survival time and the location of the lesion are the important guidelines. Possible procedures are: opioids via CSF route, lesions in nociceptive pathways and PV-PAG stimulation of the thalamus. In neurogenic pain, neurostimulation procedures, tailored to the location of the pain are procedures of first choice. There are however specific indications for other procedures depending on the aetiology of the pain. Caus

The compensatory role of the parvocellular division of the red nucleus in operantly conditioned rats.

To assess the role of the parvocellular division of the red nucleus in motor control, rats were operantly conditioned to walk on a rotating bar and their red nuclei were lesioned with the fiber sparing agent, quinolinic acid. Since both parvocellular and magnocellular divisions of the red nucleus overlap in the rat, they both became involved in this lesion. To differentiate between them, two paradigms were used. (1) Prior to the lesion, the magnocellular division was dysfunctioned by transecting its spinal output, namely, the rubrospinal tract, in the dorso-lateral funiculus (DLF) of the spina

Reversible disruption of lordosis via midbrain infusions of procaine and tetrodotoxin.

Behavioral effects of bilateral intracranial infusions of tetrodotoxin (1, 3.3 or 10 ng/rat), 50% procaine (2 microliters/rat) or phosphate-buffered saline (PBS-2 microliters/rat) into the dorsal midbrain of conscious, lightly-restrained female rats were evaluated. High levels of lordotic responsiveness were induced in ovariectomized animals treated with estradiol (E2) capsules or subcutaneous injections of estradiol benzoate (EB) followed by progesterone (P). The effect of each of the 3 infusates on lordosis was determined using manual stimulation and lordosis quotient determinations. In addi

[Central pontine and extrapontine myelinolysis following rapid correction of hyponatremia--report of an autopsy case].

A case of central pontine myelinolysis (CPM) following rapid correction of hyponatremia was reported and literatures were reviewed. The case was 61-year-old nonalcoholic female who had taken an operation of craniopharyngioma 23 years ago. Fifteen years later, she received re-operation for the recurrent tumor, followed by replacement therapy of corticosteroid and clofibrate. She was otherwise well until two weeks before entry, when she noticed abrupt onset of high grade fever, nausea, vomiting and general malaise. She was admitted to an emergency hospital because of weakness, disorientation and

Progesterone in the ventromedial hypothalamus of ovariectomized, estrogen-primed rats inhibits subsequent facilitation of estrous behavior by systemic progesterone.

The putative neural target sites of progesterone's (P) inhibitory influence on estrous behavior were re-examined utilizing intracranial hormone implants. Subjects were estrogen-primed ovariectomized, Long-Evans rats, and all were outfitted with permanent indwelling guide cannulae aimed for the ventromedial hypothalamus (VMH), preoptic area (POA), or midbrain. In a series of 4 weekly testing paradigms, the ability of blank or P-filled implants to both facilitate estrous responsiveness and to interfere with an animal's ability to exhibit estrous behavior following a systemic injection of P 24 ho
