The Presence of Blood in a Strain Gauge Pressure Transducer Has a Clinical Effect on the Accuracy of Intracranial Pressure Readings.

IMPORTANCE: Patients admitted with cerebral hemorrhage or cerebral edema often undergo external ventricular drain (EVD) placement to monitor and manage intracranial pressure (ICP). A strain gauge transducer accompanies the EVD to convert a pressure signal to an electrical waveform and assign a numeric value to the ICP. OBJECTIVES: This study explored ICP accuracy in the presence of blood and other viscous fluid contaminates in the transducer. DESIGN: Preclinical comparative design study. SETTING: Laboratory setting using two Natus EVDs, two strain gauge transducers, and a sealed pressure ch

Application of portable sleep monitoring devices in pregnancy: a comprehensive review.

Objective. The physiological, hormonal and biomechanical changes during pregnancy may trigger sleep disordered breathing (SDB) in pregnant women. Pregnancy-related sleep disorders may associate with adverse fetal and maternal outcomes including gestational diabetes, preeclampsia, preterm birth and gestational hypertension. Most of the screening and diagnostic studies that explore SDB during pregnancy were based on questionnaires which are inherently limited in providing definitive conclusions. The current gold standard in diagnostics is overnight polysomnography (PSG) involving the comprehensiv

Neurological Monitoring and Management for Adult Extracorporeal Membrane Oxygenation Patients: Extracorporeal Life Support Organization Consensus Guidelines.

BACKGROUND: Critical care of patients on extracorporeal membrane oxygenation (ECMO) with acute brain injury (ABI) is notable for a lack of high-quality clinical evidence. Here, we offer guidelines for neurological care (neurological monitoring and management) of adults during and after ECMO support. METHODS: These guidelines are based on clinical practice consensus recommendations and scientific statements. We convened an international multidisciplinary consensus panel including 30 clinician-scientists with expertise in ECMO from all chapters of the Extracorporeal Life Support Organization (E

Batch Fabrication of Flexible Strain Sensors with High Linearity and Low Hysteresis for Health Monitoring and Motion Detection.

In recent years, flexible strain sensors have gradually come into our lives due to their superiority in the field of biomonitoring. However, these sensors still suffer from poor durability, high hysteresis, and difficulty in calibration, resulting in great hindrance of practical application. Herein, starting with interfacial interaction regulation and structure-induced cracking, flexible strain sensors with high performance are successfully fabricated. In this strategy, dopamine treatment is used to enhance the bonding between flexible substrates and carbon nanotubes (CNT). The combination wit

Dually-crosslinked ionic conductive hydrogels reinforced through biopolymer gellan gum for flexible sensors to monitor human activities.

Human-machine interactions, monitoring of health equipment, and gentle robots all depend considerably on flexible strain sensors. However, making strain sensors have better mechanical behavior and an extensive sensing range remains an urgent difficulty. In this study, poly acrylamide-co-butyl acrylate with gellan gum (poly(AAm-co-BA)@GG) hydrophobic association networks and intermolecular hydrogen bonding interactions are used to fabricate dual cross-linked hydrogels for wearable resistive-type strain sensors. This could be an acceptable way to minimize the limitations in hydrogels previously

iEat: automatic wearable dietary monitoring with bio-impedance sensing.

Diet is an inseparable part of good health, from maintaining a healthy lifestyle for the general population to supporting the treatment of patients suffering from specific diseases. Therefore it is of great significance to be able to monitor people's dietary activity in their daily life remotely. While the traditional practices of self-reporting and retrospective analysis are often unreliable and prone to errors; sensor-based remote diet monitoring is therefore an appealing approach. In this work, we explore an atypical use of bio-impedance by leveraging its unique temporal signal patterns, wh

Review on design of real-time posture monitoring system for the cervical region.

In cervical health, the Posture Monitoring System (PMS) employs sensors to capture and transmit posture data to the cloud via Wi-Fi. This systematic review examines wearable PMS devices for cervical posture, analysing their attributes, findings, and limitations. Using systematic literature analysis, related studies were collected from diverse databases concentrating on wearable cervical posture devices. The review analysed the outcomes of each neck posture and each monitor type on the CVA ratio based on PMS. However, limitations, such as small sample sizes, limited functions, and privacy conce

Accuracy of patient-reported spell duration: A comparative study.

Clinicians rely heavily on patient histories to make medical diagnoses, most of which are inherently subjective and prone to inaccuracies. The aim of this study is to compare the subjective versus objective duration of spells through a retrospective chart review of patients admitted to the epilepsy monitoring unit at our tertiary care medical center. One hundred patients were analyzed. Differences in the accuracy of subjective estimations versus objective duration were compared by age, sex, focal versus generalized, location (frontal versus non-frontal), and spell type (focal aware versus impa

Utility of intracranial pressure monitoring in patients with traumatic brain injuries: a propensity score matching analysis of TQIP data.

PURPOSE: Intracranial pressure monitoring (ICPM) is central to traumatic brain injury (TBI) management, but its utility is controversial. METHODS: The 2016-2017 TQIP database was queried for isolated TBI. Patients with ICPM [(ICPM (+)] were propensity-score matched (PSM) to those without ICPM [ICPM (-)] and divided into three age groups by years (< 18, 18-54, ≥  55). RESULTS: PSM yielded 2125 patients in each group. Patients aged < 18 years had a higher survival probability (p = 0.013) and decreased mortality (p = 0.016) in the ICPM (+) group. Complications were higher and LOS was longer in

Intracranial Pressure Monitoring and Management in Aneurysmal Subarachnoid Hemorrhage.

Aneurysmal subarachnoid hemorrhage is a medical condition that can lead to intracranial hypertension, negatively impacting patients' outcomes. This review article explores the underlying pathophysiology that causes increased intracranial pressure (ICP) during hospitalization. Hydrocephalus, brain swelling, and intracranial hematoma could produce an ICP rise. Although cerebrospinal fluid withdrawal via an external ventricular drain is commonly used, ICP monitoring is not always consistently practiced. Indications for ICP monitoring include neurological deterioration, hydrocephalus, brain swelli

The importance of monitoring cerebral oxygenation in non brain injured patients.

Over the past few years, the use of non-invasive neuromonitoring in non-brain injured patients has increased, as a result of the recognition that many of these patients are at risk of brain injury in a wide number of clinical scenarios and therefore may benefit from its application which allows interventions to prevent injury and improve outcome. Among these, are post cardiac arrest syndrome, sepsis, liver failure, acute respiratory failure, and the perioperative settings where in the absence of a primary brain injury, certain groups of patients have high risk of neurological complications. A Novel Form Factor For PPG-based Blood Pressure Monitoring.

Blood pressure (BP) is one of the four main vital signs in medicine and may be a useful signal for wellness tracking and for user-aware interfaces in human-computer interaction. The current standard for BP measurement uses cuff-based devices that block an artery temporarily to get a single, discrete measurement of BP. Recently, there have been significant efforts to measure correlates of BP continuously and non-invasively from relevant signals like photoplethysmography (PPG), which responds to volumetric changes in arteries due to blood pulsations. In this paper, we explore a novel setup with

Remotely monitored transcranial direct current stimulation in pediatric cerebral palsy: open label trial protocol.

BACKGROUND: Pediatric applications of non-invasive brain stimulation using transcranial direct current stimulation (tDCS) have demonstrated its safety with few adverse events reported. Remotely monitored tDCS, as an adjuvant intervention to rehabilitation, may improve quality of life for children with cerebral palsy (CP) through motor function improvements, reduced treatment costs, and increased access to tDCS therapies. Our group previously evaluated the feasibility of a remotely monitored mock tDCS setup in which families and children successfully demonstrated the ability to follow tDCS inst

Probabilistic prediction of increased intracranial pressure in patients with severe traumatic brain injury.

Traumatic brain injury (TBI) causes alteration in brain functions. Generally, at intensive care units (ICU), intracranial pressure (ICP) is monitored and treated to avoid increases in ICP with associated poor clinical outcome. The aim was to develop a model which could predict future ICP levels of individual patients in the ICU, to warn treating clinicians before secondary injuries occur. A simple and explainable, probabilistic Markov model was developed for the prediction task ICP ≥ 20 mmHg. Predictions were made for 10-min intervals during 60 min, based on preceding hour of ICP. A prediction

Optimizing oxygen delivery to the injured brain.

PURPOSE OF REVIEW: The principle of optimizing oxygen delivery to the injured brain rests on the premise that both hypoxia and hyperoxia are important mediators of secondary brain injury and should be avoided. This rationale has prompted a move towards incorporating oxygenation endpoints into the management of neurocritical care patients, particularly those with traumatic brain injury. The present review will seek to describe clinical strategies to optimize oxygenation in the acutely brain-injured patient, drawing upon relevant physiologic principles and clinical data, where it exists.
