Helicobacter pylori infection and association with chronic diseases: A focus on cardiovascular disease, MASLD, and type 2 diabetes.

Helicobacter pylori (H. pylori) infection is a globally prevalent gastrointestinal pathogen increasingly linked to various extra-gastric non-communicable diseases (NCDs). This review addresses the guiding question: What epidemiological and mechanistic links explain the association between H. pylori infection and chronic conditions such as cardiovascular disease (CVD), metabolic dysfunction-associated steatotic liver disease (MASLD), and type 2 diabetes mellitus (T2DM)? The manuscript synthesizes evidence from epidemiological studies and mechanistic research. In CVD, H. pylori exacerbates chron

The effect of chronic stress on sensitivity to dexamethasone treatment of HPA axis gene expression in C57Bl/6 mice.

Chronic social stress is a major risk for psychopathologies such as depression, often leading to altered hypothalamic-pituitary-adrenal (HPA) axis function and glucocorticoid resistance. This study examines how chronic social defeat stress (CSDS) affects sensitivity to dexamethasone by analyzing HPA axis genes expression in C57Bl/6 mice. Adult male mice were subjected to 30 days of stress, followed by dexamethasone or saline administration. Genes expression was analyzed in the hypothalamus, prefrontal cortex (PFC; Nr3c1 only), and adrenal glands at multiple time points post-treatment. CSDS ind

Geriatric syndromes associated with long-term mortality in adults aged 90 years and older.

This study examines the impact of geriatric syndromes on the mortality of nonagenarians, a group often underrepresented in research. Conducted as a cohort study in hospital, it included nonagenarians who underwent comprehensive geriatric assessments between November 2016 and January 2019. Mortalities were tracked using the National Death Notification System until May 1, 2022. This cohort study included 235 nonagenarians who underwent a comprehensive geriatric assessment, which evaluated multiple geriatric syndromes, functional status, and chronic diseases. Participants were followed up for a m

Polyclonal Hypergammaglobulinemia Associated With Multimorbidity and Malnutrition in an Elderly Man: A Comprehensive Diagnostic Approach.

A 76-year-old man with a history of liver cirrhosis, chronic obstructive pulmonary disease (COPD), secondary adrenal insufficiency, and psychiatric comorbidities presented with severe weight loss and laboratory findings of pancytopenia, hypoalbuminemia, and IgG-predominant hypergammaglobulinemia, raising suspicion for multiple myeloma. However, serum protein electrophoresis, immunofixation, and urinary Bence Jones testing showed no monoclonal protein, and the kappa/lambda (κ/λ) ratio remained normal, indicating polyclonal hypergammaglobulinemia. Further evaluation revealed decompensated liver

Exercise as Medicine for People with a Substance Use Disorder: An ACSM Call to Action Statement.

Over 20 million Americans are living with a substance use disorder (SUD) and nearly 100,000 die annually from drug overdoses, with a majority involving an opioid. Many people with SUD have co-occurring chronic pain and/or a mental health disorder. Exercise is a frontline treatment for chronic pain and is an effective strategy for reducing depression and anxiety and improving overall mental health. Several studies have shown that exercise improves SUD-related outcomes including abstinence; however, there is limited large-scale randomized clinical trial evidence to inform integration of exercise

Challenges in Diagnosis and Therapeutic Approach of Acute on Chronic Liver Failure-A Review of Current Evidence.

Acute-on-chronic liver failure (ACLF) is a syndrome characterized by acute and severe decompensation of chronic liver disease (CLD) correlated with multiple organ failure, poor prognosis, and increased mortality. In 40-50% of ACLF cases, the trigger is not recognized; for many of these patients, bacterial translocation associated with systemic inflammation is thought to be the determining factor; in the other 50% of patients, sepsis, alcohol consumption, and reactivation of chronic viral hepatitis are the most frequently described trigger factors. Other conditions considered precipitating fact

Differential screen and treatment of sternocleidomastoid syndrome versus eagle syndrome: a case report.

BACKGROUND/PURPOSE: Differential screening is a complex process in chronic pain conditions. There is significant uncertainty that surrounds the pathophysiology of many chronic pain syndromes that may lead to misdiagnosis and treatment failures. Such differential screening is even more challenging where there is regional overlapping from surrounding tissues. This case report chronicles the differential screening and treatment of a patient with sternocleidomastoid syndrome (SCMS) originally diagnosed as Eagle's syndrome (ES). CASE DESCRIPTION: A 55-year-old woman, referred to a physical therapi

Establishment of the Diagnostic Signature of Ferroptosis Genes in Multiple Sclerosis.

Ferroptosis is a novel form of membrane-dependent cell death that differs from other cell death modalities such as necrosis, apoptosis, and autophagy. Multiple sclerosis (MS) is a chronic inflammatory disease of the central nervous system primarily affecting brain and spinal cord neurons. Although the pathogenesis of these two conditions may seem unrelated, recent studies have indicated a connection between ferroptosis and multiple sclerosis. In fact, ferroptosis plays a significant role in the development of MS, as evidenced by the presence of elevated iron levels and iron metabolism abnormal

Cannabinoid receptor 2: potential role in immunomodulation and neuroinflammation.

An accumulating body of evidence suggests that endocannabinoids and cannabinoid receptors type 1 and 2 (CB(1), CB(2)) play a significant role in physiologic and pathologic processes, including cognitive and immune functions. While the addictive properties of marijuana, an extract from the Cannabis plant, are well recognized, there is growing appreciation of the therapeutic potential of cannabinoids in multiple pathologic conditions involving chronic inflammation (inflammatory bowel disease, arthritis, autoimmune disorders, multiple sclerosis, HIV-1 infection, stroke, Alzheimer's disease to nam

Religious involvement and seroprevalence of six infectious diseases in US adults.

OBJECTIVE: In the United States, religious practice is inversely associated with several chronic conditions, but no reports show whether it is inversely associated with prevalence of positive serology for infections. METHODS: Data on a multiethnic, national sample included 11,507 persons aged 17 years and over with complete data on frequency of attendance at religious services (FARS) and serologic testing for six pathogens. RESULTS: Even after controlling for multiple confounders, persons attending religious services weekly (19.8%) or more (19.6%) were less likely to be seropositive for herp

Screening and behavioral management: medication overuse headache--the complex case.

The new appendix criteria for a broader concept of chronic migraine from the International Headache Society no longer require headache resolution or return to the previous headache pattern to confirm the diagnosis of medication overuse headache (MOH). MOH can be subdivided into simple (Type I) and complex (Type II). Complex cases may involve long-term use of daily opioids or combination analgesics, multisourcing, multiple psychiatric comorbidities, and/or a history of relapse. Daily use of opioids for other medical conditions, psychiatric comorbidity including borderline personality disorder,

Mechanisms of axon-glial injury of the optic nerve.

The central concept underlying ideas on the pathogenesis of multiple sclerosis is that inflammatory events cause acute injury of axons and myclin. The phases of symptom onset, recovery, persistence, and progression in multiple sclerosis can be summarized as functional impairment with intact structure due to direct effects of inflammatory mediators; demyelination and axonal injury with recovery through plasticity and remyelination; and chronic axonal loss due to failure of enduring remyelination from loss of trophic support for axons normally provided by cells of the oligodendrocyte lineage. Management of locally aggressive sinus disease using craniofacial exposure and the galeal frontalis fascia-muscle flap.

Histologically, benign sinus diseases can behave aggressively, potentially causing sight- and life-threatening conditions requiring surgical treatment. Four patients illustrating acute and chronic infection, single and multiple sinus polyposis, and mucoceles are presented and discussed to illustrate how the principles of craniofacial exposure, resection, and reconstruction may be employed to treat aggressive sinus disease. The bicoronal and buccal sulcus approaches minimize visible scarring; wide exposure of the orbital contents and dura reduce the risk of excessive bleeding, neurologic compli

Chronic Medical Illness as a Risk Factor for Poor Mammography Screening Adherence.

Objective: The aim of this study was to determine the association between the presence of chronic medical disease and mammography screening … screening, which is likely compounded by the comorbidity burden of being simultaneously treated for multiple chronic conditions. Increased health care interactions seen in this group may represent missed opportunities to improve screening adherence.

Bio-sociological and clinical factors of chronic pain and pain interference in patients undergoing hemodialysis: a cross-sectional study.

BACKGROUND: End-stage kidney disease is an irreversible and incurable alteration of kidney function, requiring renal replacement therapy such as hemodialysis. Hemodialysis patients are one of the most symptomatic of all chronic kidney disease groups, including chronic pain. Overlooked, underestimated, and undermanaged, pain is associated with altered health-related quality of life. Therefore, this study aims to explore the biological, clinical, and social variables associated with chronic pain and its interference with hemodialysis patients. METHODS: A cross-sectional study was conducted on M

Chronic inflammation and accelerated atherosclerosis as important cofactors in nephrogenic systemic fibrosis following intravenous gadolinium exposure.

Nephrogenic systemic fibrosis (NSF) is a rare disorder in patients with chronic kidney disease characterized by an increased tissue deposition of collagen. Its pathogenesis remains unclear. Prior studies indirectly suggested a possible impact of chronic inflammation and accelerated atherosclerosis--a common feature in kidney diseased patients--whereas recent data focused almost exclusively on gadolinium (Gd)-based MR contrast agents. Usually NSF develops a maximum of 2-3 months after Gd. Longer intervals have not yet been described. Therefore, we present the first case with an extraordinary lo

Association of multimorbidity trajectories from early adulthood through middle age with middle-age physical function.

BACKGROUND: Chronic conditions can develop early in the adult life course and accumulate at different rates. However, the association between multimorbidity trajectory groups from young adulthood and physical function in midlife has not been well studied. METHODS: Data are from 2018 Coronary Artery Risk Development in Young Adults (CARDIA) study participants who completed a PROMIS Function Short Form and five physical performance tests (gait speed, grip strength, balance, chair stands, 6-minute-walk, composite score range 0-20, higher is better). [Current approaches to supportive care in multiple myeloma].

Myeloma multiplex is a malignant blood disease in which monoclonal expansion of malignant plasma cells occurs, together with hyperproduction of monoclonal protein, as well as impairment of normal haematopoiesis. Specific features of myeloma include bone destruction, renal failure and immunologic deficiency which decreases the overall quality of the patient's life. Thus, prevention and supportive therapy of skeletal disease, anaemia, pain, nausea, infection and hypercalcemia, represent the essential part of therapy in myeloma patients. Improvements achieved in the specific haematological treatm

Does rehabilitation improve work participation in patients with chronic spinal pain after spinal surgery: a systematic review.

OBJECTIVE: Patients with therapy-refractory chronic spinal pain after spinal surgery experience increased disability, resulting in substantial loss of employment and consequently lower quality of life. Despite findings that rehabilitation improves socio-economic outcomes in other chronic pain conditions, evidence for patients with chronic spinal pain after spinal surgery is limited. A systematic review was conducted to provide an overview of rehabilitation interventions and their effectiveness to improve work participation for patients with chronic spinal pain after spinal surgery.
