Clinical-molecular correlation in 104 mild X-linked muscular dystrophy patients: characterization of sub-clinical phenotypes.

A multidisciplinary study was conducted in order to assess dystrophin expression in a large series of mild X-linked muscular dystrophy patients, with well-defined clinical phenotype. Patients (104) were divided in 4 clinical groups, according to clinical severity: asymptomatic (sub-clinical), benign, moderate and severe, Cardiopathy was also assessed, and dilated cardiomyopathy was found in 47% of sub-clinical and benign cases. Myoglobinuria, cramps and myalgia were also associated with a sub-clinical or benign clinical status. Dystrophin immunohistochemical pattern of labelling and dystrophin…

[Toxic myopathy with kidney failure as a colchicine side effect ifn familial Mediterranean fever].

A 24-year-old woman with familial Mediterranean fever (FMF) had for one year been treated with colchicine, 1 mg daily, for repeated bouts of fever, abdominal pain and arthritis. She was also known to have renal amyloidosis. Lately she had developed gastrointestinal symptoms, muscle pain and obvious, predominantly proximal muscular weakness in both legs. The cause of the symptoms was rhabdomyolysis with an increased creatinine activity of 1000 U/l and marked myoglobinuria (1600 micrograms/l), as well as renal failure with normal uric acid and a creatinine clearance of 3 ml/min per 1.73 m2. Seru…

Acute exercise-induced bilateral thigh compartment syndrome.

Acute compartment syndrome of the thigh is rare due to the space's ability to accommodate large volumes of fluid and, with the exception of the lateral septum, its thin compliant linings. This article describes a case of bilateral exercise-induced severe compartment syndrome treated with anterior and posterior fasciotomies. A 29-year-old man was admitted to intensive care with myoglobinuria. His left thigh was evaluated 18 hours later for compartment syndrome. The patient reported that 14 hours prior to initial presentation, he had participated in a 1-hour session of vigorous basketball. He gr…

Rhabdomyolysis and acute myoglobinuric renal failure in a patient with bilateral pheochromocytoma following open pyelolithotomy.

Rhabdomyolysis is an unusual manifestation of pheochromocytoma. Early diagnosis and prompt management are crucial, as it may have life-threatening consequences. This is the case of a 19-year-old man with bilateral pheochromocytoma complicated with rhabdomyolysis and acute myoglobinuric renal failure after surgery for nephrolithiasis. A massive catecholamine release during the procedure manifested itself as a hypertensive crisis, producing severe vasoconstriction and thereby provoking ischemia of the patient's muscle tissue. This insult resulted in rhabdomyolysis and acute myoglobinuric renal f

Rhabdomyolysis in 119 students after repetitive exercise.

Exercise induced rhabdomyolysis is well known, but has rarely been reported in high school students. This is the report of 119 cases in high school students who exercised vigorously (120 push ups in five minutes) in cold weather. Most of them developed muscle pain and dark urine within two to four days of the exercise.

Acute toxic renal failure.

Acute renal failure (ARF) is a common problem in intensive care medicine. Even modest degrees of ARF not requiring dialysis treatment increase the risk of death approximately fivefold. Despite the widespread appreciation of the role of nephrotoxic drugs in their contribution to ARF, these drugs continue to have an ongoing aetiological role. Potentially nephrotoxic drugs include non-steroidal anti-inflammatory drugs, radiocontrast agents, antimicrobial and anaesthetic agents. Endogenous compounds such as myoglobin and haemoglobin may furthermore cause toxic nephropathy. Tubular injury initiated

Reversible myoglobinuric renal failure following rhabdomyolysis as a rare complication of cardioversion.

Reversible myoglobinuric renal failure following rhabdomyolysis that was related to repeated countershocks delivered for the treatment of refractory recurrent VT and VF attacks during acute myocardial infarction is presented in this case report, in which scan with technetium-99m pyrophosphate has been used for in the diagnosis of extensive skeletal muscle damage.

Protective effect of a bioflavonoid proanthocyanidin-BP1 in glycerol-induced acute renal failure in the rat: renal stereological study.

Renal ischemia as well as oxygen metabolites play an important role in renal injury during myoglobinuric acute renal failure (ARF). On the other hand, flavonoids, a diverse group of constituents naturally occurring in plants, have a strong antioxidative activity, and have been implicated in vascular relaxation. In this study the protective effect of a new bioflavonoid proanthocyanidin-BP1 (BP1), extracted from seeds of grapes, was evaluated in glycerol-induced ARF in rats. Stereological methods were used to quantify changes in renal morphology associated with ARF. Volume density of tubular lum

Cocaine-associated acute myoglobinuric renal failure.

PURPOSE: Abuse of cocaine is associated with serious medical complications involving the heart, central nervous system, and gastrointestinal tract. Renal complications appear to be uncommon. We describe herein four patients with rhabdomyolysis and acute myoglobinuric renal failure temporally related to cocaine use. CASE REPORTS: Acute cocaine intoxication was the most common presentation and rhabdomyolysis was an unexpected finding. Renal failure progressed rapidly in all patients, necessitating dialysis in two. The prognosis was good, and all patients survived. The mechanism of cocaine injur

Muscle damage and endurance events.

Intensive training for and competition in endurance events like the marathon are accompanied by injury to fibres in the active skeletal muscles. Evidence for the injury comes from the increases in intramuscular enzymes and myoglobin found in the blood following the exercise, from the subjective sensation of soreness in the muscles in the post-exercise period, and from direct histological examination of samples of the damaged muscles. Histological studies demonstrate that some muscle fibres undergo degenerative changes following the exercise; the necrosis is accomplished by macrophages and othe

Demonstration of myoglobin in formalin-fixed renal sections by immunoperoxidase technic.

The use of an immunohistochemical technic offers a very sensitive method to detect myoglobin in formalin-fixed, paraffin-embedded tissues. Thus, the technic is suitable for retrospective studies. Myoglobin can be demonstrated at the brush border and apical cytoplasm of the tubular epithelium of proximal tubules. Granular substances and casts in the lumens also react positively with antimyoglobin. Our findings confirm the results of a previous animal study, showing that myoglobin is filtered through glomeruli and reabsorbed by the proximal tubules.

Renin release and the refractoriness to acute renal failure of rats recovering from prior renal failure.

Renin substrate concentrations and the release of renin in response to the rapid removal of 15 ml/kg weight blood or subcutaneous administration of 500 microgram/kg body weight isoproterenol were measured in control rats and animals recovering from prior myohemoglobinuric acute renal failure (recovery rats). Blood for these assayas was drawn from chronically implanted aortic catheters, obviating the need for animal handling and anesthesia. Baseline plasma renin titers of recovery rats were modestly elevated but renin substrate concentrations were the same as in controls. Hemorrhage produced eq

Rhabdomyolysis with cardiac and respiratory involvement.

A case of acute nontraumatic rhabdomyolysis in a heroin addict is described. In addition to respiratory failure, the patient's course was complicated by life-threatening arrhythmias and electrocardiographic changes compatible with primary cardiac involvement. This case suggests that rhabdomyolysis may involve cardiac muscle on rare occasions.
