[Comparative analysis of clinical and neurophysiological indices in patients with multiple sclerosis and acute demyelinating polyneuropathy].

A comparison study of some clinical and neuropsychological indices in two demyelinating disorders--multiple sclerosis (17 patients, mean age 31.6 years) and inflammatory demyelinating polyneuropathy (7 patients, mean age 49.5 years) was conducted, using electromyography with registration of impulse velocity conduction along the nerve and M-responses as well as of the parameters of movement units potentials. The data obtained suggest the presence of demyelinating lesions of central and peripheral nervous systems, namely, symptoms of axonal demyelinating polyneuropathy with the prevalence of dem

Diagnosis of carpal tunnel syndrome: electrodiagnostic and MR imaging evaluation.

In clinically classic carpal tunnel syndrome (CTS) without symptoms or signs to suggest other disorders that can mimic CTS, it remains somewhat controversial as to whether performing nerve conduction studies is necessary or cost-effective. MR imaging reliably depicts normal carpal tunnel anatomy. It can also identify pathologic nerve compression and mass lesions, such as ganglion cysts, that compress nerves. Currently, MR imaging is most commonly used to image patients with ambiguous electrodiagnostic studies and clinical examinations. MR diffusion-weighted imaging of peripheral nerves might p

Changes in nerve conduction velocity across the elbow due to experimental error.

One diagnostic criterion for ulnar nerve mononeuropathy at the elbow (UNE) is a decrease in across-elbow nerve conduction velocity (NCV) > 10 m/s compared to the forearm segment. Distance and latency measurement errors are an inherent part of NCV calculations. Twenty electromyographers measured the latencies of stored ulnar compound muscle action potentials and measured the forearm and across-elbow distances along the ulnar nerve. Based on previously published equations, experimental error in NCV was calculated for various NCVs. The mean distances and standard deviations for the forearm and el

Creutzfeldt-Jakob disease with amyotrophy and demyelinating polyneuropathy.

OBJECTIVE: To report the clinical and neuropathological features in a patient with Creutzfeldt-Jakob disease with amyotrophy and demyelinating polyneuropathy. DESIGN: Case report. PATIENT AND RESULTS: A 62-year-old man had progressive numbness of the left foot, unsteady gait, diminished deep reflexes, fasciculations, and tactile hypesthesia on the feet. Cerebrospinal fluid, electroneurography, and electromyography were suggestive of chronic inflammatory demyelinating polyneuropathy. He was treated with plasmapheresis, corticosteroids, and immunglobulins, with minimal improvement. After 2 mon

[A case of slowly progressive "pharyngeal-cervical-brachial variant of Guillain-Barré syndrome (PCB)"].

A 69-year-old woman was admitted to our hospital because of slowly progressive weakness in the neck, shoulders, proximal arms, oropharyngeal muscles. From a viewpoint of clinical course and signs, it was suspected that the patient was suffered from motor neuron disease. However, serial electrophysiological studies showed the existence of local demyelination of the motor nerves. The immunoadsorption was then performed and marked recovery of symptoms was obtained. In this case, we could not detect any established anti-ganglioside antibodies which was related to pharyngeal-cervical-brachial varia

Altered structure and function of the carotid body at high altitude and associated chemoreflexes.

The ventilatory response to hypoxia is complex. First contact with hypoxia causes an increase in ventilation within seconds that reaches full intensity within minutes because of an increase in carotid sinus nerve (CSN) input to the brain stem. With continued exposure, ventilation increases further over days (ventilatory acclimatization). Initially, it was hypothesized that ventilatory acclimatization arose from a central nervous system (CNS) mechanism. Compensation for alkalosis in the brain and restoration of pH in the vicinity of central chemoreceptors was believed to cause the secondary inc

Prolonged peroneal nerve dysfunction after high tibial osteotomy: pre- and postoperative electrophysiological study.

To evaluate electrophysiological incidence and the type of peroneal nerve lesions seen after high tibial osteotomy we conducted an electrophysiological study (electromyography and nerve conduction velocity studies) in 11 patients who were suffering from medial gonarthrosis and treated by Maquet barrel-vault type high tibial valgization osteotomy. All the patients were tested both pre- and postoperatively. Every patient was examined postoperatively for a minimum of a 6 months after surgery to eliminate spontaneously reversible lesions. Results obtained from nonoperated legs served as controls.

[Polyneuropathy due to glue-sniffing intoxication. Report of a case].

We report a case of a 24 years old female who presented a glue sniffing neuropathy. Symptoms began with progressive weakness which initially involves the lower extremities. Neurological examination noted a decreased Achille reflexes. Nerve conduction studies showed a slowing nerve conduction velocities, a delayed F waves and distal latencies and diffuse nerve conduction blocks. The disease continued to worsen beyond two weeks after stopping intoxication with persistent evidence of ongoing demyelination. 40 days later we have noted an improvement in nerve conduction velocities and in conduction

Neurologic evaluation of the upper extremity.

Symptoms and disease processes can overlap, and neurologic symptoms of the hand or arm are not necessarily due to local pathology. To combat these difficulties, the authors present a basic review of neuroanatomy, the neurologic examination, the principles of electromyography, and the process of formulation.

Effects of lidocaine on nucleus pulposus-induced nerve root injury. A neurophysiologic and histologic study of the pig cauda equina.

STUDY DESIGN: Application of autologous nucleus pulposus on nerve roots and treatment with local application of lidocaine in the pig. OBJECTIVES: Studies of the effects of lidocaine on nucleus pulposus-exposed nerve roots. SUMMARY OF BACKGROUND DATA: Nerve root infiltration may improve radicular symptoms beyond the pharmacologic duration of local anesthetics, but the mechanisms for this effect are not known. METHODS: Nucleus pulposus was harvested from a lumbar disc and placed onto the sacrococcygeal cauda equina in pigs. In Series 1, early lidocaine treatment of nucleus pulposus-induced ne

Pregnancy-related carpal tunnel syndrome.

Clinical and electrophysiological features were studied in 52 cases of carpal tunnel syndrome (CTS) associated with 30 pregnancies. The duration of symptoms was less than 3 months before electrodiagnosis was performed. This study revealed a higher incidence of persistent, painful diurnal symptoms in pregnancy-related CTS (PRCTS) than in idiopathic CTS. PRCTS usually occurs for the first time (de novo) (24/30 cases), in primigravidas (15 cases) as well as multigravidas. The onset of symptoms was in the first or second trimester in 11 women, the third trimester in 12 women, or the post-partum pe

Silent period abnormalities in carpal tunnel syndrome.

Cutaneous silent period (CSP) was measured on stimulating digits 2 and 5 in 19 patients with carpal tunnel syndrome (CTS) and compared with 20 healthy volunteers. In 2 patients with severe CTS, CSP was absent on digit 2 but present on digit 5. In mild/moderate CTS digit 2 CSP mean duration was 50.9+/-13.9 ms, significantly longer than control subjects (36.9+/-8.7 ms) (P = 0.005). CSP was found to be abnormal in CTS, being absent in severe and prolonged in mild/moderate cases.

Spatial and temporal variations in pacemaking and conduction in the isolated renal pelvis.

In renal pelvis preparations isolated from the sheep, the location of the pacemaker and the pathway of conduction of the electrical impulse in the pelvis were analyzed in detail. An electrophysiological acquisition system was used allowing simultaneous recordings from 240 extracellular electrodes. Reconstruction of the spread of activity showed that the site of the pelvis pacemaker was, in virtually all cases, located at the pelvicalyceal border and never in the body of the pelvis or in the area of the pelviureteric junction. One single pacemaker was responsible for a particular spread of acti

Evaluation of central motor conduction in hypothyroid and hyperthyroid patients.

Deficiency or excess of thyroid hormones is associated with central nervous system (CNS) disturbances. Although the CNS involvement either in hypothyroidism or in hyperthyroidism have previously been shown on the basis of visual, auditory and somatosensory evoked potentials studies, less is known about the function of central motor pathways in both disorders. We studied the motor evoked potentials (MEPs) following the magnetic stimulation of the motor cortex and spinal roots in 20 patients with hypothyroidism and in 19 patients with hyperthyroidism both before treatment and after they became e
