Amygdalar neuromelanosis intractable epilepsy without leptomeningeal involvement.

Neurocutaneous melanosis (NCM) is a rare, congenital neuroectodermal dysplasia generally resulting in early death from neurological decline due to leptomeningeal involvement. Nonmeningeal CNS epileptogenic lesions presenting in later childhood in the absence of leptomeningeal disease are rare. This report summarizes a rare presentation and curative epilepsy surgery. The authors discuss the case of a 14-year-old girl with NCM who originally presented with intractable partial-onset seizures. The MRI, PET, and SPECT studies subsequently revealed a focal epileptogenic source in the right temporal

Risk of dysphagia and speech and language delay in PHACE syndrome.

PHACE (posterior fossa, hemangioma, arterial lesions, cardiac, and eye) syndrome consists of infantile hemangiomas of the head and neck along with a spectrum of noncutaneous anomalies. Neurodevelopmental abnormalities have also been noted. Here we describe the association between PHACE syndrome and abnormalities in oropharyngeal development and coordination manifesting as dysphagia or speech and language delay. A retrospective chart review was conducted of 34 patients with PHACE syndrome. Data were collected from prior clinical notes and radiographic studies and the results of a comprehensive

[Exacerbation of skin lesions during fever in a patient with chronic infantile neurologic cutaneous articular (CINCA) syndrome].

Chronic infantile neurologic cutaneous articular (CINCA) syndrome is a serious chronic systemic inflammatory disease that presents at a young age and that is characterized by skin, joint, and central nervous system disease. Skin symptoms are the first to appear, in the form of a longstanding nonpruritic urticarial rash, with exacerbations coinciding with episodes of fever, arthritis, and enlarged lymph nodes. The findings of biopsy of skin lesions are extremely variable but characterized by perivascular neutrophilic infiltrate. With the discovery of mutations in the CIAS1 gene, which encodes a

Moyamoya syndrome and PHACE syndrome: clinical and radiographic characterization of the intracranial arteriopathy and response to surgical revascularization.

OBJECTIVE: PHACE syndrome (PHACES) has been linked to cervical and cerebral vascular anomalies, including persistent embryonic anastomoses and progressive steno-occlusive disease. However, no prior studies have documented the long-term response of PHACES patients with moyamoya disease to surgical revascularization with pial or myosynangiosis. The authors present their experience with 8 consecutive patients with PHACES and moyamoya disease. METHODS: Retrospective review of patients who underwent pial synangiosis revascularization for moyamoya disease with concurrent diagnosis of PHACES. RESUL

Early viral suppression improves neurocognitive outcomes in HIV-infected children.

OBJECTIVE: To estimate the association of age of viral suppression and central nervous system penetration effectiveness (CPE) score with neurocognitive functioning among school-age children with perinatally acquired HIV infection (PHIV+). DESIGN: We analyzed data from two US-based multisite prospective cohort studies. METHODS: Multivariable general linear regression models were used to evaluate associations of age at viral suppression and CPE scores (of initial antiretroviral therapy regimen and weighted average) with the Wechsler Intelligence Scale for Children, Third or Fourth Edition neur

Gómez-López-Hernández syndrome in a child born to consanguineous parents: new evidence for an autosomal-recessive pattern of inheritance?

BACKGROUND: Gómez-López-Hernández syndrome is a rare genetic disease characterized by scalp alopecia with trigeminal anesthesia, brachycephaly or turribrachycephaly, midface retrusion, and rhombencephalosynapsis. We report the second case with this condition who presented with consanguineous parents. PATIENT: This boy was evaluated shortly after birth because of suspected craniosynostosis. He was the only son of healthy, consanguineous parents (his maternal grandmother and his paternal great-grandfather were siblings). His examination was notable for turribrachycephaly, prominent forehead, bi

Ultra-Widefield Swept-Source OCTA Findings in Coats Plus Syndrome.

Coats plus syndrome (CPS), also referred to as cerebroretinal microangiopathy with calcifications and cysts (CMCC), is a rare autosomal recessive disease that primarily targets the microvasculature of the retina, brain, bones, and gastrointestinal system. This study reports the case of a 24-year-old female patient who was initially diagnosed with familial exudative vitreoretinopathy (FEVR) and was lost to follow-up. The patient underwent multimodal retinal imaging, including ultra-widefield swept-source optical coherence tomography angiography (SS-OCTA) (DREAM OCT, Intalight). Examination reve

Office-Based Subglottic Evaluation in Children With Risk of Subglottic Hemangioma.

PURPOSE: Children with V3 cutaneous infantile hemangiomas (IH) and PHACE syndrome have a high incidence for airway hemangioma, 29% and 52%, respectively. Therefore, a clinical evaluation for these high-risk children is essential. We report our experience with in-office lower airway evaluation (OLAE) in these high-risk children. RESULTS: Since 2003, 5 children with IH of the V3 cutaneous distribution and 3 children with PHACE syndrome underwent OLAE. Average age of presentation was 2.75 months. Two children had stridor at initial evaluation, and 1 child had subglottic hemangioma. This child wa

Wyburn-Mason Syndrome Associated With Cutaneous Reactive Angiomatosis and Central Retinal Vein Occlusion.

Retinal venous occlusive events are a rare complication of arteriovenous malformations of the retina found in Wyburn-Mason syndrome. The authors present a case of a 28-year-old man diagnosed with Wyburn-Mason syndrome and cutaneous reactive angiomatosis, a reactive angioproliferative disorder induced by vascular occlusion. He developed a central retinal vein occlusion complicated by macular edema and received treatment with intravitreal bevacizumab, which led to resolution of the edema. To the best of the authors' knowledge, this is the first report of an anti- vascular endothelial growth fact

Development and characterization of a mouse with profound biotinidase deficiency: a biotin-responsive neurocutaneous disorder.

Biotinidase deficiency is the primary enzymatic defect in biotin-responsive, late-onset multiple carboxylase deficiency. Untreated children with profound biotinidase deficiency usually exhibit neurological symptoms including lethargy, hypotonia, seizures, developmental delay, sensorineural hearing loss and optic atrophy; and cutaneous symptoms including skin rash, conjunctivitis and alopecia. Although the clinical features of the disorder markedly improve or are prevented with biotin supplementation, some symptoms, once they occur, such as developmental delay, hearing loss and optic atrophy, a

Neuroimaging and Clinical Features of Parry-Romberg Syndrome and Linear Morphea En-coup-de-sabre in a Large Case Series.

RATIONALE AND OBJECTIVES: Parry-Romberg Syndrome (PRS) and linear morphea en-coup-de-sabre (ECDS) are rare neurocutaneous disorders characterized by unilateral progressive hemifacial atrophy and linear scleroderma, respectively1,2. Imaging is important for assessing soft tissue and intracranial involvement3, though literature is limited to case reports and series 2,4-8. We aim to describe radiologic and clinical epidemiologic features of PRS and ECDS. MATERIALS AND METHODS: A retrospective review of our institutional radiology database identified patients with PRS and ECDS who underwent MRI o

Seizure and developmental outcomes following epilepsy surgery for children with Sturge-Weber syndrome.

OBJECTIVE: Sturge-Weber syndrome (SWS) is a rare neurocutaneous disorder. Up to 90% of children have epilepsy. Epilepsy surgery is a treatment option. However, the seizure and neurodevelopmental outcomes following this are not well defined. This study aimed to determine the outcomes for children undergoing epilepsy surgery for SWS in relation to seizure control, cognition, language, motor function and social communication. METHODS: A retrospective case-note review was completed for children with SWS undergoing epilepsy surgery at Great Ormond Street Hospital from 1993 to 2022. Results of stan

Introduction of a Brain MRI Scoring System with Clinical Relevance for Sturge-Weber Syndrome.

RATIONALE AND OBJECTIVES: Sturge-Weber syndrome (SWS) is a sporadic neurocutaneous disorder marked by cerebral venous abnormalities, progressive parenchymal damage, and early-onset neuro-cognitive complications. Existing imaging assessments lack standardized, quantitative approaches to capture the full disease burden. Here we tested an magnetic resonance imaging (MRI)-based scoring system that comprehensively captures both vascular and parenchymal brain abnormalities in SWS. MATERIALS AND METHODS: Twenty-five young patients (mean age, 9.5 years; range, 1-24 years) with unilateral SWS brain in
