Assessment of infants with peripherally inserted central catheters: Part 2. Detecting less frequently occurring complications.

Peripherally inserted central catheters (PICCs) are frequently placed in neonates to optimize nutrition and provide stable infusions of critical medications into the central vascular system. PICCs have been associated with a number of device-specific complications that can manifest during insertion, while the line is indwelling, and/or after the removal of the line. The first article of this series in Focus on the Physical, titled "Assessment of Infants With Peripherally Inserted Central Catheters: Part I. Detecting the Most Frequently Occurring Complications," presented assessment strategies

[No solution for neonatal mortality in sub-saharan Africa? Evaluation and perspectives in the urban environment of Niamey, Niger].

Niger has one of the highest mortality rates of infants (222/1000) and children under five years old (318/1000), with 15% of them suffering from malnutrition. Yet, neonatal mortality was not considered as the top priority of public health in Niger, where 85 to 90% of the deliveries succeed without any medical care and 70% of the population live more than 10 kilometers from the nearest medical center. Also, in the African countries which have adopted expensive neonatal care centers following the occidental model, the lethality rate is high and maintenance is difficult. Thus, alternative strateg

Integration of Virtual and Augmented Reality Into Obstetric Nursing and Midwifery Education: Systematic Review.

BACKGROUND: Technologies such as virtual reality (VR) and augmented reality (AR) have been increasingly incorporated into nursing education to support the development of cognitive, psychomotor, and behavioral competencies. In midwifery, immersive environments offer opportunities to simulate high-risk or low-frequency clinical scenarios, strengthening students' confidence and preparedness for professional practice. However, there is still significant variability in how VR and AR are pedagogically implemented, with limited understanding of their theoretical grounding, instructional design, and e

The Beneficence of Cuddle Therapy in Hyperekplexia: A Case Report.

BACKGROUND: Benevolent injustice occurs when well-intentioned treatment efforts produce an outcome that limits the potential of a patient. The unintended harm can result in significant moral distress for the family and the healthcare team. CLINICAL FINDINGS: We discussed an ethical dilemma regarding a neonate who had suspected seizure and hypoxic-ischemic encephalopathy after home birth delivery. The healthcare team experienced moral distress about the mother's desire to not use anti-seizure medications and instead trial other interventions such as cuddling. Subsequently, clinical analysis ru

Feasibility of Air Transport Simulation Training: A Case Series.

Limited clinical site availability and an increased need for clinical training experiences often make it difficult for prehospital health care providers to complete new and annual training requirements. Medical simulation provides an alternative learning environment that provides trainees the opportunity to acquire and perfect new clinical skills without compromising patient care. The following is a detailed description of an air medical transport simulation of a neonate with hypoxic ischemic encephalopathy requiring transport to a higher level of care. Patient parameters were altered during f

Examination of the newborn with congenital scoliosis: focus on the physical.

Although scoliosis at birth is rare, conditions at birth and in the newborn period predispose newborns to the development of scoliosis in later life. Scoliosis is congenital when associated with abnormal vertebral segmentation regardless of the age of diagnosis. Other conditions may predispose neonates to vertebral damage or the development of sustained uneven forces on the developing spine. Although it is difficult to know which newborns will progress to developing scoliosis, it is important to be aware of risk factors to provide anticipatory education for parents and to arrange appropriate f

Prevention, assessment, and treatment of central venous catheter occlusions in neonatal and young pediatric patients.

Central venous catheter occlusions disrupt delivery of needed therapies to vulnerable neonatal and young pediatric patients. Nursing practices that minimize risk of catheter occlusion include proper infusion and flushing techniques. Treatment strategies vary on the basis of the source of the occlusion. Mechanical occlusions can often be resolved by altering the patient's position, correcting catheter kinks, or adjusting clamps. Occlusions caused by precipitation can be cleared through instillation of ethanol, hydrochloric acid, or sodium bicarbonate, depending on the chemistry of the infusates

Control of oxygenation during mechanical ventilation in the premature infant.

Maintenance of oxygen saturation targets is a demanding and tedious task because of the frequency with which oxygenation changes, especially in small infants receiving prolonged respiratory support. It is clear that the achievement of oxygenation targets can be improved by a higher nurse-to-patient ratio and by intense staff training. Automated control systems can also improve target maintenance, and this is achieved mainly by reducing exposure to hyperoxemia. The long-term benefits and safety of this strategy are yet to be determined in clinical trials.

Congenital adrenal hyperplasia due to 21-hydroxylase deficiency.

PURPOSE: To provide an in-depth exploration of the complex and potentially life-threatening condition, congenital adrenal hyperplasia (CAH). CONCLUSIONS: CAH affects adrenal gland function, resulting in abnormal steroidogenesis caused by a deficiency or complete lack of the enzyme 21-hydroxylase (accounting for 90% of CAH cases). Clinical manifestations include ambiguous genitalia in female newborns and life-threatening salt-wasting crisis in both male and female newborns. PRACTICE IMPLICATIONS: Nurses should encourage and assist CAH patients and their parents in taking an active role in the

Arthrogryposis multiplex congenita.

Arthrogryposis multiplex congenita (AMC) is a term that is used to describe the presence of multiple joint contractures at birth. AMC can be seen singularly or in conjunction with other abnormalities. Historically, the term arthrogryposis was used as a disease diagnosis, but it is now clear that AMC is not a disease entity but a syndrome, involving a manifestation of many fetal and neonatal disorders of the neuromuscular system. Its etiology is multifocal, and there is a wide variation in the degree to which muscles and joints are affected. Early identification and implementation of a plan of

Hyperthyroidism and pregnancy.

Association of hyperthyroidism and pregnancy is not an unusual event, and has an impact on both the mother and fetus. After delivery, it may also affect the newborn and the nursing mother. Clinical management of this situation is quite different from that required by non-pregnant hyperthyroid women and poses significant diagnostic and therapeutic challenges. This review addresses aspects related to the unique characteristics of biochemical assessment of thyroid function in pregnancy, the potential causes of hyperthyroidism in pregnancy, and the clinical and therapeutic approach in each case. Undetected apnea and bradycardia in infants.

Despite routine monitoring, a number of prolonged apneic and bradycardic episodes were undetected in a group of infants in the neonatal intensive care unit (NICU). Sixty-one infants were evaluated by 12-hour pneumocardiograms at a post-conceptional age of 35 +/- 3 (SD) weeks. Nursing documentation failed to detect 11 infants with prolonged apnea and bradycardia. Three of these infants were not detected in spite of increased awareness following in-service education. Such a lack of documentation may lead to improper medical management of infants at risk for pathologic apnea and suggests the need

Neonatal heart rate variability and intraventricular hemorrhage: a case study.

PURPOSE: The purpose of this study was to compare heart rate variability (HRV) in low-risk, pre-term infants to one infant diagnosed with intraventricular hemorrhage (IVH). METHOD: A case study design was used to compare HRV of one subject diagnosed with IVH to a convenience sample of 38 low-risk, pre-term infants at 30 and 31 post-menstrual weeks of age. Heart periods were recorded for 300-s with the infant in an active sleep state. Heart rate variability was quantified by spectral analysis. A confidence interval comparison of the total spectral components (0.02-2.0 Hz), high-frequency compo

The lived experiences of South African parents during high-risk pregnancy and prolonged neonatal intensive care unit admission.

BACKGROUND: Becoming a parent is a significant life event; however, premature birth abruptly alters this transition, creating short- and long-term consequences for both parents and infants. AIM: This study sought to explore the lived experiences of South African parents throughout the trajectory of premature birth and the subsequent hospitalisation of their baby in the neonatal intensive care unit (NICU). SETTING: The research was conducted in Johannesburg, South Africa, within the private healthcare setting. METHODS: A quantitative, exploratory and descriptive phenomenological study design

The medical and surgical management of gastroschisis.

Gastroschisis (GS) is a full-thickness abdominal wall defect in which fetal intestine herniates alongside the umbilical cord into the intrauterine cavity, resulting in an intestinal injury of variable severity. An increased prevalence of gastroschisis has been observed across several continents and is a focus of epidemiologic study. Prenatal diagnosis of GS is common and allows for delivery planning and treatment in neonatal intensive care units (NICUs) by collaborative interdisciplinary teams (neonatology, neonatal nursing and pediatric surgery). Postnatal treatment focuses on closure of the
