Update on orbital trauma.

PURPOSE OF REVIEW: To evaluate the recent developments in the evaluation and treatment of orbital trauma from April 2003 to March 2004. The scope of orbital trauma included a MEDLINE search under the keywords of zygoma and orbital fracture, and traumatic optic neuropathy. RECENT FINDINGS: Preoperative volume assessment with computed tomography of orbital fractures can help surgical planning of volume replacement. Ultrasonography can visualize anterior orbital fractures, including rim and zygoma trauma. Orbital trauma has a high incidence of concurrent ocular trauma. Fractures in the presence …

Orbital cystic neurinoma.

A 64-year-old woman presented with a 6-month history of exophthalmus of her right eye. She had no neurological deficit except for the exophthalmus and numbness in the right side of her forehead (frontal nerve territory). Neuro-imaging demonstrated a cystic mass in the right orbit. The tumour was totally removed by microsurgical transcranial surgery. The histological diagnosis was neurinoma. The origin of the tumour was considered to be the frontal nerve. Only two other cases of orbital cystic neurinoma have been reported. This rare clinical entity should be included in the differential diagnos…

Endoscopic orbital surgery.

Various applications of endoscopy to orbital surgery are described in this article. The endoscopic approach offers improved surgical exposure, enhanced illumination, simultaneous visualization of operative field by multiple members of surgical team, and superior cosmesis compared with conventional surgical interventions. The procedure can be performed with minimal morbidity. The endoscopic orbital surgery, however, is technique dependent and should be performed only by surgeons who are familiar with orbital anatomy and basic endoscopic technique. It would be beneficial to surgeons to have prev…

CT of orbital trauma.

In a patient with acute orbital trauma, visual acuity and extraocular muscle motility are the two most important ophthalmologic functions to be evaluated emergently. The assessment of these capabilities may sometimes be difficult due to the severity of the head injury, the extent of periorbital soft tissue edema, inadequate cooperation in alert patients, and a reduced level of consciousness in obtunded individuals. Consequently, computed tomography (CT) has come to play a major role in the orbital examination of acute trauma patients. In this study, in conjunction with clinical evaluation, we …

[Rhino-orbital zygomycosis].

Zygomycosis is rare but highly invasive fungal infection, with high mortality rate. A 67 years old diabetic man was presented with rhino-ocular form of the disease. Fungal elements invaded the skin and subutaneous facial tissue, with involvement of the nose, paranasal sinuses and orbit. The portal of entry of fungus was through paranasal sinuses, after the tooth extraction. Various clinical manifestations were presented: headache, facial swelling, tenderness over the involved sinuses, unilateral orbital cellulitis with proptosis, facial and orbital pain, black nasal discharge, decreased visual…

Orbital involvement in cherubism.

Cherubism is a rare, inherited condition characterized by fibro-osseous lesions of the maxilla and mandible. It has recently been localized to chromosome 4p16.3. The fullness of the lower half of the face and retraction of the lower lids gives the characteristic 'eyes raised to heaven' cherubic appearance. A case report of a 7-year-old girl with extensive orbital involvement of cherubism is presented. The patient underwent multidisciplinary surgery for the bony lesions, which extended from the maxillary antrum into adjacent structures, including extensive extraperiosteal orbital involvement. C…

A sighting of orbital pseudotumor.

A 39-year-old woman developed bilateral proptosis, photophobia, and pain with extraocular movements over the course of 5 days. Her findings initially were ocular pain and photophobia which progressed to periorbital edema and nasal discharge ultimately resulting in proptosis with vertical globe displacement and decreased visual acuity. She was diagnosed with corneal abrasion and sinusitis respectively during two initial emergency department visits. On her third visit to the emergency department within 4 days, she developed acute visual deficits. The patient was subsequently diagnosed with orbit…

Radiology of the orbital apex.

The orbital apex, formed by the superior orbital fissure and optic canal, is the cross-road between the orbit and the intracranial structures. Pathological processes may extend intracranially via the superior orbital fissure and vice versa. In addition to intrinsic soft tissue lesions, various pathological processes may involve the surrounding osseous anatomy. Malignant lesions arising from adjacent structures or from haematogeneous metastasis may also infiltrate this region.

Congenital orbital teratoma.

A case of congenital orbital teratoma is described in which there was no organized eye only microscopic evidence of ocular tissues within the disorganized teratoma. A baby boy presented at birth with a 10-x-8-cm mass extruding from the left orbit. Magnetic resonance imaging (MRI) showed a mixed cystic-solid orbital mass containing areas of calcification and deforming the bony orbit around its margins. There was no organized eye and no intracranial extension. The eye was removed with reconstruction of the eyelids. Histopathology showed representation from all three germ cell layers consistent w…

Orbital infections and inflammations.

Orbital infections and inflammations present to the clinician with similar findings: periorbital edema, erythema, proptosis, and pain. History and clinical examination determine the work-up required to better define the disease process. Orbital infections continue to be associated primarily with diseases of the paranasal sinuses. Haemophilus influenza type B is no longer a significant pathogen, because of an effective vaccine. Fungal infections extending to the orbit are becoming more frequent due to the prevalence of immunocompromised patients. Orbital inflammations continue to be poorly unde…

[Calcifying pseudotumor of the orbits].

Orbital pseudotumor can be divided histopathologically into three basic types: lymphoid, granulomatous and sclerosing. Chronic lymphoid and granulomatous types may eventually transform into the sclerosing type. The calcifying orbital pseudotumor is a very rare disorder due to a chronic, idiopathic inflammatory process of the orbit. The granulomatous type shows a good response to steroids while radiotherapy is the method of choice for lymphoid lesions. In general, calcifying pseudotumors can be treated only by operative exploration and tumor removal. We report a patient with a calcifying pseudo…
