Management of Legg-Calve-Perthes disease with acupuncture: a case report.

Legg-Calve-Perthes disease (LCPD) is a rare temporary hip joint deformity mostly effecting young children from 4-10 years of age. It involves mainly the head of the femur, which softens and breaks down due to interruption of blood supply (avascular necrosis). We report a case of LCPD that was treated late and had a poor prognosis, but improved significantly during a prolonged course of acupuncture. A 12-year-old boy reported to an orthopaedic clinic in 2006 with limping and was diagnosed with LCPD. Surgeons applied orthosis without improvement and decided to perform surgery in 2008. However, t

Femoral nerve palsy in Pavlik harness treatment for developmental dysplasia of the hip.

BACKGROUND: Transient femoral nerve palsy is a potential complication of the use of a Pavlik harness to treat developmental dysplasia of the hip. Our hypothesis was that patients who develop a femoral nerve palsy while undergoing Pavlik harness treatment for developmental dysplasia of the hip are more likely to have unsuccessful orthotic treatment and to require closed or open hip reduction. METHODS: We performed a retrospective review of all patients who underwent Pavlik harness treatment for developmental dysplasia of the hip within a seventeen-year period (1992 to 2008). All cases of femor

[Effect of abducens orthosis combined with walker on developmental dysplasia of the hip].

OBJECTIVE: To evaluate the effect of abducens orthosis combined with walker on developmental dysplasia of the hip (DDH). METHODS: A total of 126 patients (224 hips) with DDH aged 6-36 months in Xiangya Hospital was randomly divided into 2 groups: an orthosis combined with walker group and an improved hip frog cast fixation group. Seventy patients (130 hips) were treated by the orthosis combined with walker and 56 patients (94 hips) were treated by the improved hip frog cast fixation. We compared the effect and complications of the 2 groups. RESULTS: The fineness rates of the orthosis combine

Scientific assessment of over-the-counter foot orthoses to determine their effects on pain, balance, and foot deformities.

BACKGROUND: A scientific study was conducted to evaluate the effects of non-custom-molded (over-the-counter) foot orthoses. METHODS: Several parameters were examined, including foot, knee, hip, and back pain; balance; and reduction in flexible deformities, such as hammer toes and hallux valgus. Wherever possible, objective measurements were used, including measurements of shifts in center of pressure to assess balance and changes in bone position examined on radiographs. Forty-one individuals were analyzed using one of two types of prefabricated, non-custom insoles. Insoles were fit by an ass

Effect of ankle-foot orthoses on walking efficiency and gait in children with cerebral palsy.

OBJECTIVE: To determine the effect of ankle-foot orthoses on walking efficiency and gait in a heterogeneous group of children with cerebral palsy, using barefoot walking as the control condition. DESIGN: A retrospective study. METHODS: Barefoot and ankle-foot orthosis data for 172 children with spastic cerebral palsy (mean age 9 years; hemiplegia: 21, diplegia: 97, and quadriplegia: 54) were compared. These data consisted of non-dimensional speed, net non- dimensional energy cost of walking (NN-cost), and NN-cost as a percentage of speed-matched controls (NN-cost(pct)). For 80 of these child

Conservative treatment of obstructive sleep apnea: a case report.

Conservative treatment of obstructive sleep apnea includes weight loss, changes in sleep posture, drug therapy, nasal continuous positive airway pressure and placement of an intraoral prosthesis. This article describes the success achieved in a case treated with conservative therapy, that is, intraoral prosthesis. A mandibular advancement prosthesis was fabricated for a 35-year-old male patient suffering from obstructive sleep apnea. The prosthesis was fabricated at a raised vertical dimension, with the central area open for the air passage. This advanced mandibular position enhanced the paten

The diabetic foot--an update.

Despite an improvement in our understanding of the aetiopathogenesis of diabetic foot problems in the last 2 decades, the 21st Century epidemic of type 2 diabetes will ensure that the incidence of foot problems will continue to increase in the diabetic population. In the aetiopathogenesis it is important to understand that a number of factors working together usually result in foot ulceration: the commonest trio is neuropathy, deformity and trauma. In Western countries, the incidence of neuroischaemic ulcers is now increasing making early detection of those at risk even more important. In the

Effects of locomotion training with assistance of a robot-driven gait orthosis in hemiparetic patients after stroke: a randomized controlled pilot study.

BACKGROUND AND PURPOSE: The success of gait rehabilitation after stroke depends on active walking exercises. However, the disabling after-effects of stroke often make such exercises impossible at the onset of therapy. To facilitate treadmill training of paraparetic patients, a robot-driven gait orthosis (Lokomat) was developed. We investigated the effects of the Lokomat when used with hemiparetic patients. METHODS: The authors conducted a randomized, controlled pilot study of 30 acute stroke survivors. The treatment group received 30 minutes of robotic training daily and the control group 30

A transepiphyseal fracture of the proximal femur combined with a fracture of the mid-shaft of ipsilateral femur in a child: a case report and literature review.

The paper describes a rare fracture of proximal femur, classified by Delbet (Am J Surg 6:793-797, 1929) as type I (transepiphyseal type) combined with a fracture of the midshaft of ipsilateral femur in a 2-year-old child. Immediate operation with open reduction and internal fixation was successful. During the postoperative course, avascular necrosis (AVN) of capital femoral epiphysis was seen by bone scan. Applying an abduction orthosis used for the treatment of Legg-Calvè-Perthes disease, collapse of capital epiphysis was prevented. Although, a minimal area of AVN and coxa vara remained, no c
