Early epidemiological investigations: World Health Organization UNITY protocols provide a standardized and timely international investigation framework during the COVID-19 pandemic.

BACKGROUND: The declaration of Coronavirus disease 2019 (COVID-19) as a Public Health Emergency of International Concern (PHEIC) on 30 January 2020 required rapid implementation of early investigations to inform appropriate national and global public health actions. METHODS: The suite of existing pandemic preparedness generic epidemiological early investigation protocols was rapidly adapted for COVID-19, branded the 'UNITY studies' and promoted globally for the implementation of standardized and quality studies. Ten protocols were developed investigating household (HH) transmission, the first…

Surveillance Metrics and History of the COVID-19 Pandemic in Central Asia: Updated Epidemiological Assessment.

BACKGROUND: This study updates the COVID-19 pandemic surveillance in Central Asia we conducted during the first year of the pandemic by providing 2 additional years of data for the region. The historical context provided through additional data can inform regional preparedness and early responses to infectious outbreaks of either the SARS-CoV-2 virus or future pathogens in Central Asia. OBJECTIVE: First, we aim to measure whether there was an expansion or contraction in the pandemic in Central Asia when the World Health Organization (WHO) declared the end of the public health emergency for th

Ten pressure points in primary care during COVID-19: findings from an international narrative review.

BACKGROUND: Strong primary care (PC) services are the foundation of high-performing health care systems and can support effective responses to public health emergencies. Primary care practitioners (PCPs) and PC services played crucial roles in supporting global health system responses to the COVID-19 pandemic. However, these contributions have come at a cost, impacting on PC services and affecting patient care. This secondary analysis of data from an integrative systematic review across international PC settings aimed to identify and describe burdens and challenges experienced by PCPs and PC s

A Cross-Sectional Study to Understand COVID-19 Vaccine Hesitancy Among Nursing Students and Health Workers: Implications for Future Outbreak Preparedness in South Africa.

BACKGROUND: Vaccine hesitancy is a global challenge affecting workplaces and academic institutions. Despite efforts, low vaccination rates persist in South Africa, increasing the risks associated with pathogen exposure. This study assessed the knowledge, attitudes, and perceptions (KAPs) of healthcare workers (HCWs) and students at a Nursing College regarding voluntary and mandatory COVID-19 vaccination. METHODS: A quantitative cross-sectional study was conducted using a structured, self-administered electronic questionnaire. All eligible nursing students and staff at the participating Nursin

A scoping review of authorisation pathway for COVID-19 vaccines among selected countries.

BACKGROUND: In response to COVID-19 pandemic, NRAs implemented expedited mechanisms such as EUAs and CMAs, relying on international bodies, to authorize vaccines. Documenting these authorization pathways across countries informs NRAs and strengthens future pandemic responses. Existing studies on COVID-19 vaccine approvals are limited, underscoring the need for a comprehensive review. This study aimed to explore approval pathways, investigate supporting strategies, and review times for COVID-19 vaccines. METHODS: A scoping review was conducted, covering countries with stringent regulatory auth

The Association Between Prepandemic ICU Performance and Mortality Variation in COVID-19: A Multicenter Cohort Study of 35,619 Critically Ill Patients.

BACKGROUND: During the COVID-19 pandemic, ICUs remained under stress and observed elevated mortality rates and high variations of outcomes. A knowledge gap exists regarding whether an ICU performing best during nonpandemic times would still perform better when under high pressure compared with the least performing ICUs. RESEARCH QUESTION: Does prepandemic ICU performance explain the risk-adjusted mortality variability for critically ill patients with COVID-19? STUDY DESIGN AND METHODS: This study examined a cohort of adults with real-time polymerase chain reaction-confirmed COVID-19 admitted

Anxiety and loneliness among older people living in residential care facilities or receiving home care services in Sweden during the COVID-19 pandemic: a national cross-sectional study.

BACKGROUND: Older people were subjected to significant restrictions on physical contacts with others during the COVID-19 pandemic. Social distancing impacts older people's experiences of anxiety and loneliness. Despite a large body of research on the pandemic, there is little research on its effects on older people in residential care facilities (RCF) and in home care services (HCS), who are the frailest of the older population. We aimed to investigate the effect of the first wave of the COVID-19 pandemic in March-May 2020 on experiences of anxiety and loneliness among older people living in R

Sustained and discontinued operational changes to ethics committees in the post-pandemic era: a qualitative study in Kenya.

BACKGROUND: Ethics Committees (ECs) play a vital role in protecting the rights, safety, and welfare of human participants enrolled in research. In special circumstances such as pandemics, ECs conduct a rapid review of research protocols to respond to a Public Health Emergency (PHE). Globally, many ECs made operational changes to their processes during the COVID-19 era in a bid to continue providing research oversight amidst strict measures to combat the spread of the disease. Little is known about any sustained and/or discontinued EC operational adjustments made during the COVID-19 pandemic. T

Essential work, unequal places: Neighborhood characteristics of the essential workforce in the United States during the COVID-19 pandemic.

INTRODUCTION: At the onset of the COVID-19 pandemic, more than 55 million U.S. workers continued providing "essential" services crucial to public health, safety, and economic stability, even as large segments of the workforce sheltered at home. Despite the central role these workers played in safeguarding the economy and the greater American public, little is known about the neighborhood environments in which essential workers lived and how well these communities were equipped to support the health and well-being of essential workers during the pandemic. Therefore, the primary objective of thi

Post-Pandemic Resurgence and Seasonal Patterns of Influenza Viruses and Respiratory Syncytial Virus in Arequipa, Peru (2021-2023).

Background/Objectives: The coronavirus disease 2019 (COVID-19) pandemic profoundly disrupted global respiratory virus circulation, with sharp declines during 2020-2021, followed by a resurgence after the relaxation of public health measures. In South America, post-pandemic respiratory virus dynamics remain insufficiently characterized, particularly in ecologically diverse regions. Arequipa, a high-altitude city in southern Peru, has unique environmental conditions, including marked seasonal temperature variability, that may influence viral transmission. Methods: We performed a cross-sectional

"We're all in the same storm, but not all of us are in the same boat": qualitative exploration of UK response-focused civil servants experiences of working from home during COVID-19.

INTRODUCTION: The experiences of UK Government response-focused employees, who were considered frontline workers during the coronavirus response, are missing from current literature. Meeting the demands of being on the frontline, whilst also adjusting from a normal and practiced way of working to having to work from within one's home, may bring a plethora of new barriers and facilitators associated with providing an effective pandemic response. METHOD: This interview study collected and analysed data from 30 UK Civil servants who worked on the COVID-19 pandemic response from their own homes.

Exploring the effects of COVID-19 outbreak control policies on services offered to people experiencing homelessness.

BACKGROUND: The COVID-19 pandemic and subsequent implementation of public health policies exacerbated multiple intersecting systemic inequities, including homelessness. Housing is a key social determinant of health that played a significant part in the front-line defence against COVID-19, posing challenges for service providers working with people experiencing homelessness (PEH). Public health practitioners and not-for-profit organizations (NFPs) had to adapt existing COVID-19 policies and implement novel measures to prevent the spread of disease within congregate settings, including shelters.

Prospective Cohort Study of Emergency Department Visit Frequency and Diagnoses Before and During COVID-19 Pandemic in Urban, Low-Income, US- and Foreign-Born Mothers in Boston, MA.

BACKGROUND: The coronavirus 2019 (COVID-19) pandemic fundamentally changed how populations interface with the healthcare system. Despite historical spikes in US mortality during the pandemic, emergency department (ED) visits were paradoxically low. This is a concerning phenomenon that raises a red flag regarding access to care, especially among vulnerable populations. In this study we sought to understand how ED utilization evolved during the COVID-19 pandemic among traditionally understudied, low-income, racially diverse US- and foreign-born mothers. METHODS: This is a secondary analysis of

Enhancing public health intelligence workforce capacity and capability: insights from English local authorities' COVID-19 response.

INTRODUCTION: The COVID-19 pandemic provided a stimulus to improve emergency response capabilities in the United Kingdom. Compared to healthcare workers, there is a paucity of research examining the experiences of public health staff involved in the response. During the pandemic, English local authorities (LAs) responded to limit virus spread and mitigate its impacts by providing public health intelligence (PHI). This article reports workforce-related findings from a larger study which focused on understanding sub-national provision of PHI during an infectious disease emergency. METHODS: A mi

Local government responses for COVID-19 management in the Philippines.

BACKGROUND: Responses of subnational government units are crucial in the containment of the spread of pathogens in a country. To mitigate the impact of the COVID-19 pandemic, the Philippine national government through its Inter-Agency Task Force on Emerging Infectious Diseases outlined different quarantine measures wherein each level has a corresponding degree of rigidity from keeping only the essential businesses open to allowing all establishments to operate at a certain capacity. Other measures also involve prohibiting individuals at a certain age bracket from going outside of their homes.

Characterizing hospital workers' willingness to report to duty in an influenza pandemic through threat- and efficacy-based assessment.

BACKGROUND: Hospital-based providers' willingness to report to work during an influenza pandemic is a critical yet under-studied phenomenon. Witte's Extended Parallel Process Model (EPPM) has been shown to be useful for understanding adaptive behavior of public health workers to an unknown risk, and thus offers a framework for examining scenario-specific willingness to respond among hospital staff. METHODS: We administered an anonymous online EPPM-based survey about attitudes/beliefs toward emergency response, to all 18,612 employees of the Johns Hopkins Hospital from January to March 2009. S

A Phenomenological Study of Nursing Home Residents' Experience During the COVID-19 Pandemic: Lessons Learned.

BACKGROUND: The COVID-19 pandemic has inflicted widespread fear and distress on older populations globally. As highlighted by the World Health Organization, those residing in nursing homes are especially at risk due to the communal living arrangements and the high prevalence of chronic health conditions among residents. Because of the increased vulnerability of nursing home residents, harsher restrictive measures, such as social isolation, were implemented as part of infection control strategies, with a particular emphasis on reducing mortality rates and limiting the spread of disease. As a re
