The benefits and prospects of interleukin-6 inhibitor on systemic sclerosis.

Systemic sclerosis (SSc) is a connective tissue disease, the pathogenesis of which is thought to involve interleukin-6 (IL-6), an inflammatory cytokine. This is based on findings of its concentration in patient serum, the results of an IL-6 suppression experiment in an animal model, and the results of a pilot study using IL-6 receptor antibody. However, it appears that a number of factors are involved in the pathology of SSc depending on the state of disease progression. In addition, the degree of involvement of IL-6 differs depending on the difference of organs within particular severe sympto

Acceptance of different multifocal contact lenses depending on the binocular findings.

In a clinical study the subjective acceptance as well as the quality of different visual functions was examined during the wearing of three multifocal lenses (Ciba Spectrum Bifocal, Allergan Echelon, and Essilor Variations). Ten subjects wore each of the 3 contact lens types for a period of 12 to 21 days after the binocular finding (bicentral or disparate vision within the Panum's area); further functions such as contrast or scotopic vision were qualified and/or quantified in a basic examination. All subjects (N = 5) with the binocular finding orthophoria/motorcompensated heterophoria preferre

Incidental retinal phototoxicity associated with ingestion of photosensitizing drugs.

BACKGROUND: to report on the possible correlation between incident retinal phototoxicity and the use of photosensitizing drugs. METHODS: four patients were examined because of scotomas and visual loss after an incidental exposure to a strong light source. One patient (two eyes) was exposed to standard camera flash; one patient (one eye) had a brief exposure to welding light; one patient (two eyes) underwent uncomplicated phacoemulsifications with intraocular lens implantation. The fourth patient had a severe retinal phototoxicity following a secondary intraocular lens implantation. All four p

Visual evoked cortical potential can be used to differentiate between uncorrected refractive error and macular disorders.

The visual evoked cortical potential (VECP) is widely used to verify complaints of reduced visual performance and to identify the site of the disorder. In this study, we investigated the correlation between reduced visual acuity and VECP in volunteers with normal corrected visual acuity and in patients suffering from inherited macular degeneration or from age related macular degeneration (ARMD). Flash evoked VECP was not affected by the visual acuity in the cases of refractive error and in ARMD patients but was reduced in amplitude and delayed in implicit time in the patients suffering from in

Lightning-induced cataract with concomitant optic nerve damage.

Lightning-induced ocular injuries are rare but potentially severe sequelae. This case report describes a 35-year-old male patient who suffered a lightning strike injury 20 years ago, resulting in traumatic cataract and optic nerve damage in his left eye. When the patient presented for consultation, he exhibited a significant decrease in left eye visual acuity. Detailed ophthalmic examinations and auxiliary tests confirmed the presence of cataract and optic nerve impairment. The patient underwent left eye phacoemulsification cataract surgery and intraocular lens implantation, leading to improve

A case of reverse amblyopia in a myopic anisometropic patient: an atypical presentation.

We herein report a case of a 4-year-old female patient who presented with anisometropic amblyopia with initial visual acuity recorded at 20/400 OD and 20/100 OS. The patient was recommended for patching. Eighteen months later, the patient presented with visual acuity of 20/60 OD and 20/80 and reverse amblyopia was noted. In settings of amblyopia, where alternate patching may be used, it is most likely that reverse amblyopia, if present, will affect the more myopic eye. However, unexpectedly, in this case, reverse amblyopia occurred in the less myopic eye. With discontinuation of occlusion ther

Severe craniofacial fibrous dysplasia associated with decreased visual acuity: a case report.

Fibrous dysplasia is a developmental abnormality characterized by the replacement of normal bone tissue by fibrous connective tissue with poorly organized bone trabeculae. This disorder rarely occurs in the craniofacial region, but in such cases it causes facial asymmetries and has severe clinical implications for the patient. This case report describes the treatment of an 18-year-old man who presented with complaints of facial deformity and decreased visual acuity. Cone beam computed tomography revealed a diffuse bone lesion affecting the region of the maxillary, frontal, and nasal bones on t

Dehiscence of radial keratotomy wounds without globe rupture following explosion injury.

PURPOSE: To report a case of explosive trauma to a patient with a history of radial keratotomy (RK), which resulted in multiple wound dehiscences but not globe rupture. METHODS: A 29-year-old male underwent radial keratotomy 1 year before he sustained facial trauma from a gasoline tank explosion. Corneal abrasions were treated with copious irrigation, topical antibiotics, corticosteroids, and scopolamine. He did not have a ruptured globe in either eye. RESULTS: The incision sites from radial keratotomy had evidence of anterior dehiscence in both eyes. One month following the injury, the pati

Radial keratotomy in intact epikeratoplasty graft.

BACKGROUND: Epikeratoplasty can be used to flatten the cornea in keratoconus but is not a satisfactory tool to correct refractive errors in this setting. Radial and astigmatic keratotomy are unpredictable in keratoconus but can be used in an eye which has previously undergone epikeratoplasty for keratoconus. METHODS: We report a case of a pseudophakic eye with Keratoconus which received epikeratoplasty and subsequently underwent radial and astigmatic keratotomy in the graft. RESULTS: Before the refractive keratotomy, the patient had a refraction of -8.00 + 3.25 x 25. At 5 months after radial

Visual outcome of 25 Gauge vitrectomy for acute post operative infectious endophthalmitis.

BACKGROUND: The correlation between the change in foveal thickness measured using optical coherence tomography (OCT) following surgery for infectious endophthalmitis and preoperative and postoperative visual acuity is uncertain, and there are few pertinent studies on this topic. OBJECTIVE: We explored the variations in macular thickness using OCT after emergency vitrectomy for post-cataract infectious endophthalmitis and the relationship between macular thickness with changes in visual function. METHODS: We included 10 cases of post-cataract infectious endophthalmitis. Each patient underwent

Stereoscopic acuity measurement in aphakia.

We performed a variety of tests to assess binocular function on monocular aphakic patients wearing contact lenses, monocular pseudophakic patients, bilateral spectacle-wearing aphakic patients, bilateral aphakic contact lens wearers, and bilateral pseudophakic patients. Stereoscopic acuity levels at distance and near when visual acuity was at least 6/12 (20/40) provided the most useful data. Pseudophakic patients and contact lens wearers performed equally well and outperformed spectacle wearers. Implant patients did not have superior stereoscopic acuity.

Acupuncture combined with Tuina therapy for the treatment of anisometropic amblyopia and track visual cortex activation status: A case report.

RATIONALE: Amblyopia is a common pediatric neurodevelopmental disorder of vision, necessitating early intervention to improve visual acuity. We report a case of a child with anisometropic amblyopic whose best corrected visual acuity of the right eye (OD, amblyopic eye) remained stable at 0.6 LogMAR despite 10 months of conventional occlusion therapy. We further evaluated the therapy's efficacy using functional near-infrared spectroscopy (fNIRS) to monitor visual cortex activation. PATIENT CONCERNS: A 6-year-old boy with anisometropic amblyopia came to seek Chinese medicine treatment in June 2

Risk of cardiac events after elective versus urgent or emergent noncardiac surgery: Implications for quality measurement and improvement.

INTRODUCTION: Patient populations differ for elective vs urgent and emergent surgery. The effect of this difference on surgical outcome is not well understood and may be important for improving surgical safety. Our primary hypothesis was that there is an association of surgical acuity with risk of postoperative cardiac events. Secondarily, we examined elective vs urgent and emergent patients separately to understand patient characteristics that are associated with postoperative cardiac events. METHODS: We performed a retrospective cohort study of patients ≥65 years undergoing noncardiac elect
