Sevoflurane provides better recovery as compared with isoflurane in children undergoing spinal surgery.

Rapid recovery is desirable in pediatric neurosurgical patients to obtain an early neurologic assessment. We compared the recovery characteristics of 2 commonly used anesthetic agents, sevoflurane and isoflurane, under bispectral index-guided anesthesia in children undergoing spinal surgery. Eighty children who underwent surgery for occult spinal dysraphism at the lumbar and sacral level were randomized to anesthesia with sevoflurane or isoflurane in oxygen and nitrous oxide. Anesthesia depth was guided by a bispectral index monitor kept between 40 and 60. In addition to time at emergence, ext

Caudal epidural anesthesia for a 2-year old child with congenital myasthenia gravis.

Myasthenia gravis is an autoimmune disease with antibodies directed against the acetylcholine receptor at the neuromuscular junction. Anesthetists have a special interest in myasthenia gravis because of its interaction with various anesthetic agents. Unlike adult myasthenic patients; very little report has been written about the anesthetic management in children, other than in relation to thymectomy. Although the use of caudal anesthesia in pediatric patients is common, have not seen any report concerning its use in a myasthenic child. In this case report, we represented a 2 year-old boy was p

[Use of MEOPA during nasolacrimal duct probing in children].

BACKGROUND: Congenital nasolacrimal duct obstruction affects more than 6% of newborns. Whereas some recommend nasolacrimal probing under general anesthesia from 1 year of age when the condition does not spontaneously resolve, some decide in favor of early probing from 4 to 6 months of age with topical anesthesia. We present nasolacrimal probing with inhalation of MEOPA, an equimolar mixture of nitrous oxide and oxygen, as a therapeutic alternative. PATIENTS AND METHODS: We conducted a descriptive, prospective study including 63 children (75 eyes) who had nasolacrimal duct probing with inhalat

Can Bispectral Index aid laryngeal mask placement in children?

BACKGROUND: Laryngeal mask airway (LMA) placement requires an adequate depth of anesthesia to prevent patient movement and adverse airway responses. Patient movement, coughing, stridor or laryngospasm at LMA placement may result in transient hypoxia, injury to the patient and prolong time spent in the anesthetic room. The Bispectral Index Score (BIS) is a relatively new tool that has not yet established its place in routine clinical pediatric anesthesia practice. One potential use may be to predict an adequate depth of anesthesia for successful intraoperative interventions such as LMA placemen…

Life-threatening airway obstruction as a complication to the management of mediastinal masses in children.

Life-threatening airway obstruction from large mediastinal masses in children poses a difficult diagnostic and therapeutic dilemma, requiring the close coordination of a pediatric surgeon, anesthesiologist, radiologist, and oncologist. To focus on this problem, the anesthetic and surgical management of 50 consecutive children with mediastinal masses treated between 1978 and 1984 were reviewed. Thirty children presented with respiratory symptoms; nine had life-threatening respiratory compromise with dyspnea, orthopnea, and stridor. Thirteen of these symptomatic children had marked compression o

Ultrasound-Guided Femoral Nerve Blocks.

Pediatric acute femur fractures are a relatively common major orthopedic injury seen in emergency departments. Providing adequate and safe analgesia is essential while patients await definitive management of these fractures. Opioid medications are typically used to treat fracture-associated pain but have well-known adverse effects including respiratory and central nervous system depression, pruritus, nausea, and allergic reactions. Dose titration of opioids in pediatric patients may be difficult and requires frequent nursing and physician reassessments. Regional anesthesia using ultrasound gui

Adhesive skin closure technique for closure of fasciotomy wounds in pediatric patients: a case series.

BACKGROUND: Closure of fasciotomy wounds poses a challenge, particularly in pediatric cardiac patients who are too high risk for general anesthesia and often require anticoagulant treatment. The adhesive skin closure technique enables wound closure without the need for a secondary procedure such as surgery requiring anesthesia. OBJECTIVES: This study sought to describe a treatment modality that assists in fasciotomy wound edge approximation without the need for surgery, while additionally aiding in achieving fast and aesthetic results in the aforementioned patient population. A case series of

Low-dose rocuronium as an adjuvant to remifentanil for laryngeal mask airway-assisted flexible fiberoptic bronchoscopy in children: protocol for a randomized placebo-controlled trial.

BACKGROUND: Flexible fiberoptic bronchoscopy is being increasingly carried out under general anesthesia in pediatric patients. However, the optimal anesthetic protocol still remains unclear. This study aims to test the hypothesis that the addition of low-dose rocuronium during laryngeal mask airway-assisted flexible fiberoptic bronchoscopy in children can enhance procedural outcomes and improve overall management. METHODS: In this randomized, double-blind, placebo-controlled trial, 140 children aged 1-6 years scheduled for flexible fiberoptic bronchoscopy under general anesthesia will be rand

Ambulatory pediatric ophthalmic surgery.

A retrospective analysis of 100 consecutive cases of ambulatory pediatric ophthalmic surgery was performed. The diagnoses included strabismus surgery, nasolacrimal duct probing, and excision of chalazion. The hospital stay was extended in only two cases and that was due to vomiting. This is a known side effect of halothane anesthesia and falls well within the known incidence of this occurrence. Ambulatory surgery appears to be a very viable method for short-stay surgery in children.

A prospective observational cohort study on the incidence of postoperative sore throat in the pediatric population.

BACKGROUND: Postoperative sore throat is common after general anesthesia. The incidence in pediatric anesthesia is variable, and the etiology unclear. Establishing risk factors would enable prevention and could improve quality of care. AIMS: We performed a prospective single-center cohort study aiming to establish the incidence of postoperative sore throat in children undergoing GA with an endotracheal tube or laryngeal mask airway. Secondary aims were to identify independent risk factors for sore throat and the incidence of other postoperative complications including stridor, laryngospasm, n…

Perioperative management of emergency non-cardiac surgery in a pediatric patient with Eisenmenger syndrome: a case report.

BACKGROUND: Eisenmenger syndrome (ES) is a late-stage complication of untreated congenital heart disease. It is characterized by irreversible pulmonary arterial hypertension (PAH), right-to-left shunting, and cyanosis. Pediatrics with ES who undergo emergency non-cardiac surgery are at an extremely high perioperative risk. Potential complications include pulmonary hypertension crisis (PHC), right ventricular failure, and sudden cardiac death. There is limited evidence guiding perioperative management in this population. CASE DESCRIPTION: An 11-year-old girl required emergency surgery under ge

A Pediatric Epidural Catheter Fracture Suspected to Be Caused by a Glue.

A 65-day-old baby boy underwent the Kasai procedure under general and epidural anesthesia. The epidural catheter was inserted between the T11 and T12 vertebrae under general anesthesia, and secured with sterile tape, ethyl-2-cyanoacrylate glue, and film. Intra- and postoperative epidural analgesia was effective and there was no leakage around the insertion site. On the third day post-surgery, we tried to remove the catheter but discovered it was fractured 67mm from the tip. During the ultrasound examination, we observed a hyper-echoic structure located between the laminae of T11/T12. The pedia

Assessing Caudal Block Concentrations of Bupivacaine With and Without the Addition of Intravenous Fentanyl on Postoperative Outcomes in Pediatric Patients: A Retrospective Review.

Caudal blocks are a significant and efficacious aspect of pediatric anesthesia, especially in urologic and many general surgery cases. This type of regional anesthesia is common because it has a high success rate and provides between 6 and 8 hours of postoperative pain control. The aim of this study was to determine whether the concentration of bupivacaine or the addition of intravascular (i.v.) fentanyl affected the postanesthesia care unit (PACU) discharge time. A retrospective cohort study comparing the outcomes in pediatric patients who have received varying caudal concentrations with and …
