BCG vaccination by bifurcated needle in a pilot vaccination programme.

The bifurcated needle technique for BCG vaccination was compared with intradermal injection in a mass BCG and smallpox vaccination programme in Afghanistan. In all population groups the bifurcated needle technique produced substantial tuberculin sensitivity, which however was significantly inferior to that following intradermal injection. The advantages of the bifurcated needle technique did not result in a significant increase in vaccination coverage and output. Under the present circumstances vaccination by bifurcated needle would probably be more expensive than intradermal vaccination.

Effects of sedating and nonsedating antihistamines on flying performance.

The purpose of this double-blind study was to compare the effects on flying performance of a nonsedating antihistamine, (terfenadine), two sedating antihistamines, (chlorpheniramine and diphenhydramine), and a placebo. Twelve USAF pilots were tested at 1-month intervals with the above medications, administered during separate testing periods. Medication was given twice daily for 3 days. On the third day, each pilot performed three landing approaches in a C5-B flight simulator, followed by assessment with psychological and neuropsychological tests. Evaluation of the flight data showed no signif…

Pilot Implementation of a Post-Hypertensive Disorders of Pregnancy Education and Follow-Up Package for Health Care Providers: Protocol for a Mixed Methods Pilot Study.

BACKGROUND: Medical complications of pregnancy provide a window into a woman's future health risk. Hypertensive disorders of pregnancy (HDP) affect 1 in 10 pregnant women and elevate the risk for women of experiencing long-term health complications within 5 years of the affected pregnancy, continuing lifelong. These risks include a doubled to tripled risk of developing cardiovascular disease, a doubled risk of developing type 2 diabetes, and a 5- to 10-fold risk of developing chronic kidney disease. Early assessment and intervention following HDP are therefore crucial to improving women's life

Health-related quality of life in obstructive sleep apnea patients treated with maxillomandibular advancement surgery: A cross-sectional pilot study (PILOT-QOMAS).

BACKGROUND: Patients with Obstructive Sleep Apnea(OSA) have a lower Health-Related Quality of Life(HRQoL) than individuals without OSA. Maxillomandibular Advancement(MMA) surgery effectively reduces OSA severity, which is often the focus of research. However, evaluating HRQoL seems essential to understand the treatments overall impact. STUDY DESIGN: The primary objective of this cross-sectional pilot study is to evaluate the post-operative HRQoL of patients who had MMA surgery for OSA, compared to a control group of patients who had bimaxillary surgery for facial skeletal deformity. The prima

A randomized feasibility pilot trial of hearing treatment for reducing cognitive decline: Results from the Aging and Cognitive Health Evaluation in Elders Pilot Study.

INTRODUCTION: Hearing loss (HL) is prevalent and independently related to cognitive decline and dementia. There has never been a randomized trial to test if HL treatment could reduce cognitive decline in older adults. METHODS: A 40-person (aged 70-84 years) pilot study in Washington County, MD, was conducted. Participants were randomized 1:1 to a best practices hearing or successful aging intervention and followed for 6 months. clinicaltrials.gov Identifier: NCT02412254. RESULTS: The Aging and Cognitive Health Evaluation in Elders Pilot (ACHIEVE-P) Study demonstrated feasibility in recruitme

Pilot implementation of 'outsourced oxygen to the bedside' models in five countries: a mixed methods impact assessment.

BACKGROUND: Medical oxygen is an essential medicine that is often unavailable for patients when they need it. We explored if 'outsourced oxygen to the bedside' (O2B) pilots, where private providers deliver a package of services, were successful in ensuring reliable oxygen access at the patient bedside. METHODS: We conducted a sequential explanatory mixed-methods assessment of O2B pilots in Kenya, Nigeria, India, Tanzania and Uganda from September 2024 to January 2025. A quantitative cross-sectional facility audit described facility contexts, tested equipment functionality and assessed healthc…

Exerted pressure by an in-flight oxygen mask.

BACKGROUND: F-16 pilots wear an in-flight oxygen mask to prevent hypoxemia. In the contact area with the mask, the pilots experience nasal discomfort and pain, and may even develop nasal deformities. A likely cause is the pressure exerted by the mask. In this study the hypothesis was that the change in pressure exerted on the nose would be higher than on the chin, an area free of symptoms. METHODS: Six F-16 pilots and five healthy former F-16 pilots volunteered to participate in this observational pilot study. At baseline the pilots donned mask and helmet. The median delta pressure (deltap) r…

Monitoring antioxidant defenses and free radical production in space-flight, aviation and railway engine operators, for the prevention and treatment of oxidative stress, immunological impairment, and pre-mature cell aging.

Degenerative diseases, immune impairment, and premature ageing commonly affect professional categories exposed to severe environmental and psychological stress. Among these, cosmonauts routinely experience extreme conditions due to microgravity, space radiation, altered oxygen supply, physical and mental fatigue during training, spaceflight, and post-flight. Long route aviation pilots display elevated oncogenic risk, connected with cosmic radiation overexposure, and high mortality rates for cardiovascular causes. Engine drivers, like pilots, are affected by health consequences of psycho-emotio…

Fit to Fly of Commercial Pilot after Corneal Refractive Surgery.

Corneal refractive surgery (CRS) is gaining recognition these days as a choice to correct refractive errors permanently, and the aviation community is no exception. Although studies have revealed that the number of pilots who have undergone CRS is relatively small, it is still deemed important to have knowledge about the intermediate and long-term visual outcomes after CRS, as vision is very important for pilots to ensure flight safety during their duty. This literature review elaborates the effect of CRS on contrast sensitivity, refractive regression, and dry eye, as these components are rela…

Impaired brain networks functional connectivity after acute mild hypoxia.

This study aimed to analyze the changes in brain networks functional connectivity of pilots exposed to simulated hypoxia using resting-state functional magnetic resonance imaging (fMRI). A total of 35 healthy male pilots exposed to 14.5% oxygen concentration (corresponding to an altitude of 3000 m) underwent resting-state fMRI scans. The independent component analysis (ICA) approach was used to analyze changes in the resting-state brain networks functional connectivity of pilots after hypoxic exposure, and 9 common components in brain functional networks were identified. In the functional conn…

Crashes of commuter aircraft and air taxis. What determines pilot survival?

Pilots have been recognized as an occupational group having very high work-related death rates. Commuter aircraft and air taxis are far more likely to be involved in fatal crashes than are larger airplanes. To examine the factors related to pilot survival in commuter aircraft and air taxi crashes, we analyzed National Transportation Safety Board data for the years 1983 to 1988. Case fatality rates were calculated for the pilots-in-command and odds ratios were estimated from a multivariate logistic regression model for variables related to pilots, aircraft, and circumstances. During the 6-year …

Ventricular pre-excitation.

Pilots and new applicants with asymptomatic 'safe' WPW syndrome should be allowed to fly but not as pilot in sole command. Professional pilots with tachyarrhythmia related to a pre-excitation syndrome should usually be disqualified . Personnel who have undergone successful surgical treatment by ablation of an accessory pathway could be considered for recertification to fly but drug therapy is at present unacceptable.

Repetitive high G exposure is associated with increased occurrence of cardiac valvular regurgitation.

BACKGROUND: Exposure to repeated high +Gz loads and the methods to prevent loss of consciousness cause unique stresses on the cardiovascular system. The purpose of this study was to determine if the +Gz environment is associated with an increased occurrence of valvular regurgitation in pilots of high performance aircraft. METHODS: There were 247 subjects who were divided into pilot (n = 46) and non-pilot (n = 201) groups. Pilots were defined as those individuals who had flown at least 1000 h in high performance aircraft. The echocardiographic data of these subjects were examined retrospective…
