Exploring the barriers to prehabilitation integration in Austrian healthcare: a qualitative analysis.

BACKGROUND: Prehabilitation optimises patients' physical, nutritional, and psychological readiness before major surgery, but remains largely absent from Austrian clinical practice. We aimed to identify stakeholder-perceived barriers to prehabilitation implementation and assess the readiness of the healthcare system. METHODS: An exploratory qualitative study using semi-structured interviews was conducted with eight purposively sampled stakeholders representing anaesthesiology (n=2), surgery (n=2), outpatient cardiology (n=1), clinical psychology (n=1), physiotherapy (n=1), and health insurance

Can-AP: Informing a system level model for advanced practice implementation in radiation therapy through global experience.

INTRODUCTION: This work sought insight from countries at various stages of advanced practice radiation therapy (APRT) implementation, focusing on five system-level elements: practice framework, regulatory structure, educational preparation, certification model, and role implementation. METHODS: A key informant survey targeted six national academic APRT leaders: Australia, Canada, Singapore, United Kingdom, United States, and France. Likert scale and ranking items considered the order of approach and progress in proposed elements, with additional insight sought in open-ended items. RESULTS: P

Cancer care in Lebanon and the climate-proofing of a burning phoenix.

Beirut, the capital of Lebanon, has been burnt and rebuilt seven times, earning the title of an urban phoenix. Its strategic geolocation in the heart of the Mediterranean gave the country leverage for the economy, by becoming a hotspot for trade through its different ports. Its cultural and religious diversity provided a fertile ground for businesses to grow, most importantly the banking and healthcare sector. The strategic location also put the country in the eye of the storm of different major crises, leaving the small country a crippling state, swamped with political corruption, nepotism, f

Clinical trials as disease control? The political economy of sleeping sickness in the Democratic Republic of the Congo (1996-2016).

Human African Trypanosomiasis (HAT), commonly known as sleeping sickness, is closer than ever to being eliminated as a public health problem. The main narratives for the impressive drop in cases allude to drugs discovery and global financing and coordination. They raise questions about the relationship between well-funded international clinical research and much less well-endowed national disease control programmes. They need to be complemented with a solid understanding of how (and why) national programmes that do most of the frontline work are structured and operate. We analyse archives and

The Mutual Assistance Programme of the IUATLD. Development, contribution and significance.

The Mutual Assistance Programme of the IUATLD is aimed at trying to compensate for the neglect into which tuberculosis had fallen from the part of governments, teaching institutions and international agencies, and represents an innovative approach to promote solidarity between governments and voluntary organizations of low tuberculosis prevalence countries and those of high prevalence countries. The main objective has been to develop a system of delivery of treatment and diagnosis of tuberculosis that would be efficacious even under the difficult conditions of high prevalence of tuberculosis,

HIV treatment cascade in migrants and mobile populations.

PURPOSE OF REVIEW: Health policy makers aspire to achieve an HIV treatment 'cascade' in which diagnostic and treatment services are accessed early and routinely by HIV-infected individuals. However, migrants and highly mobile individuals are likely to interact with HIV treatment programs and the healthcare system in ways that reflect their movement through time and place, affecting their successful progression through the HIV treatment cascade. We review recent research that has examined the challenges in effective and sustained HIV treatment for migrants and mobile populations. RECENT FINDIN

Building resilience in German primary care practices: a qualitative study.

BACKGROUND: In recent years, healthcare has faced many different crises around the world such as HIV-, Ebola- or H1N1-outbrakes, opioid addiction, natural disasters and terrorism attacks). In particular, the current pandemic of Covid-19 has challenged the resilience of health systems. In many healthcare systems, primary care practices play a crucial role in the management of crises as they are often the first point of contact and main health care provider for patients. Therefore, this study explored which situations are perceived as crises by primary care practice teams and potential strategie

System within systems: challenges and opportunities for the Expanded Programme on Immunisation in Pakistan.

BACKGROUND: Pakistan has one of the highest infant and child mortality rates in the world, half of these occurring due to vaccine-preventable diseases. The country started its Expanded Programme on immunisation (EPI) in 1978. However, the programme's performance is often questioned, as the Immunisation rates have been chronically low and on-time vaccination unsatisfactory. We explored the programme's insights about its structural and implementation arrangements within the larger governance system, and the ensuing challenges as well as opportunities. METHODS: We carried out a qualitative case

After the fall of the Berlin Wall: perceptions and consequences of stability and change among middle-aged and older East and West Germans.

OBJECTIVES: This study empirically tested the self-systems theory of subjective change in light of the rapid change after the fall of the Berlin Wall. The theory predicts that individuals have a tendency to perceive stability and that perceived stability exerts a strong positive effect on subjective well-being. We would expect perceptions of decline and, to a lesser extent, perceptions of improvement to be related to lower levels of subjective well-being. METHODS: Data were from respondents aged 40-85 years who participated in the German Aging Survey. We used measures of well-being and tempor

Operationalizing One Digital Health and FAIR Data Principles for Disease Surveillance in a Low-Medium Income Country in Africa: Qualitative Study in the Democratic Republic of the Congo.

BACKGROUND: Recent crises involving zoonotic diseases (Ebola, COVID-19, and Mpox) have highlighted the limitations of fragmented public health systems for the prevention and response to health emergencies at the international and continental level, and particularly in Africa, more specifically in the Democratic Republic of the Congo (DRC). In this context, the One Health (OH) approach and its extension, One Digital Health (ODH), articulated with the findable, accessible, interoperable, reusable (FAIR) principles, offer a framework for rethinking the digital transition in health in the DRC. A Codesigned Approach to Develop Tailored Antimicrobial Stewardship Strategies in Bangladesh: A Social-Ecological Systems Framework Analysis.

BACKGROUND: Multistakeholder collaboration and codesign are increasingly recognized as effective approaches for addressing complex public health challenges such as antimicrobial resistance (AMR), yet stewardship efforts in low- and middle-income countries remain fragmented and are often limited to education-focused interventions with minimal community engagement. This study used a codesign approach to develop context-specific antimicrobial stewardship (AMS) strategies for community pharmacies in rural Bangladesh, where over-the-counter antibiotic sales are common and pharmacy-led care is limit

Aging under scarcity and insecurity: chronic disease, functional limitation, and basic need material insecurity among older adults in Haiti.

BACKGROUND: Older adults in low-income and fragile states often age with intersecting burdens of chronic disease, functional decline, and material deprivation, yet they remain largely invisible in research and policy. In Haiti, recurrent political crises, violence, and weak health and social protection systems may amplify late-life inequities, but national data are scarce. We examined the distribution and social determinants of health, functioning, and material insecurity among older Haitian adults. METHODS: We analyzed data from adults aged ≥ 60 years (n = 1,408) drawn from a 2024 nationally

Designing a Standardized and Sustainable Training Framework for Healthcare Workforce Capacity Building in Punjab, Pakistan.

Background: A well-trained healthcare workforce is crucial for effective health service delivery and policy implementation. Despite the Primary and Secondary Healthcare Department's (P&SHD) efforts to improve physical infrastructure, critical gaps persist in the training system. This study aims to develop a standardized, effective, and sustainable training framework for healthcare professionals. Methods: The study was conducted from September 2024 to July 2025 using a convergent mixed-methods design. Responses were received from 26 DHDCs, representing an 83.9% response rate, with the DHDC serv

Whose diseases matter? Parasitic and viral infections in the political economy of global health research.

Global health research agendas are shaped not only by epidemiological need but also by political, economic, and institutional priorities. This study examines the relationship between disease burden and research and development investment (R&D) for parasitic and viral infectious diseases over the period 2000-2023. We integrate data from the Global Burden of Disease Study (2000-2023) and the G-FINDER database (2007-2023) to compare prevalence and cumulative R&D funding across three panels: parasitic neglected tropical diseases (NTDs), viral NTDs, and selected high‑profile viral infections. Policy Frameworks for Nursing Practice in Community-Based Palliative Care: A Four-Country Comparative Documentary Analysis.

Nurses are central to palliative care, yet their community contributions remain undervalued. This study aimed to clarify how political and professional documents conceptualize nursing autonomy in community palliative care. Using the READ (Reading materials, Extracting data, Analyzing data, Distilling findings) methodology, a comparative documentary analysis was conducted in the Netherlands, Portugal, Spain, and the United Kingdom. Sixty-eight strategic documents, identified through systematic searches and expert consultation, underwent targeted qualitative content analysis, combining deductive

"Oh, You've Come to Visit the Yard?": Phenotypic Capital, Intragroup Marginalization, and the Gated Sanctuary in Black LGBTQ+ Communities.

Identity-based communities that share common characteristics, beliefs, and experiences (e.g., Black LGBTQ+ communities) have historically been conceptualized as protective bubbles that buffer Black LGBTQ+ individuals against the deleterious effects of systemic racism and cisheterosexism. However, this monolithic narrative often masks the internal power dynamics that divide belonging. This study explores the exclusionary dynamics embedded within these safe spaces, examining how internal hierarchies of skin tone, socioeconomic status, and gender performance function as proximal stressors. Accelerating access to paediatric medicines: lessons learned from the Global Accelerator for Paediatric Formulations, a WHO-hosted network.

Despite progress in reducing neonatal and child mortality, access to age-appropriate medicines remains inequitable, particularly in low-income and middle-income countries. The Global Accelerator for Paediatric Formulations (GAP-f), a WHO-hosted network established in 2020, addresses these gaps by uniting 33 partners to promote innovation and access to child-friendly formulations. Phase 2 (2022-24) of GAP-f's work focused on therapeutic areas where innovation and access efforts often did not have stakeholder alignment and coordination of designing and implementing innovative clinical trial meth

"At the mercy of the State": Transgender and nonbinary young adults' experiences navigating restrictive healthcare legislation.

BACKGROUND: Florida Senate Bill (SB) 254 is among legislation restricting gender-affirming medical treatment (GAMT) for transgender and nonbinary (TNB) people, which has surged in the U.S. Restrictive healthcare legislation is a political determinant of health, associated with poor health outcomes for TNB people. It remains uncertain how legislation operates in healthcare experiences, especially among diverse TNB young adults. METHODS: This study involved a thematic analysis of qualitative in-depth interviews regarding healthcare access and utilization conducted with 105 TNB young adults (age
