Inpatient Sleep Testing.

Inpatient sleep testing (IST) provides a unique although underused opportunity to improve access to care by identifying patients with sleep-disordered breathing (SDB) who may benefit from timely initiation of positive airway pressure (PAP) therapy. SDB identified in hospitalized patients with cardiovascular and pulmonary comorbidities is associated with worse clinical outcomes, but initiation of PAP therapy based on inpatient testing may improve patient outcomes. Various types of IST can be performed, with portable monitoring being the most common given its accuracy, convenience, and cost-effe

[Parasomnias Excluded RBD].

Parasomnia is an undesirable physical phenomena that occurs during sleep and is classified into three groups: non-rapid eye movement (REM), … f symptoms to confirm whether the mechanism is due to non-REM sleep, REM sleep, or wakefulness with polysomnography and to make a diagnosis. It is important to explain each disease to patients and caregivers and provide treatment for each parasomnia.

Narcolepsy and rapid eye movement sleep.

Since the first description of narcolepsy at the end of the 19th Century, great progress has been made. The disease is nowadays distinguishe … recognized. The diagnostic tools have been improved, but sleep-onset rapid eye movement periods on polysomnography and Multiple Sleep Latency Test remain key criteria. The pathogenic mechanisms of narcolepsy type 1 have been partly elucidated after the discovery of strong HLA class II association and orexin/hypocretin deficiency, a neurotransmitter that is invol …

Research progress on the clinical subtyping of obstructive sleep apnea hypopnea syndrome.

Obstructive sleep apnea hypopnea syndrome (OSAHS) is a common sleep-disordered breathing condition that exhibits a notable degree of heterogeneity, a feature not fully considered in current diagnostic and therapeutic strategies. This article reviews and analyzes research progress in the subtyping of OSAHS from multiple perspectives, including clinical feature-based subtyping, comorbidity-based subtyping, polysomnography (PSG) parameter-based subtyping, and other classification approaches. Existing studies have identified common subtypes based on clinical features and clarified the characterist…

Non-rapid eye movement sleep and wake neurophysiology in schizophrenia.

Motivated by the potential of objective neurophysiological markers to index thalamocortical function in patients with severe psychiatric illnesses, we comprehensively characterized key non-rapid eye movement (NREM) sleep parameters across multiple domains, their interdependencies, and their relationship to waking event-related potentials and symptom severity. In 72 schizophrenia (SCZ) patients and 58 controls, we confirmed a marked reduction in sleep spindle density in SCZ and extended these findings to show that fast and slow spindle properties were largely uncorrelated. We also describe a no

Fragmentary Hypnic Myoclonus and Other Isolated Motor Phenomena of Sleep.

Excessive fragmentary hypnic myoclonus, hypnic jerks, hypnagogic foot tremor, alternating leg muscle activation, and sleep-related cramps are less known sleep-related motor disorders (SRMDs). These manifestations are frequently missed or misinterpreted polygraphic findings that can be frequently confused with the more frequent SRMDs. These symptoms can present as isolated motor symptoms but can be also the cause of otherwise cryptogenic insomnias and somnolence. Expanding the knowledge on these isolated symptoms and defining their polygraphic and clinical features are essential for their ident

[Why and how to diagnose sleep respiratory disorders?].

Gas exchange abnormalities occur firstly during sleep in restrictive and obstructive chronic respiratory failure. Nocturnal hypoxemia is often a revealing feature of a sleep-related hypoventilation/hypoxemia syndrome in patients who will have later a diurnal hypoxemia. On the other hand, sleep may induce breathing abnormalities in individuals without lung diseases, like in obstructive sleep apnea syndrome (OSAS). In OSAS, repeated closure and/or narrowing of the pharynx during sleep increases the inspiratory effort and induces sleep fragmentation. Intermittent hypoxemia is another consequence

Modelling of polysomnographic respiratory measurements for artefact detection and signal restoration.

Polysomnography (PSG), which incorporates measures of sleep with measures of EEG arousal, air flow, respiratory movement and oxygenation, is universally regarded as the reference standard in diagnosing sleep-related respiratory diseases such as obstructive sleep apnoea syndrome. Over 15 channels of physiological signals are measured from a subject undergoing a typical overnight PSG session. The signals often suffer from data losses, interferences and artefacts. In a typical sleep scoring session, artefact-corrupted signal segments are visually detected and removed from further consideration. T…

Central Apnoea in Infants: Apnoea of Prematurity and Apnoea of Infancy.

Central apnea is cessation of breathing resulting from insufficient respiratory drive from the central respiratory network to the muscles of respiration. In this article, we present the pathophysiology, treatment of central apnea in preterm infants using the construct of loop-gain. We also present and discuss the diagnostic criteria, pathophysiology, assessment and treatment of central apnea of infancy, and the importance of polysomnography. Lastly, we present the pros and cons of monitoring devices and techniques and algorithms used to detect and measure the frequency of central apnea in pret…

Neurodegenerative Disorders and Sleep.

Patients suffering from neurodegenerative conditions frequently report sleep complaints, such as insomnia and excessive daytime sleepiness. These symptoms are likely multifactorial, caused by their underlying neurologic disorder and also by medications and other comorbidities associated with the progressive condition. A detailed history, sleep logs, actigraphy, or polysomnography may be necessary to properly diagnosis and manage these patients. Improvement in sleep may result in improvement in neurologic symptoms and quality of life in this population. There is growing evidence that disrupted …

Obstructive sleep apnoea with co existent Kleine-Levine syndrome in a middle-aged man.

Hypersomnia is a symptom which is pathognomonic of patients with Kleine-Levin syndrome (KLS), but the cause of this finding remains undefined. Given the pervasive association between obstructive sleep apnoea (OSA) and excessive daytime sleepiness, there exists the possibility that OSA might contribute to the sleepiness exhibited by these patients. Here we report a middle age man who had excessive daytime sleepiness and diagnosed as OSA after polysomnography. Prescribing of continuous positive airway pressure (CPAP) lead to unmasking of episodes of increased sleepiness and other behavioural fea…

Pseudo Status Cataplecticus in Narcolepsy Type 1.

The rare constellation of multiple episodes of cataplexy that are refractory to therapy is called status cataplecticus. We describe a young adult with known narcolepsy type 1 who reported a marked exacerbation of her symptoms that was initially suspicious for status cataplecticus. Neurological evaluation, polysomnography, and additional history revealed that the spells were not consistent with cataplexy, but rather with a somatoform disorder. The case highlights the importance of considering psychosomatic factors in narcolepsy type 1 when presumed cataplexy remains suboptimally controlled desp…

Snoring. The not so silent partner.

Snoring, widespread in this country, has important social and interpersonal ramifications. It may be a symptom of serious associated disorders, or it may be a health hazard in and of itself. The pathophysiology is related to loose vibrating tissue in the pharynx and is often associated with obesity. The assessment of snoring may include polysomnography if symptoms suggest air flow disruption and/or sleep fragmentation. Treatment options include behavioral therapeutic measures as well as surgical approaches. It is important to differentiate primary snoring from OSA and other sleep-related breat…

[Sleep disturbances from the internist's viewpoint].

Sleep disorders are common and can be caused by somatic diseases. Chronic pain as well as heart and lung conditions are able to interfere with the normal sleep pattern. Obstructive sleep apnea is caused by a deranged central control of breathing occurring during sleep and leads to a sleep-fragmentation and consequently to daytime hypersomnolence. A problem oriented meticulous history can give valuable insights and distinct diagnostic considerations form the basis for an eventually successful therapeutic approach. Polysomnography is done in particularly puzzling cases and for evaluation of inva…

Epilepsy and sleep-disordered breathing as false friends: a case report.

Because signs of nocturnal seizures can overlap with sleep respiratory events, clinicians can have difficulty distinguishing abnormal events related to sleep disorders from epileptic seizures. We describe the case of a 3-year-old child presenting with ictal electroencephalographic (EEG) activity associated with a particular form of atypical obstructive sleep apnea, characterized by increased respiratory rate, paradoxical breathing, desaturations, and tonic-dystonic posture associated with movement artifacts. Following cardiorespiratory polysomnography, the patient was initially misdiagnosed as…
