18F-FDG PET/CT findings in fatal Balamuthia Mandrillaris encephalitis in brain stem: A case report.

We presented a case of a 66-year-old female whose initial symptom was headache without obvious inducement. The patient's condition progressed rapidly to a semi-coma state after symptomatic treatment. The 18F-FDG PET/CT scan revealed circular FDG hypermetabolism and central metabolic defect of the pons and left frontal lobe lesions. The combination of clinical findings, MRI, and Metagenomic next-generation sequencing (NGS) of cerebrospinal fluid led to the diagnosis of Balamuthia mandrillaris encephalitis. The patient died 5 days after discharge.

MRI findings in the young infant with brainstem disconnection and extracerebral features. Report of one case and review of the literature.

We present the young infant with the extremely rare brain abnormality-brainstem disconnection. Additionally, several extracerebral abnormalities were diagnosed: bilateral anotia, micrognatia, hypertelorism, scoliosis, ribs and vertebral anomalies. MR brain examination precisely demonstrated absence of the pons, with disruption between midbrain and hypoplastic medulla oblongata. The thin strands connecting the medulla with the midbrain and medulla with both cerebellar hemispheres were revealed. The large hamartoma of the tuber cinereum was found. In this study we review case reports published p…

Unilateral hearing disturbance could be an isolated manifestation prior to ipsilateral anterior inferior cerebellar artery infarction.

A 50-year-old man presented with a sudden onset of right-sided hearing disturbance. His hearing disturbance improved gradually, however, dysarthria, right-sided facial weakness and dysesthesia, and gait disturbance was developed 11 days after the onset of hearing disturbance. MR imaging revealed fresh infarctions of the right dorsolateral pons and middle cerebellar peduncle localized in the territory of anterior inferior cerebellar artery. Unilateral hearing disturbance could be an isolated manifestation prior to ipsilateral anterior inferior cerebellar artery infarction.

Wall-Eyed Monocular Internuclear Ophthalmoplegia (WEMINO) with Contraversive Ocular Tilt Reaction.

Wall-eyed monocular internuclear ophthalmoplegia (WEMINO) with contraversive ocular tilt reaction has not been previously reported. A 71-year-old woman suddenly developed blurred vision. Examination revealed left internuclear ophthalmoplegia, left exotropia, right hypotropia, and rightward head tilt. Magnetic resonance imaging showed a tiny infarction at the area of the left medial longitudinal fasciculus in the upper pons. WEMINO with contraversive ocular tilt reaction may be caused by a paramedian pontine tegmental infarction that selectively involves the medial longitudinal fasciculus.

[Diagnosis of extrapontine myelinolysis preceding central pontine myelinolysis].

An MR study performed on a patient who presented a tetraplegia after liver transplantation, revealed bilateral hypersignal of internal globus pallidus on T2 weighted images. Diagnosis of extrapontine myelinolysis was proposed based on clinical presentation and lesions topography, although absence of natremic disorders. Confirmation of diagnosis was obtained on an MR study performed a month later, which demonstrated a pontine extension. Extrapontine myelinolysis can precede pontine involvement. Diagnosis of myelinolysis should be evoked although MR normal appearance of the pons.

MRI of the brain in muscle-eye-brain (MEB) disease.

Muscle-eye-brain (MEB) disease belongs to the spectrum of rare congenital syndromes with migration disorders of the brain and muscular dystrophy, along with the Walker-Warburg syndrome and Fukuyama congenital muscular dystrophy. Their features overlap, and differential diagnosis presents some difficulties. We examined the brain of 10 patients with MEB using high-field MRI and found a uniform pattern consisting of a pachygyria-type cortical migration disorder, septal and corpus callosum defects and severe hypoplasia of the pons in 7 of them.

Regional changes in brain histamine levels following dietary-induced thiamine deficiency in rats.

Histamine levels in thiamine deficient rats were significantly lower in the hippocampus, amygdala, olfactory bulb, thalamus and pons-medulla oblongata than those of normal and pair-fed groups. In the case of the hypothalamus, thiamine deficiency produced a significant increase in histamine levels. These changes observed in the thiamine deficient group were reversed to the normal levels by supplying the normal diet. These data present a new finding that thiamine deficiency affects the central histaminergic neuron system as well as other monoaminergic systems.

Subcortical arteriosclerotic encephalopathy: brain stem findings with MR imaging.

Magnetic resonance images of 44 patients who had varying degrees of supratentorial signal abnormalities compatible with subcortical arteriosclerotic encephalopathy (SAE) were reviewed for posterior fossa findings. Brain stem lesions frequently accompanied supratentorial SAE and were seen as multiple, fairly symmetric areas of poorly defined, increased signal intensity on axial T2-weighted images. Involvement was generally confined to the central portions of the mid and upper pons. The typical appearance of brain stem involvement by SAE and its probable pathogenesis are reviewed.

The effects of acute and chronic morphine on regional distribution of cardiac output in brain.

Both acute and chronic administration of morphine resulted in an increase in the percent cardiac output received by brain. However, various brain regions were affected differently by the drug treatments. The greatest increases in percent cardiac output received after chronic administration of morphine occurred in pons and cerebellum, while the greatest increases after acute administration occurred in cortex and midbrain. The changes found are in contrast with earlier studies which suggest that morphine has no effect on cerebral blood flow.

Visual and auditory evoked potentials in a case of Marchiafava Bignami Disease.

Brainstem auditory evoked potentials and visual evoked potentials to both flash and pattern reversal stimulation were recorded in a case of clinically diagnosed Marchiafava Bignami Disease (M.B.D.) A CT scan revealed dilation of the supratentorial ventricular system and fairly extensive cortical atrophy. The brainstem potentials showed a non-specific abnormality of the N4 component, (in the region of the caudal pons) and were inconclusive. The visual evoked potentials to flash stimulation were delayed while pattern reversal responses were within normal limits.

[Hydrocephalus due to a large aneurysm in the basilar artery (author's transl)].

In the light of the literature the authors discuss the case of a 66 year-old hypertensive patient presenting headaches, disorders of gait and deglutition, dysphonia and urinary incontinence. A CAT scan showed dilatation of the entire ventricular system. Anatomo- pathological investigation showed a voluminous bisaccular aneurysmal malformation in the basilar artery. By deforming the pons and the medulla oblongata, the aneurysm had compressed the fourth ventricle and cerebral aqueduct, thus causing hydrocephalus. (Acta neurol. belg., 1980, 80, 80-85).

Ipsilateral hemiparesis and contralateral lower limb paresis caused by anterior cerebral artery territory infarct.

Ipsilateral hemiparesis is rare after a supratentorial stroke, and the role of reorganization in the motor areas of unaffected hemisphere is important for the rehabilitation of the stroke patients. In this study, we present a patient who had a subclinical remote infarct in the right pons developed ipsilateral hemiparesis and contralateral lower limb paresis caused by a new infarct in the left anterior cerebral artery territory. Our case suggests that the motor areas of the unaffected hemisphere might be reorganized after stroke, which is important for the rehabilitation of stroke patients.

Bispectral index monitoring in the management of sedation in an intensive care unit patient with locked-in syndrome.

Locked-in syndrome is an extremely rare neurological state caused by injury of the ventral pons. The syndrome is characterized by quadriplegia and anarthria with concomitant preservation of cortical function. When a reversible underlying pathological abnormality is identified and managed aggressively, meaningful recovery is possible. Because patients retain consciousness throughout their illness, a dependable method for titrating sedation may improve their quality of life. The case presented suggests that bispectral index monitoring may be a cost-effective and reliable method for managing seda…

Pontine tegmentum hematoma. Report of a case with pure hemiplegia.

The authors report the case of a 50 year-old hypertensive male patient with a pontine hematoma. The clinical presentation was characterized by pure pyramidal deficit signs (no other signs or symptoms were present). A pure hemiplegia syndrome, although common in supratentorial lesions, is considered to be a rare event in pontine vascular lesions. The pathophysiologic mechanisms of these neurological findings are unclear. The exclusive involvement of the pyramidal tract in this case is likely due to a variation in the vascular anatomy of the pons but, in some cases, a vascular malformation may b…

Wallenberg's syndrome caused by a craniopharyngioma "en plaque".

Wallenberg's syndrome is the clinical manifestation of the obliteration of arterial blood vessels supplying the dorsolateral part of the medulla oblongata, the posterior inferior cerebellar artery being involved in most cases. A patient is described in whom the typical features of Wallenberg's syndrome correlated with cystic necrosis in the perfusion area of the posterior inferior cerebellar artery. A craniopharyngioma spreading along the pons and involving the posterior inferior cerebellar artery, the basilar artery, and its branches was found to be responsible for the infarction of the dorso…

Regional distribution of pyridoxal-5'-phosphate in developing and mature brains and its depletion in pyridoxine deficiency.

A study was made of (a) the distribution of the coenzyme pyridoxal-5'-phosphate (PLP) in four discrete regions of developing and mature rat brains and (b) the effect of dietary pyridoxine deficiency on the distribution. There was an increase in PLP concentration of all the regions from infancy to adulthood. Highest concentration of PLP was found in the medulla and pons in both infants and adults. Pyridoxine deficiency resulted in a more marked reduction of PLP in all regions of the neonatal brain as compared with those in the mature brain. This is consistent with the vulnerability of the devel…
