Intracranial pressure monitoring in Reye's syndrome.

The neurologic damage caused by rapidly rising intracranial pressure can be halted or reversed only if specific therapy is instituted early. The key to such successful management is continuous monitoring by either subarachnoid bolt or ventricular cannula. Indications and techniques for monitoring are outlined, as is a ventricular cannula. Indications and techniques for monitoring are outlined, as is a step-by-step approach to management of intracranial hypertension as it develops.

Current status of pressure chamber treatment.

Forty-five patients with unilateral and one patient with bilateral Meniere's disease were exposed to underpressure in a pressure chamber during an acute attack that was primary in 24 and recurrent in 23. The patients were instructed to avoid active equilibration during reduction of the pressure, in order to induce an overpressure in the middle ear. Treatment was promptly followed by improvement of hearing in 20 patients (21 ears) and by a remission for more than three years in seven cases. No side effects were observed. We therefore believe that this procedure may be useful in the prevention o…

Psychosocial factors associated with pressure sores.

To determine if psychosocial factors, not purely mechanical aspects, play an important part in the incidence of pressure sores, 141 subjects were tested on 3 psychosocial measures. Results indicate that differences exist among groups in pressure sore history and psychosocial measures. Contrary to the expectations of a mechanical skin problem model, quadriplegics as a group (rather than paraplegics) had a history of fewer pressure sores. Two variables, the subject's responsibility in skin care (RESPON) and his satisfaction with the activiities of life (SATIS), were significantly associated with…

Psychotic symptoms in normal-pressure hydrocephalus.

Two patients with a psychiatric history of about 20 years, and clinical and neuroradiological signs of normal-pressure hydrocephalus (NPH) are reported. One had a periodic psychosis subsequent to a tuberculous meningitis, and this overshadowed the slight classical symptoms of NPH. She had received at least 120 treatments with electro-convulsive therapy. The second patient suffered from a paranoid psychosis; other signs of NPH were moderate though progressive. Both patients showed definite improvement of their NPH symptoms after ventriculo-atrial shunting, and psychotic symptoms ceased totally.…

Tissue pressure to evaluate compartmental syndrome.

This article presents a simplified, direct way of measuring tissue pressure and shows that in a series of 45 patients with serious extremity injuries, it is possible to predict the need for fasciotomy, eliminating much of the risk to the patient.

Detrusor pressure in bladder outlet obstruction.

Synchronous urinary flow and pressure studies were done on 51 male adults consiting of 12 normal subjects and 39 patients with varying degrees of bladder outlet obstruction due to benign prostatic hypertrophy. Mean maximum detrusor pressure levels of 50 mm. Hg and above were found to be indicative of bladder outlet obstruction. Elevated mean maximum detrusor pressure fell preoperatively within normal limits after prostatecomy. Chronic retention of urine was not associated with poor contraction of the detrusor muscle. Clinical findings of obstruction were shown to be inadequate for estimation o…

Diabetes mellitus in normal pressure hydrocephalus.

Seventeen of 33 patients (51.5%) with normal pressure hydrocephalus were discovered to have diabetes mellitus. This was significantly greater than the 12.1% incidence found in age-matched control subjects. The diabetes was not accounted for either by the patients' ages nor by their physical inactivity due to hospitalisation. Diabetes mellitus concurrent with normal pressure hydrocephalus may result from involvement of hypothalamic and brainstem autonomic structures by the expanding ventricles during the evolution of hydrocephalus.

Normal pressure hydrocephalus: a case presentation.

There are presently no successful medical treatments available for most of the chronic dementias. For this reason it is most important to diagnose potentially reversible syndromes such as Normal Pressure Hydrocephalus and to determine if that patient is a candidate for neurosurgical treatment. A typical clinical case exemplifying the most common features of this syndrome is presented. This is followed by a discussion of the physiology, etiology, signs and symptoms, methods of diagnosis, and treatment of this illness. Material for this discussion was obtained from personal clinical experience a…

Spontaneous positive end-expiratory pressure (sPEEP).

Spontaneous positive end-expiratory pressure (sPEEP) is a simple, low-cost method of providing low to moderate levels of PEEP as an alternative to continuous positive airway pressure (CPAP), whether used alone or with continuous positive pressure ventilation (CPPV) in intermittent mandatory ventilation (IMV). The therapeutic effects of sPEEP therapy are demonstrated in 7 adult respiratory distress syndrome patients. All patients, initially hypoxemic on a high F1O2, had an acceptable PaO2 (with F1O2 less than or equal to 0.4) within 12 hours of initiation of sPEEP therapy. sPEEP, unlike CPAP an…

Intracranial subarachnoid pressure monitoring in children.

A technique for subarachnoid monitoring of intracranial pressure in children is presented. This is based on a subarachnoid bolt that can be inserted under local anesthesia. The advantages of continuous recording of intracranial pressure in certain conditions such as encephalitis, diffuse encephalopaties, and Reye's syndrome are discussed.

Neurogenic bladder in normal pressure hydrocephalus.

Five patients wíth normál pressure hydrocephalus who had urinary incontinence were found to have neurogenic bladders by cystometry. The bladder disturbance in this disorder whould be considered the result of a specific defect of brain function and not just and artifact of gait disturbance or of dementia.
