Prison Buprenorphine Implementation and Postrelease Opioid Use Disorder Outcomes.

IMPORTANCE: Agonist medications for opioid use disorder (MOUD), buprenorphine and methadone, in carceral settings might reduce the risk of postrelease opioid overdose but are uncommonly offered. In April 2019, the Massachusetts Department of Correction (MADOC), the state prison system, provided buprenorphine for incarcerated individuals in addition to previously offered injectable naltrexone. OBJECTIVE: To evaluate postrelease outcomes after buprenorphine implementation. DESIGN, SETTING, AND PARTICIPANTS: This cohort study with interrupted time-series analysis used linked data across multipl

Incidence and Predictors of COVID-19 Infection in Prison Healthcare Workers.

OBJECTIVE: The aim of the study is to assess the predictors of SARS-CoV-2 infection among correctional healthcare workers (HCWs). METHODS: We conducted a retrospective chart review to describe the demographic and workplace characteristics of New Jersey correctional HCWs between March 15, 2020, and August 31, 2020, using univariate and multivariable analysis. RESULTS: Among 822 HCWs, patient-facing staff had the highest incidence of infection (7.2%). Associated risk factors include being Black and working in a maximum-security prison. There were few statistically significant findings due to s

Tuberculosis Outbreak in a State Prison System - Washington, 2021-2022.

During 2014-2020, no tuberculosis (TB) cases were reported within the Washington state prison system. However, during July 2021-June 2022, 25 TB cases were reported among persons incarcerated or formerly incarcerated in two Washington state prisons. Phylogenetic analyses of whole genome sequencing data indicated that Mycobacterium tuberculosis isolates from all 11 patients with culture-confirmed TB were closely related, suggesting that these cases represented a single outbreak. The median infectious period for 12 patients who were considered likely contagious was 170 days. As of November 15, 2…

Caring for people in prison with palliative and end-of-life care needs.

PURPOSE OF REVIEW: The prison population is growing and ageing, and many people will die from natural causes while incarcerated. This article provides a contemporary review of key issues related to palliative and end-of-life care in prisons. RECENT FINDINGS: Few countries have integrated prison hospices. Palliative care needs may go unrecognised in prison. Older offenders may not trust the prison to care for them and may benefit from segregation. Cancer remains a major cause of death. Training staff remains a priority, and technology can help facilitate this. The coronavirus disease 2019 (COV…

A Scoping Review of Nutrition Education Interventions Applied in Prison Settings.

PURPOSE OF REVIEW: Over 11 million individuals are incarcerated globally, facing health challenges such as obesity, diabetes, and cardiovascular disease, possibly exacerbated by prison diets. The objective of this scoping review is to explore the various available types of nutrition education interventions that currently exist in the literature applied in prison settings. RECENT FINDINGS: Utilizing the PRISMA Extension for Scoping Reviews, 19 databases were searched for studies on nutrition interventions in prisons from 2000 to May 2023, guided by the "PICO" structure. Inclusion criteria enco…

Characterization of pharmaceutical services in the prison system: cross-sectional study.

This study aimed to characterize pharmaceutical services (PS) in the prison system of Rio Grande do Sul in terms of organization and infrastructure in the context of primary health care. This is a descriptive study, with a cross-sectional design carried out between August and September 2022. Data collection was carried out remotely, using a questionnaire prepared and adapted based on the Pesquisa Nacional sobre Acesso, Utilização e Promoção do Uso Racional de Medicamentos no Brasil (PNAUM - Brazilian National Survey on Access, Use and Promotion of Rational Use of Medications in Brazil). Lactating women and the perception of useful value for breastfeeding in a prison environment.

The aim is to unveil the useful value of breastfeeding for lactating women in a prison environment, based on Max Scheler's axiological perspective. This work was a qualitative, developed in a prison unit in Rio de Janeiro, where seven lactating women were interviewed. The phenomenological interview was used for data collection, while Laurence Bardin's content analysis was used for data analysis and treatment. The value revealed, in light of Max Scheler's findings, was the useful value, which was detected through the needs and wants experienced by lactating women during the act of breastfeeding

Epidemiology of TB in prisoners: a metanalysis of the prevalence of active and latent TB.

BACKGROUND: Tuberculosis (TB) in prisons usually occurs at higher rates than in the general population, especially in developing countries. TB has been reported as the most common cause of death among prisoners. Studies have shown limitations for early detection of TB in prisons that seem to result from mistaken concepts about TB, delayed diagnosis mainly due to the naturalization of lack of healthcare for this population METHODS: A scoping review was performed using the methodology of the Joanna Briggs Institute to assess "What are the scientific evidences on the epidemiology of TB in the pri…

High Failure Rates in Prisoners Undergoing Spine Fusion Surgery.

OBJECTIVE: The medical literature on prisoner health care is limited, despite data showing that prisoners experience high rates of physical and mental health challenges. We compared clinical outcomes for prisoners undergoing spine fusion with comparable nonincarcerated patients and determined what factors were implicated in differences in outcomes. METHODS: Prisoners who underwent spinal fusion in 2011-2021 were retrospectively compared with an age-, sex-, and procedure-matched 3:1 control group of nonincarcerated spinal fusion patients. Fusion failure was confirmed by lack of bridging bone b

Elimination of hepatitis C virus in a prison: An 18-year experience.

INTRODUCTION: Despite the decrease of hepatitis C in Spanish prisons in the last years, it still remains a reservoir for infection. The aim of this work is to analyze the characteristics of these patients and the response to antiviral treatment over the last 18 years. METHODS: Retrospective observational study in inmates of Araba penitentiary center diagnosed with HCV infection between 2002 and 2020. A descriptive analysis of patient characteristics and the response to the three antiviral treatment modalities was performed: peg-interferon and ribavirin, peg-interferon, ribavirin and a first-g…

NHS Health Check Programme: a qualitative study of prison experience.

BACKGROUND: NHS Health Checks began in England in 2009 and were subsequently introduced into English prisons. Uptake has been patchy and there is limited understanding about factors that may limit or enhance uptake in prison settings. Uptake of this programme is a key policy in reducing the risk of cardiovascular disease and death in these settings. METHOD: Semi-structured focus groups were conducted with groups of prisoners (attendees and non-attendees to the health check), prison healthcare staff, custodial staff and ex-prisoners (n = 50). Participants were asked about their awareness and e…

The Cultural Hybridization of Mothering in French Prison Nurseries: A Qualitative Study.

In France, women can be incarcerated during pregnancy and can keep their babies with them in prison up to the age of 18 months. The small number of nurseries in France and their unequal geographic distribution as well as the high percentage of foreign prisoners often result in women's isolation from their usual cultural environment. Family members and cultural community play a crucial role in the process of mothering. The aim of this study is to explore through these mothers' narratives how they experience the cultural aspects of this process in the prison environment. We conducted semi-struct

Do Prisoners With Reintegration Needs Receive Relevant Professional Assistance?

Ex-prisoners often experience barriers to successful transition regarding employment, finances, housing, healthcare, and valid identification. Based on the Offender Management framework, assistance during imprisonment by prison- and community-based professionals is considered key in preparing prisoners for release regarding these reintegration needs. Therefore, the current study examines the degree to which prisoners with reintegration needs are assisted by relevant professionals. We used self-reported data from 4,309 prisoners of the Dutch Prison Visitation Study, part of the Life in Custody
