[Carcinoma-associated retinopathy in breast carcinoma and carcinoid tumor].

BACKGROUND: Carcinoma-associated retinopathy (CAR) is a rare paraneoplastic syndrome characterized by diffuse retinal photoreceptor degeneration in the presence of an epithelial tumor. We report on three patients, who developed paraneoplastic retinopathy in the presence of breast carcinoma and a cervical carcinoid tumor. MATERIALS AND METHODS: In addition to biomicroscopic, psychophysical, electrophysiological and angiographic examinations, serum samples were obtained for immunohistochemical staining of human retina. RESULTS: Ring-shaped visual field defects with statokinetic dissociation an

A quantitative model of the "effective" signal processing in the auditory system. I. Model structure.

This paper describes a quantitative model for signal processing in the auditory system. The model combines a series of preprocessing stages with an optimal detector as the decision device. The present paper gives a description of the various preprocessing stages and of the implementation of the optimal detector. The output of the preprocessing stages is a time-varying activity pattern to which "internal noise" is added. In the decision process, a stored temporal representation of the signal to be detected (template) is compared with the actual activity pattern. The comparison amounts to calcul

[Cetyltrimethylammonium naproxenate as vaginal lavage in the therapy of vulvovaginal dystrophies in the postmenopause].

A peculiar aspect of the postmenopausal period is atrophy of the lower genital tract, vagina and vulva. This is due to estrogen deficiency and is accompanied by vaginal pH elevation, lower number of lactobacilli and increased incidence of aspecific vulvovaginitis. We tested a new vaginal douche with an antibacterial-anti-inflammatory (non antibiotic-nonsteroidal) principle, cetyltrimethylammonium naproxenate 0.223% as an adjunct to classical estrogen topical replacement therapy. We treated 34 menopausal patients (mean age 67 yrs) in a double parallel controlled (15 A only estrogen topical ther

[Psycho-functional and objective tests applied to early diagnosis of POAG, indications for general ophthalmologists and therapeutic incidences].

The early diagnostic of glaucoma (before the appearance of visual field defects) can be made in some cases with the help of certain objective tests and, to a lesser degree, with subjective or psychophysical tests. Retinal nerve fiber layer photography is certainly the most rewarding objective test, providing the technique has been mastered and there is a clear fundus view without chorio-retinal atrophy. The identification of nerve fiber defects is an almost sure sign of glaucomatous damage in the absence of other pathology. An abnormally small neuroretinal rim area can also be an indicator of

Visual loss in multiple sclerosis and its relation to previous optic neuritis, disease duration and clinical classification.

Visual function was investigated in a group of 58 clinically classified cases of multiple sclerosis (MS). Psychophysical measures of luminance and chromatic threshold sensitivity and temporal contrast sensitivity were undertaken, together with visual evoked potentials and Bjerrum screen perimetry. The patient group was divided on the basis of optic neuritis (ON), clinical disease duration and clinical classification. A comparison of the results of all visual measures suggested a nonuniform loss of function in the patient group without ON and a more consistent loss of function in the group with

Tonometry in question: can visual screening tests play a more decisive role in glaucoma diagnosis and management?

Chronic open-angle glaucoma is a disease in which characteristic changes occur in optic nerve morphology and in ganglion cell function. The utility of intraocular pressure readings in the diagnosis and ongoing management of glaucoma patients is pragmatically considered in the light of the availability of potentially more sensitive means of glaucoma monitoring. Conservative appraisal of the literature suggests that traditional clinical tonometry has low diagnostic sensitivity and specificity. The majority of new cases of glaucomatous visual field loss are most likely to arise from the normotens

Electrophysiological measures of cochlear function in guinea pigs with long-term endolymphatic hydrops.

Experimental endolymphatic hydrops was induced in guinea pigs by obliteration of the endolymphatic sac and duct. From 3 to 24 months after this operation, cochlear action potential (AP) audiograms and AP tuning curves were measured. The purpose of this study was to establish parallels, if any, between this supposed animal model of Menière's disease and the auditory symptoms of the disease in man. In some animals, low and middle frequency AP threshold elevations were observed whilst higher frequency regions maintained normal sensitivity. Other animals developed flat or very gradually sloping AP

Some practical considerations in development of multichannel scala tympani prostheses.

There are several basic issues regarding the safety and feasibility of implanting multichannel scala tympani cochlear prostheses in humans. Physiological and technical studies have been designed to resolve some of these questions. Results of physiological investigations demonstrate that (1) nerve viability is not affected by the presence (without activation) of a multichannel array; (2) use of a highly specified bipolar electrode configuration permits discrete and predictable stimulation of sectors of the auditory nerve array, and (3) some deleterious effects (i.e., nerve damage and calcificat

Sensory and Choice Responses in MT Distinct from Motion Encoding.

The macaque middle temporal (MT) area is well known for its visual motion selectivity and relevance to motion perception, but the possibility of it also reflecting higher-level cognitive functions has largely been ignored. We tested for effects of task performance distinct from sensory encoding by manipulating subjects' temporal evidence-weighting strategy during a direction discrimination task while performing electrophysiological recordings from groups of MT neurons in rhesus macaques (one male, one female). This revealed multiple components of MT responses that were, surprisingly, not inter

Assessment of outcome measures after audiological computer-based auditory training in cochlear implant children.

OBJECTIVE: To validate the clinical use of acoustic change complex (ACC) as an objective outcome measure of auditory training in Egyptian cochlear implant (CI) children and explore how far electrophysiological measures could be correlated to behavioral measures in assessing training outcome. Also to explore the efficacy of computer-based auditory training programs (CBATP) in the rehabilitation process of CI children. METHODS: Sixty Arabic speaking children participated in the present study. Forty children using monaural CI device served as study group (20 children in subgroup A and 20 childre

Domain-general and domain-specific computations in single word processing.

Language comprehension relies on a multitude of domain-general and domain-specific cognitive operations. This study asks whether the domain-specific grammatical computations are obligatorily invoked whenever we process linguistic inputs. Using fMRI and three complementary measures of neural activity, we tested how domain-general and domain-specific demands of single word comprehension engage cortical language networks, and whether the left frontotemporal network (commonly taken to support domain-specific grammatical computations) automatically processes grammatical information present in infle

Interactions of flicker and motion.

We present a series of novel observations about interactions between flicker and motion that lead to three distinct perceptual effects. We use the term flicker to describe alternating changes in a stimulus' luminance or color (i.e. a circle that flickers from black to white and visa-versa). When objects flicker, three distinct phenomena can be observed: (1) Flicker Induced Motion (FLIM) in which a single, stationary object, appears to move when it flickers at certain rates; (2) Flicker Induced Motion Suppression (FLIMS) in which a moving object appears to be stationary when it flickers at cert

The impact of pre-existing anxiety on affective and cognitive processing of a Virtual Reality analogue trauma.

Dysfunctional processing of traumatic events may be in particular related to high trait anxiety as a pre-traumatic risk factor for the development of post-traumatic stress disorder (PTSD). However, as this has rarely been investigated in prospective, experimental studies, we aimed to analyse the association between high trait anxiety and affective as well as cognitive processing of stress using a new prospective Virtual Reality analogue trauma paradigm to overcome limitations of retrospective or current analogue designs. Individuals with high and low trait anxiety (N = 80) were exposed to a mu

Interaction between articulatory gestures and inner speech in a counting task.

Interaction between covert and overt orofacial gestures has been poorly studied apart from old and rather qualitative experiments. The question deserves special interest in the context of the debate between auditory and motor theories of speech perception, where dual tasks may be of great interest. It is shown here that dynamic mandible and lips movement produced by a participant result in strong and stable perturbations to an inner speech counting task that has to be realized at the same time, while static orofacial configurations and static or dynamic manual actions produce no perturbation.

Impaired processing of 3D motion-defined faces in mild cognitive impairment and healthy aging: an fMRI study.

Mild cognitive impairment (MCI), which shows high risk for conversion to Alzheimer's disease (AD), is accompanied by progressive visual deteriorations that so far are poorly understood. Here, we compared dorsal and ventral visual stream functional magnetic resonance imaging (fMRI) activity among amnestic MCI, healthy elderly, and young participants during structure-from-motion (SFM) face categorization performance. Task performance varied with stimulus depth and duration levels and differences among groups were highly correlated with face-related fMRI activation patterns. Young participants sh
