Cerebral air embolism from angioinvasive cavitary aspergillosis.

Background. Nontraumatic cerebral air embolism cases are rare. We report a case of an air embolism resulting in cerebral infarction related to angioinvasive cavitary aspergillosis. To our knowledge, there have been no previous reports associating these two conditions together. Case Presentation. A 32-year-old female was admitted for treatment of acute lymphoblastic leukemia (ALL). Her hospital course was complicated by pulmonary aspergillosis. On hospital day 55, she acutely developed severe global aphasia with right hemiplegia. A CT and CT-angiogram of her head and neck were obtained demonstr

Acute and chronic pulmonary thromboembolism: current perspectives. Part I: Glossary of terms, historic evolution and prevalence.

Despite considerable medical as well as surgical advances, the problem of pulmonary thromboembolism, both acute and chronic, continues to plague clinicians of all medical disciplines. Approximately 650,000 to 700,000 patients per year suffer from acute pulmonary thromboembolism in the United States. Of these, approximately 60,000-70,000 die as a direct result of it. Our knowledge of the etiology, pathology, pathogenesis of pulmonary infarction, pathophysiology, accurate diagnosis, proper management (including both primary and secondary prophylaxis) of deep vein thrombi and pulmonary thromboemb…

[Drepanocytosis and diffuse interstitial pulmonary fibrosis (author's transl)].

Respiratory disorders in homozygous drepanocytosis and double SC heterozygosis are mainly dependent on two factors : repeated infections with, more particularly, pneumococcus or mycoplasma, and epidoses of occlusion of the pulmonary circulation. Mutual reinforcement of these two factors occurs, in so far as the relative hypoxia of an infected lung increases the risk of falciform and thrombosis formation in the pulmonary arterioles. The particular physical and chemical properties of the drepanocyte red cell, and the anaemia, themselves lead to parallel disturbances in pulmonary circulation func

Successful treatment of bilateral multiple pulmonary arteriovenous malformations in a patient with brain abscess and severe hypoxemia using a combination of transcatheter embolotherapy and surgical resection: a case report.

BACKGROUND: A pulmonary arteriovenous malformation is an abnormal dilated blood vessel that makes direct communication between a pulmonary artery and pulmonary vein and can be associated with hypoxemia or neurological complications, including brain abscess and cerebral infarction. Treatment of pulmonary arteriovenous malformation includes surgical resection and transcatheter embolotherapy, however the adaptation of therapies should be considered when a patient is in bad condition. CASE PRESENTATION: A 51-year-old man was admitted after developing fever, consciousness disorder, and hypoxemia.

Multiple organ manifestations in thromboangiitis obliterans (Buerger's disease). A case report.

Thromboangiitis obliterans (TAO) occurs almost exclusively in young male smokers. Its involvement of the small and medium-sized arteries and veins leads to ischemic complaints and/or changes in the extremities. The possibility of organ involvement is a matter of controversy. The authors report a case of TAO with multiple organ involvement, including myocardial, splenic, and cerebral infarctions; pulmonary embolisms; and probable intestinal ischemia during a twenty-three-year course.

A case report of a triple patch reconstruction of a post-infarction ventricular septal rupture: microaxial flow pump support as a bridge to surgery.

BACKGROUND: Ventricular septal rupture is a rare but devastating complication of acute myocardial infarction, frequently associated with cardiogenic shock and poor prognosis. Data regarding successful treatment strategies remain limited, and operative mortality significantly decreases when surgery can be delayed by 7-14 days. CASE SUMMARY: We present the case of a 61-year-old patient diagnosed with a posterior transmural myocardial infarction complicated by a large ventricular septal rupture. Right heart catheterization showed significant right ventricular (RV) overload, with elevated right a

Characteristic lesion pattern and echocardiographic findings in extra-cardiac shunt-related stroke.

Among embolic strokes of undetermined source, under-recognized etiology such as extra-cardiac shunt could be a potential risk factor. We sought to characterize infarction patterns on diffusion-weighted imaging (DWI) and transesophageal echocardiography (TEE) findings in extra-cardiac shunt-related stroke. We enrolled 96 consecutive patients with cryptogenic stroke who had an extra- or intra-cardiac shunt. Diagnosis of the shunt was performed using TEE with agitated saline contrast and pulmonary vein isolation. Infarction patterns on DWI and total lesion volume were analyzed. Bubble amounts thr

A rare case of sudden death due to thrombus from the internal iliac vein.

A 40-year-old healthy housewife died after joining a special summer camp for her diabetic daughter. The victim sat for 4h 40 min while traveling by car on the day before her death. Autopsy revealed she died from a pulmonary thromboembolism (PTE), whereby portions of a blood-clot that had been formed in the internal iliac veins detached and traveled to the lungs, blocking the pulmonary blood flow. The leg veins are the most common source of thromboembolism; PTE caused by thromboemboli from the internal iliac veins is rare. We discuss the mechanisms of the present victim's death, and the conditi

A Case of Postoperative Wernicke Encephalopathy Mimicking Cerebellar Infarction Complicated by Postoperative Nausea and Vomiting: A Critical Diagnostic Pitfall for Thoracic Surgeons.

INTRODUCTION: Wernicke encephalopathy (WE) is a potentially life-threatening neurological disorder caused by a thiamine deficiency, most commonly associated with alcoholism or malnutrition. Although its occurrence after gastrointestinal surgery has been increasingly recognized, WE following thoracic surgery remains extremely rare and is often underrecognized by thoracic surgeons. Given that neurological symptoms of WE can mimic those of cerebral infarction, a timely diagnosis is challenging, especially when complicated by postoperative nausea and vomiting (PONV). CASE PRESENTATION: We report

Right ventricular involvement in transmural inferior wall infarction: two-dimensional echocardiographic and clinical correlations.

We used two-dimensional echocardiography to assess asynergy in the right (RV) and left (LV) ventricles in 34 selected patients with electrocardiographic evidence of acute transmural inferior myocardial infarction (IMI) 40 +/- 3 (SE) h from the onset of pain. We measured the extent of asynergy, defined as akinesis and/or dyskinesis, for RV and LV in 4 short axis sections and computed asynergy for both ventricles. The incidence of asynergy was 100% in the inferior LV, 76% in the inferior ventricular septum and 76% in the inferior RV. We found a direct correlation between peak creatine phosphokin

Intravascularly infused extracellular matrix as a biomaterial for targeting and treating inflamed tissues.

Decellularized extracellular matrix in the form of patches and locally injected hydrogels has long been used as therapies in animal models of disease. Here we report the safety and feasibility of an intravascularly infused extracellular matrix as a biomaterial for the repair of tissue in animal models of acute myocardial infarction, traumatic brain injury and pulmonary arterial hypertension. The biomaterial consists of decellularized, enzymatically digested and fractionated ventricular myocardium, localizes to injured tissues by binding to leaky microvasculature, and is largely degraded in abo

Cardiovascular risk factors for acute stroke: Risk profiles in the different subtypes of ischemic stroke.

Timely diagnosis and control of cardiovascular risk factors is a priority objective for adequate primary and secondary prevention of acute stroke. Hypertension, atrial fibrillation and diabetes mellitus are the most common risk factors for acute cerebrovascular events, although novel risk factors, such as sleep-disordered breathing, inflammatory markers or carotid intima-media thickness have been identified. However, the cardiovascular risk factors profile differs according to the different subtypes of ischemic stroke. Atrial fibrillation and ischemic heart disease are more frequent in patient

Case report: necrosis of the anterolateral papillary muscle--an unusual mechanical complication of myocardial infarction.

We report the case of a 66-year-old woman with no significant past medical history who presented to the Emergency Department at Houston Methodist Hospital with 24 hours of chest pain. An electrocardiogram was done, an electrocardiogram confirmed a posterolateral ST elevation myocardial infarction. An immediate and successful percutaneous coronary intervention of a totally occluded ramus intermedius was performed. Six hours later she developed pulmonary edema, cardiogenic shock, severe acidosis, and anuria. Echocardiography showed severe mitral regurgitation due to a ruptured anterolateral papi

[Echocardiography in emergency diagnostics].

Echocardiography plays an important role in emergency medicine because this non-invasive method is universally available and provides crucial diagnostic findings for acute decision making. The cardiac etiology in the presence of acute chest pain, acute dyspnea, hemodynamic instability or shock, new heart murmurs, chest trauma, peripheral embolism and cardiac arrest can be determined by echocardiography in the emergency scenario. The analysis of left ventricular function documents myocardial ischemia and myocardial infarction. Analysis of right ventricular function documents right heart infarct

Myocardial infarction in a young athlete with non-obstructive hypertrophic cardiomyopathy and normal coronary arteries.

We describe an acute myocardial infarction in a young athlete with non-obstructive hypertrophic cardiomyopathy (HCM) and normal coronary arteries. A 18 year old athlete came to our attention for dispnoea and chest pain during effort. Echocardiography showed non-obstructive HCM, with restrictive physiology. Cardiac catheterization and coronary angiography showed elevated pulmonary artery pressure and normal coronary arteries. He was discharged on sotalol and diuretics. Nine months later, he was resuscitated from a cardiac arrest during mild effort. The ECG revealed a high rate of atrial fibrill

Comparative effects of morphine, meperidine and pentazocine on cardiocirculatory dynamics in patients with acute myocardial infarction.

The cardiocirculatory effects of the commonly used parenteral analgesics, morphine sulfate 15 mg, meperidine hydrochloride 100 mg and pentazocine 60 mg, each administered intravenously, were compared in 12 patients with acute myocardial infarction during cardiac catheterization and by echocardiogram. No untoward hemodynamic effects occurred following the administration of morphine or meperidine. In contrast, pentazocine produced significant (p less than 0.01 to less than 0.05) increases in systemic and pulmonary arterial pressures, left ventricular filling pressure, systemic vascular resistanc
