Contemporary Trends in Pulsed Field Ablation for Cardiac Arrhythmias.

Pulsed field ablation (PFA) is a catheter-based procedure that utilizes short high voltage and short-duration electrical field pulses to induce tissue injury. The last decade has yielded significant scientific progress and quickened interest in PFA as an energy modality leading to the emergence of the clinical use of PFA technologies for the treatment of atrial fibrillation. It is generally agreed that more research is needed to improve our biophysical understanding of PFA for clinical cardiac applications as well as its potential as a potential alternative energy source to thermal ablation mo…

Intense pulsed light therapy for ocular surface diseases.

Intense pulsed light (IPL) is a non-laser, high-intensity light source that has been shown to play a valuable role in dermatology and has been adopted in ophthalmology for treating meibomian gland dysfunction (MGD). In this review, we discuss the mechanism of action of IPL, including its benefits in ophthalmology. IPL therapy has been shown to improve tear film stability, meibomian gland (MG) function, and subjective symptoms of ocular dryness in MGD patients. Moreover, emerging evidence suggests that IPL therapy is beneficial for other ocular surface diseases, such as blepharitis and chalazia…

Self-healing solitonic slip pulses in frictional systems.

A prominent spatiotemporal failure mode of frictional systems is self-healing slip pulses, which are propagating solitonic structures that feature a characteristic length. Here, we numerically derive a family of steady state slip pulse solutions along generic and realistic rate-and-state dependent frictional interfaces, separating large deformable bodies in contact. Such nonlinear interfaces feature a nonmonotonic frictional strength as a function of the slip velocity, with a local minimum. The solutions exhibit a diverging length and strongly inertial propagation velocities, when the driving …

Modeling pulsativity in the hypothalamic-pituitary-adrenal hormonal axis.

A new mathematical model for biological rhythms in the hypothalamic-pituitary-adrenal (HPA) axis is proposed. This model takes the form of a … ltradian oscillations in an isolated pituitary gland. These observations suggest that principles of pulse modulated control, familiar to control engineers, may have an increasing role to play in understanding the HPA axis.

Pulsed Radiofrequency of the Vagal Nerve for Tinnitus -A Case-Study.

TINNITUS BACKGROUND: Vagal nerve stimulation is a promising therapy for tinnitus. Pulsed radiofrequency is a minimally invasive neuromodulator technique that could be applied repeatedly without damage to neural tissue. In this case-study we document the use of pulsed radiofrequency of the vagal nerve as a successful therapy without harm for patients with tinnitus. METHODS: A 22-gauge, 60 mm-long needle with a 5 mm active tip was placed percutaneously at the inner side of the tragus. The auricular branch of the vagal nerve was subjected to pulsed radiofrequency at 42 V, 2 Hz, and 10 millisecon…

Diplomats' Mystery Illness and Pulsed Radiofrequency/Microwave Radiation.

Importance: A mystery illness striking U.S. and Canadian diplomats to Cuba (and now China) "has confounded the FBI, the State Department and US intelligence agencies" (Lederman, Weissenstein, & Lee, 2017). Sonic explanations for the so-called health attacks have long dominated media reports, propelled by peculiar sounds heard and auditory symptoms experienced. Sonic mediation was justly rejected by experts. We assessed whether pulsed radiofrequency/microwave radiation (RF/MW) exposure can accommodate reported facts in diplomats, including unusual ones. Observations: (1) Noises: Many diplomats …

A Review of Low-Intensity Pulsed Ultrasound for Therapeutic Applications.

Ultrasound therapy has a long history of novel applications in medicine. Compared to high-intensity ultrasound used for tissue heating, low-intensity ultrasound has drawn increasing attention recently due to its ability to induce therapeutic changes without biologically significant temperature increase. Low-intensity pulsed ultrasound (LIPUS) is a specific type of ultrasound that delivers at a low intensity and outputs in the mode of pulsed waves. It has minimal thermal effects while maintaining the transmission of acoustic energy to the target tissue, which is able to provide noninvasive phys…

Intense pulsed light for evaporative dry eye disease.

There is a clear association between dry eye disease (DED) and skin inflammatory diseases occurring in close proximity to the eyelids, such as facial skin rosacea. Intense pulsed light (IPL) is widely accepted as a treatment for skin rosacea. A number of recent studies demonstrated that, in patients suffering from meibomian gland dysfunction (MGD), IPL therapy also reduces signs and symptoms of DED. Despite these encouraging results, in the context of DED and MGD, the mechanisms of action of IPL are not well understood. The purpose of this review was to raise the potential mechanisms of action…

[Laser and intense pulsed light management of couperose and rosacea].

Management of couperosis and rosacea has been totally renewed by laser and vascular laser techniques, with efficacy targeted on the telangiectases and to a lesser extent on the erythrosis. Laser management of hypertrophic rosacea or rhinophyma depends on surgical treatment with decortication, continuous CO(2) ablative laser or Erbium, fractionated at high power, then vascular laser treatment for the telangiectases: lasers with pulsed dye, KTP, or pulsed lights for red laser telangiectases and long pulse Nd-Yag laser for blue telangiectases. For papulopustular rosacea, vascular laser treatment …

[Segmental Darier disease : treatment with pulsed dye laser].

Darier disease is often associated with pruritus and an unpleasant odor, causing medical and emotional problems. Ablative laser therapy has proven effective in ameliorating these symptoms. Side effects of this approach include permanent hypopigmentation and a risk of scarring. We present two cases where non-ablative therapy with pulsed dye lasers proved a safe and effective way to manage the intertriginous lesions. Although the mechanism of action is unclear, our success indicates that pulsed dye laser therapy is an option in Darier disease. Larger numbers of patients, ideally in multicenter s…
