Radiation therapy for Cushing's disease: a review.

Because patients with Cushing's disease have an increased morbidity and an age-corrected mortality, treatment is generally started as soon as possible. The goal of treatment in these patients is to induce remission. Although a variety of treatments are available, pituitary radiation is a good option for aggressive Cushing's disease that fails to respond to surgery, disease that invades the cavernous sinus, and disease that relapses following an initial remission. Conventional radiation therapy, stereotactic radiosurgery, fractionated stereotactic radiation therapy, and brachytherapy with Yttri…

[Ionizing radiation and health: data and objectives].

Three dimensions must be considered to analyze the consequences of exposure to ionozing radiation on public health: assessment of exposure, measure of the public health risk, and monitoring the perception of risk by the population. We reviewed these three dimension for several situations of exposure to ionizing radiation: exposure related to the Chernobyl accident, occupational exposure, military-related exposure and medical exposure. For each situation, we discuss the pertinence of available data an monitoring systems used in France, and propose when necessary new systems. An extension of the…

Hematologic consequences of exposure to ionizing radiation.

From the early 1900s, it has been known that ionizing radiation (IR) impairs hematopoiesis through a variety of mechanisms. IR exposure directly damages hematopoietic stem cells and alters the capacity of bone marrow stromal elements to support and/or maintain hematopoiesis in vivo and in vitro. Exposure to IR induces dose-dependent declines in circulating hematopoietic cells not only through reduced bone marrow production, but also by redistribution and apoptosis of mature formed elements of the blood. Recently, the importance of using lymphocyte depletion kinetics to provide a "crude" dose e…

Photoprotection of the eye - UV radiation and sunglasses.

Although most health scientists now agree that health risks to the skin (e.g., skin cancer) exist from exposure to the ultraviolet radiation in sunlight, a scientific consensus has not really been achieved vis-à-vis sunlight and ocular health. A growing number of scientists warn of hazards to the eye if ultraviolet radiation - and perhaps even shorter wavelength visible radiation also - is not filtered by lenses. Despite a substantial literature on the adverse effects of ultraviolet radiation (UVR) and intense light upon ocular structures, particularly upon the retina, controversy still surrou…

Characterizing the phenomenon of radiation recall dermatitis.

Radiation recall represents the 'recalling' of an effect similar in appearance to that of an acute radiation reaction in a previously irradiated field. The recall is triggered by the administration of certain drugs days to years after the exposure to ionizing radiation. This review focuses almost exclusively on the skin manifestations of radiation recall to assemble the largest data base upon which to discuss this rare phenomenon. No absolute radiation dose threshold is apparent, but rather an interplay between dose and time before drug exposure seems to affect both the risk and speed of onset…

[Optical radiation and its effect on the skin].

Optical radiation ranges from short wave ultraviolet via visible light to the far infra-red. Both natural and artificial light sources may cause desired and undesired effects, both on the skin and on the organism as a whole. The nature and extent of such effects depend primarily on the energy of the photons, the absorption capacity of the chromophores, the mean depth of penetration, and on the density and dose of the radiation. The present paper discusses the mechanisms of action and desired and undesired effects in the organism, including therapeutic applications, separately for ultraviolet r…

A rat model for radiation-induced proctitis.

Radiation proctitis is a frequent acute complication encountered with pelvic irradiation. This study was aimed at establishing the optimal radiation dose for radiation-induced proctitis in rats. Female Wistar rats were used. The rectal specimens were examined morphologically at 5th and 10th day following 10-30 Gy irradiation in single fraction. With increasing dose, mucosal damage became worse, and there was a prominent reaction after > or =15 Gy. We selected 17.5 Gy as an optimal dose for radiation proctitis and examined specimens at day 1-14 and at week 4, 6, 8, and 12 after 17.5 Gy. The rec…

[Osteogenesis in exposure to ionizing radiation in vitro].

Irradiation is a well established therapeutical concept to prevent heterotopic ossification after joint replacement. The influence of irradiation on proliferation of mature osteoblasts and their potential osteoprogenitors, matrix formation and mineralization are not well known in this setting. We therefore studied the effect of different doses of ionizing irradiation on the several steps of osteogenesis in vitro, using cells isolated from the juvenile rat. A colony forming test, the MTT-viability assay, a cell count, measurement of the cellular protein content and alkaline phosphatase activity…

Radiation therapy for benign disease of the orbit.

Benign diseases of the orbit can cause significant impairment of visual function through direct effects on the eye or supporting orbital tissues. Persistent moderate-to-severe inflammatory symptoms, diplopia, and visual loss may prompt therapeutic intervention. Low doses of external-beam irradiation (20 Gy in 2-Gy fractions) have an efficacy equivalent to corticosteroid medications in the treatment of both Graves' ophthalmopathy and orbital pseudotumor, with response rates of 50% to 80%. Appropriate patient selection, coordination with other medical subspecialties, and careful treatment planni…

Surgical management of patients with radiation enteritis.

To improve the surgical outcome in patients with radiation enteritis, 18 female cases were analyzed. Out of the 18 cases, 3 (16.7%) were treated conservatively while 15 (83.3%) underwent surgical procedures. Thirteen out of 14 patients (92.8%) with ileus underwent an operation. The overall mortality was 22.2% (4 out of 18 cases). However, no significant difference in the mortality between the operated and non-operated cases was observed. Although an analysis of the 15 operated cases did not reveal any significant factors that might have affected the prognosis, all four patients who underwent a…

Risks of radiation exposure to orthopaedic surgeons.

The risks of occupational radiation exposure for orthopaedic surgeons has not been well documented. The use of fluoroscopy in orthopaedics has reduced morbidity and by reducing operative time and by reducing the invasiveness of procedures. Increasingly procedures requiring the use of fluoroscopy are being developed. In a prospective study we have monitored cumulative radiation exposure over a 6-month period. The exposure levels are well below the maximum dose limits for ionizing radiation as recommended by European Economic Communities EURATOM directives. Experimentally using a phantom patient…

[Mechanism of the radiation-protective effect of indralin].

Pharmacological test demonstrated that radioprotective activity of indralin occurs by interaction with alpha-adrenoreceptor. Radioprotective effect of indralin decreased by alpha-adrenoblocker, aminazine and theophylline. Normobaric hyperoxia during irradiation reduced radioprotective effect of indralin in doses about ED50. In experiment with mice and rats it was shown that indralin induced acute hypoxia, impaired oxygen consumption and heat production by 30-46%, spleen bloodflow to 26.3% of control level, rectal temperature by 1.5-2 degrees C (mouse). After 30-min indralin raised resistance o…

In vivo radiation sensitivity of glioblastoma multiforme.

PURPOSE: Human glioblastoma (GBM) is one of the most resistant tumors to radiation. In previous reports, we have demonstrated a wide range of radiation sensitivity of GBM in vitro; that is, SF2 values of 0.2 to 0.8. The great sensitivity of some of the cell lines is not in accord with the almost invariably fatal clinical outcome of patients with GBM. The sensitivity of cells in vitro pertains to cells cultured in optimal nutritional conditions. The TCD50 (the radiation dose necessary to control 50% of the tumors locally) determined in lab animals is analogous to the use of radiation with curat…

[Radiation risks in dental and medical radiography].

Biological effects of exposure to ionizing radiation in diagnostic dental and medical radiology are stochastic effects such as carcinogenesis and genetical effects. Today, radiation exposure due to an x-ray or nuclear medicine examination varies from less then 0.001 mSv to more then 10 mSv effective dose. The background level of natural radiation in the Netherlands is about 2 mSv. Assuming that an appropriate indication for the examination exists, it can be stated that generally the diagnostic information obtained outweighs by far the risks involved in the exposure to radiation.
