[Case report. Decompression of a periapical lesion of great size: an unconventional approach].

Decompression of a large periapical lesion: a modified approach. Management of large periapical lesions that resist conventional root canal therapy or that are interpreted as cystic in nature is somewhat questionable. Surgery with complete enucleation might be the most expeditious method of management. This approach however may create certain undesirable complications as compromising the vitality of adjacent teeth, jeopardizing their osseous support or resulting in nerve damage. These complications should be avoided whenever possible. A decompression technique can eliminate the disadvantages o

[Iatrogenic foreign bodies within the maxillofacial region].

Five hundred different X-ray pictures of the maxillofacial region were analysed. Foreign bodies such as dental filling materials and instruments were found. It was stated that most often the excess of endodontic material was observed in the region of a treated tooth. The second site where iatrogenic foreign bodies were found was the maxillary sinus. The reason of these complications was the lack of proper radiological and endometric diagnosis before the beginning of the endodontic treatment.

Hyaline ring granuloma: a case report with histochemical and polarized microscopic studies.

An unusual case of hyaline ring granuloma arising from the connective tissue wall of a residual apical periodontal cyst is reported. Hyaline rings with foreign-body type giant cell inclusions were a significant feature in the histopathologic diagnosis. Histochemical and polarized light microscopic studies suggested that the introduction of foreign material, possibly cellulose, through the postextraction socket into the cystic wall may have caused the granulomatous reaction and formation of the hyaline ring granuloma. The clinical features of this case and 66 previously reported cases, as well

Suspension mechanism of subperiosteal implants in baboons.

It has been noted that overloading of an implant can lead to the premature loss of implants (Jones et al., 1979; Travis and Jones, 1986). An understanding of the mechanism by which subperiosteal implants support masticatory loads could lead to appropriate design changes for best utilization of this mechanism and thus increase the predictability of subperiosteal implants (James, 1983). This study attempts to analyze the suspension mechanism in implants recovered from a baboon after 10 years' use.

Melanoma with metastasis to an apical periodontal cyst.

A rare case of multiple oral foci of metastatic melanoma in a 23-year-old man is presented. A unique aspect is the metastasis of a malignant neoplasm to the wall of an apical periodontal cyst.

Concentration of phenoximethylpenicillin, clindamycin and tinidazole in dental cysts.

In the treatment of bacterial infections, adequate antibiotic concentrations must be realised at the site of infection. Levels of 3 antibiotics--phenoximethylpenicillin, clindamycin and tinidazole - were measured in serum and fluid of periapical odontogenic cysts in 30 patients undergoing enucleation of periapical cysts. After a single dose of 500 mg tinidazole, the concentration of tinidazole in the cystic fluid was 4.3 micrograms/ml, which was about 1/3 of the concentration in the serum. No measurable levels were achieved in the cystic fluid with phenoximethylpenicillin or clindamycin.

[Dental aspects of atypical neuralgia in the maxillofacial region (author's transl)].

The complicated anatomy of the masticatory system with its coordinated functional elements offers a multitude of noxae for atypical neuralgias. More specifically, affections of the pulp, apical periodontopathies with cyst formation, impacted teeth, aggressive osteopathies and malignant growth come into consideration. In addition, dysfunctional unions must be borne in mind. The styloid syndrome also deserves attention. Because of this causal complex the discovery of other puzzling painful conditions needs dental investigation.

Effect of decalcified freeze-dried bone allograft on the healing of jaw defects after cyst enucleation.

PURPOSE: This study radiographically evaluated the changes that occur in jaw defects after enucleation of cysts and either grafting with demineralized freeze-dried bone allograft (DFDBA) or packing with absorbable gelatine sponge (Gelfoam). PATIENTS AND METHODS: Thirty-two patients (14 males, 18 females) with cystic lesions of the jaws were divided into two groups. Group A underwent enucleation and bone grafting with DFDBA. Group B underwent enucleation and packing with Gelfoam. Both groups were evaluated radiographically before surgery, immediately postoperatively, and 6, 12, 24, and 36 mont

Lumbar Epidural Steroid Injection

Lumbar epidural steroid injections (LESIs) are minimally invasive, image-guided procedures used to treat lumbosacral radicular pain by depositing corticosteroid, with or without local anesthetic, into the epidural space. Radicular pain results from inflammation, irritation, or compression of a lumbar or sacral nerve root. This pain commonly presents in the low back and radiates into the buttock or lower extremity in a dermatomal distribution. Lumbar disc herniation is the most common cause; degenerative lateral recess or foraminal stenosis, spondylolisthesis, postoperative epidural fibrosis, a

Clinical, radiographic, and histologic study of endodontic treatment failures.

One hundred fifty cases of endodontic treatment failures were studied clinically, radiographically, and histologically. Fifty-seven percent … ing sinus tract was often associated with stainable bacteria inside the canal. The development of a radicular cyst associated with an endodontically treated tooth that has failed is not necessarily the cause of endodontic treatment failure.

Radiographic and histological evaluation of persistent periapical lesions associated with endodontic failures after apical microsurgery.

AIM: To determine the histology of persistent periapical lesions associated with nonsurgical endodontic treatment failures and to compare ra … rson's chi-square test. RESULTS: The 93 specimens consisted of 72% periradicular granulomas; 21.5% radicular cysts, including two keratocysts; 4.3% abscesses; and 2.2% scar tissue. Cystic prevalence increased as the size of the periapical lesion increased; however, there was no correlation between the presence of a radiopaque lamina and histological diagnosis o …

Anterior endoscopic treatment of a huge anterior sacral meningocele: technical case report.

OBJECTIVE AND IMPORTANCE: Anterior endoscopic treatment of a huge anterior sacral meningocele, a rare example of spinal dysraphism, is described. CLINICAL PRESENTATION: A 43-year-old woman presented with severe headache and fever, increasing abdominal pain, and a several-year history of right lower-extremity radicular pain. She exhibited meningeal irritation signs and Currarino's triad. Lumbosacral myelograms and magnetic resonance imaging scans revealed a huge anterior sacral cyst connected with the spinal subarachnoid space. INTERVENTION: A straight rigid endoscope was introduced into the

Mesenchymal Stem Cell Marker Expression in Periapical Abscess.

INTRODUCTION: There is evidence that acute periapical lesions present a greater potential for cyst formation. Recently, it was found that these lesions have cells with characteristics of pluripotent stem cells, which may influence cyst development. However, a more complete phenotype investigation of stem cells in a specific sample of periapical abscesses is required. The aim of this study was to analyze the immunohistochemical expression of mesenchymal stem cell (MSC) markers in periapical abscesses and to evaluate differences in their expression in relation to acute and chronic periapical les
