[Pediculated deltoid muscle flap: an alternative for coverage of chronic radionecrotic lesions in the shoulder region].

Soft tissue lesions in the clavicular region may be the result of trauma, infections or oncological resection and necessitate plastic surgery coverage. A case of an 85-year-old woman is presented with non-union of the mid-portion of the left clavicle with an overlying skin defect and a brachial plexus lesion after radiation therapy for breast cancer. The left arm was functionless so after partial resection of the medial part of the clavicle coverage of the defect was conducted by a proximally pediculated anterior part of the deltoid muscle with a split thickness skin graft.

Development of a porcine skin injury model and characterization of the dose-dependent response to high-dose radiation.

A porcine skin model was developed to characterize the dose-dependent response to high-dose radiation. The dorsal skin of a mini pig was divided into four paraspinal sections, with 11 small irradiation fields (2 cm × 2 cm) in each section, and a single fraction of 15, 30, 50 or 75 Gy was delivered to each section using a 6 MeV electron beam. A spectrophotometer measured gross skin changes, and a biopsy for each radiation dose was performed in the 1st, 2nd, 4th, 6th and 9th weeks for histology, immunostaining with anti-CD31, and western blotting with IL-6 and TGF-β1 to determine the degree of s

Fluoroscopic sentinel events in neuroendovascular procedures: how to screen, prevent, and address occurrence.

Radiation-induced skin injury during fluoroscopic procedures has been recently addressed by The Joint Commission, which defined prolonged fluoroscopy resulting in a cumulative peak skin dose of ≥15 Gy to a single field as a sentinel event (FSE). Neuroendovascular procedures can be associated with a high radiation skin dose and present risks such as potential FSEs. Managing these risks is the responsibility of the interventional neuroradiologist. In this review, we discuss hospital policies needed for screening and preventing FSEs, methods for minimizing radiation-induced skin injury, and actio

The use of errorless learning strategies for patients with Alzheimer's disease: a literature review.

The aim of this article was to review the evidence of errorless learning on learning outcomes in patients with early-stage Alzheimer's disease. A computer-aided literature search from 1999 to 2011 was carried out using MEDLINE, Cumulative Index to Nursing and Allied Health Literature (CINAHL), PsycINFO and PsycArticles. Keywords included 'errorless learning or practice' and 'Alzheimer's disease'. Four studies that fulfilled the inclusion criteria were selected and reviewed. Two of the studies were clinical controlled trials: one was a single-group pretest-post-test trial and the other was a mu

The emergence of supportive oncodermatology: the study of dermatologic adverse events to cancer therapies.

Cancer therapies have led to remarkable results due to improved toxicity profiles and effects on survival. While these medical, surgical, and radiation protocols are chiefly responsible for these noteworthy contributions, an unexpected constellation of toxicities has emerged. Most notably, dermatologic adverse events have gained considerable attention, due to their high frequency, visibility, and impact on physical and psychosocial health, all of which affect dose intensity and possibly clinical outcome. Consequently, increased attention to cutaneous health in oncology has resulted in supporti

Severe radiation dermatitis in patients with locally advanced head and neck cancer treated with concurrent radiation and cetuximab.

OBJECTIVES: A phase III trial has shown the superiority of concurrent cetuximab and radiotherapy versus radiotherapy alone for the treatment of locally advanced head and neck cancer (HNC). We evaluated our institution's experience of patients treated with concurrent cetuximab and radiotherapy to determine the rate of serious (≥ grade 3) radiation dermatitis. We also sought to more fully describe and characterize the grade 4 dermatitis that these patients develop. METHODS: We performed a retrospective review of HNC patients who were treated with concurrent cetuximab and radiation. We included

Treatment of radiation-induced dermatitis with light-emitting diode (LED) photomodulation.

BACKGROUND AND OBJECTIVE: Light-emitting diode (LED) photomodulation increases dermal collagen and reduces inflammation. This study evaluated the use of LED photomodulation in the prevention of radiation-induced dermatitis in breast cancer. MATERIALS AND METHODS: Patients (n=19) were treated with LED photomodulation (Gentlewaves, Light BioScience, LLC, Virginia Beach, VA) after each of a series of intensity-modulated radiation treatments (IMRT). Skin reactions were monitored weekly with National Cancer Institute (NCI) criteria. Age-matched controls (n=28) received IMRT without LED photomodula

Pain inhibition with pneumatic skin flattening (PSF) in permanent diode laser hair removal.

INTRODUCTION: Permanent laser-based hair removal is normally associated with acute pain. Pneumatic skin flattening (PSF) is a new technology which activates tactile and pressure skin receptors to naturally block the transmission of pain to the brain during laser-based aesthetic treatments (gate theory of pain control). OBJECTIVE: Efficacy and pain reduction was evaluated compared with controls for the treatment of hair removal with PSF and a high-energy diode laser (LightSheer, Lumenis). METHODS: Ten patients were treated for hair removal with the LightSheer at energy densities of 26-30 J/cm

Gemcitabine-related "pseudocellulitis": report of 2 cases and review of the literature.

Gemcitabine is a chemotherapeutic agent whose cutaneous toxicities are easily mistaken for infections. We describe 2 patients, 1 with gemcitabine-induced radiation recall dermatitis and 1 with gemcitabine-related erysipeloid reaction, who received misdiagnoses of infectious cellulitis and were given empirical antibiotics. Recognition of these syndromes is important to avoid unnecessary antibiotic use.

Management of radiation dermatitis in a patient after mastectomy.

Women who are diagnosed with breast cancer and undergoing chemotherapy and radiation are at high risk of developing acute radiation dermatitis. The purpose of this case study is to explore an alternative topical therapy for skin toxicity in the post-radiation care of a patient with a history of breast cancer. The patient, a 54-year-old white female, was treated by modified radical mastectomy, chemotherapy, and radiation. During post-radiation therapy the patient developed wet desquamation reaction over the midincision line into the right axilla. Balsam Peru, hydrogenated castor oil, trypsin (X

Radiation recall dermatitis in a patient treated with dacarbazine.

Radiation recall describes an inflammatory reaction at a previously irradiated site associated with the use of chemotherapeutic agents. Dacarbazine, a tetrazine cytotoxic drug, has not been noted to cause this phenomenon. We report the case history of a 44-year-old female patient who developed a recall dermatitis due to dacarbazine in a site previously irradiated for the treatment of malignant melanoma. The skin erythema responded quickly to oral corticosteroid treatment. Further cycles of dacarbazine were facilitated with oral corticosteroid premedication. We conclude that dacarbazine should

Reduction of radiation exposure in the cardiac electrophysiology laboratory.

The purpose of this study was to determine the effects of various protective measures on patient and operator radiation dose levels in catheter ablation procedures. Catheter ablation procedures are associated with significant radiation levels. The patient's skin and operator radiation levels were measured (1) at baseline, (2) after primary beam filtration by 0.3-mm copper sheet and 2-mm aluminium plate and implementation of the LocaLisa system, and (3) after reduction of the left anterior oblique fluoroscopic pulse rate and installation of a lead glass screen. Additionally, a comparative analy
