Displaced distal end radius fractures in children treated with Kirschner wires - A systematic review.

The indications for Kirschner wiring, the technique of wiring, type of cast immobilization, period of immobilization and complications of K wires are unclear. We conducted a systematic review of the literature on Kirschner wiring of distal radius fractures in children. A total of 4263 articles were identified. The full text of the remaining 78 articles was reviewed. 64 articles were finally excluded because of incomplete data leaving 14 for analysis. Complete fracture displacement and translation more than 50% are the commonest indications for Kirschner wiring of these fractures with 2 retrogr…

Minimally invasive percutaneous plate osteosynthesis for distal radius fractures with long-segment metadiaphyseal comminution.

INTRODUCTION: Distal radius fractures with both metaphyseal and diaphyseal comminution are commonly encountered injuries due to high-energy trauma. However, effectively treating patients with this disease remains challenging for the surgeon. HYPOTHESIS: The goal of this study was to evaluate the outcomes of minimally invasive percutaneous plate osteosynthesis (MIPPO) technique for distal radius fractures with long-segment metadiaphyseal comminution. MATERIAL AND METHODS: Nine patients with distal radius fractures involving long-segment metadiaphyseal comminution were treated with MIPPO from …

Predicting redisplacement after manipulation of paediatric distal radius fractures: the importance of cast moulding.

INTRODUCTION: The majority of displaced distal radius fractures are managed by closed reduction and cast immobilisation. Redisplacement is associated with initial displacement, imperfect reduction and quality of cast. The aim of this study was to establish which factors predict the risk of redisplacement. MATERIALS AND METHODS: A retrospective analysis between September 2010 and April 2013 of children who underwent closed manipulation and cast immobilisation for a distal third radius fracture was performed. Open fractures, those treated with fixation, and cases with associated dislocations or…

Randomized comparison of volar locking plates and intramedullary nails for unstable distal radius fractures.

PURPOSE: To compare minimally invasive intramedullary nails (IMN) and volar locking plates (VLP) for the treatment of unstable distal radius fractures by evaluating postoperative subjective, radiographic, and functional outcomes. The hypothesis was that IMN patients would have less pain and required less pain medication in the early postoperative period and returned to work earlier than VLP patients. METHODS: Sixty patients with closed, displaced, unstable, extra-articular, metaphyseal fractures of the distal radius were randomized to receive a VLP or an IMN for internal fixation. Functional …

No Difference in Adverse Events Between Surgically Treated Reduced and Unreduced Distal Radius Fractures.

OBJECTIVES: To determine if closed reduction is worthwhile for the subset of patients who choose operative treatment before attempted reduction of their distal radius fracture. We hypothesize that there are no differences in (1) adverse events and (2) subsequent surgeries between patients treated with manipulative reduction compared with those that were splinted without reduction. DESIGN: Retrospective cohort study. SETTING: Three affiliated urban hospitals in a single city in the United States. PATIENTS/PARTICIPANTS: One thousand five hundred eleven consecutive adult patients who underwent

Predicting radiographic changes at the first visit following operative repair of distal radius fractures.

PURPOSE: To test the hypothesis that clinical assessment reliably identifies patients with radiographic changes (including loss of reduction, hardware failure, and hardware migration) at the initial visit following operative repair of distal radius fractures. METHODS: We identified 102 patients undergoing operative repair of distal radius fractures. Radiographs and clinical notes were reviewed. RESULTS: At the initial postoperative visit, 11 patients had more than normal postoperative pain, 0 had deformity, 0 had crepitus with gentle motion, and 0 had instability at the fracture site on exam…

Multivariate injury risk criteria and injury probability scores for fractures to the distal radius.

The purpose of this study was to develop a multivariate distal radius injury risk prediction model that incorporates dynamic loading variables in multiple directions, and interpret the distal radius failure data in order to establish injury probability thresholds. Repeated impacts with increasing intensity were applied to the distal third of eight human cadaveric radius specimens (mean (SD) age=61.9 (9.7)) until injury occurred. Crack (non-propagating damage) and fracture (specimen separated into at least two fragments) injury events were recorded. Best subsets analysis was performed to find t

[Open reconstruction of radius head fractures with and without accompanying ligament instability. Treatment results].

BACKGROUND: The treatment of fractures of the radial head is commonly based on the Mason classification. Fractures of the radial head caused by a dislocation of the elbow are summarized as Mason type IV fractures. The purpose of this study was to investigate the outcome and the influence of additional ligamentous injuries after reconstruction of fractures of the radial head. PATIENTS AND METHODS: One hundred three patients with a fracture of the radial head were treated by open reduction and internal fixation between 2004 and 2009. Fifty-eight of them could be evaluated at an average of 3.6 y

Prospective evaluation of pronator quadratus repair following volar plate fixation of distal radius fractures.

PURPOSE: To evaluate the efficacy of pronator quadratus (PQ) repair after volar plating of distal radius fractures. METHODS: All consecutive distal radius fractures treated operatively with a volar plate during a 1-year period were assigned to receive a repair of the PQ versus no repair. Surgical exposure, reduction, and postoperative rehabilitation were equivalent in both groups. Clinical outcomes with a minimum follow-up of 12 months were assessed via range of motion; grip strength; Disabilities of the Arm, Shoulder, and Hand (DASH) scores; and visual analog scale (VAS) scores. RESULTS: A …

Osteosynthesis with long volar locking plates for metaphyseal-diaphyseal fractures of the distal radius.

OBJECTIVE: Metaphyseal-diaphyseal fractures of the distal radius are a major treatment dilemma and orthopaedic surgeons have to pay due consideration to restoration of anatomy of distal radius together with rotation of the radial shaft and maintenance of radial bow and interosseous space. We performed this study to evaluate the clinic-radiological outcome of metaphyseal-diaphyseal fractures of the distal radius treated with long volar locking plates. METHODS: This prospective study involved 27 patients (22 males and 5 females) with metaphyseal-diaphyseal fracture of the distal radius. Their m

Early correction of paediatric malunited distal metaphyseal radius fractures using percutaneous callus osteoclasis ("Calloclasis").

INTRODUCTION: Closed reduction of secondary displacements of distal metaphyseal radius fractures is an easy procedure only when performed within the first ten to fifteen days post-trauma, prior to the occurrence of malunion. When a hard bony callus prevents proper reduction, an open osteotomy is generally advocated. HYPOTHESIS: We suggest the use of a less invasive technique which aims at correcting early malunion when closed reduction is made impossible: the percutaneous callus osteoclasis. MATERIALS AND METHODS: Callus osteoclasis consists in a series of multiple bone-drilling in a postage…

Soft cast versus rigid cast for treatment of distal radius buckle fractures in children.

INTRODUCTION: Buckle fractures are extremely common and their optimum management is still under debate. This study aimed to ascertain whether buckle fractures of the distal radius can be safely and effectively treated in soft cast with only a single orthopaedic outpatient clinic appointment. METHODS: A total of 232 children with buckle fractures of the distal radius were included in the study. 111 children with 112 distal radius fractures were treated in full rigid cast and 121 children with 123 fractures were treated with soft cast. The rigid cast children attended outpatient clinic for remo…

Distraction plating for the treatment of highly comminuted distal radius fractures in elderly patients.

PURPOSE: To evaluate internal distraction plating for the management of comminuted, intra-articular distal radius fractures in patients greater than 60 years of age at two level 1 trauma centers. We specifically desired to determine whether patients would have acceptable results from the clinical standpoint of range of motion, Disabilities of the Arm, Shoulder, and Hand (DASH) score, and the radiographic measurements of ulnar variance, radial inclination, and palmar tilt. Our hypothesis was that distraction plating of comminuted distal radius fractures in the elderly would result in acceptable…

Quantitative 3-dimensional computed tomography measurement of volar shearing fractures of the distal radius.

PURPOSE: We used a method for quantitative measurement of articular surface area based on 3-dimensional computed tomography (CT) images to test the hypothesis that among volar shearing fractures of the distal radius, complete articular fractures have on average larger volar fracture fragments than partial articular fractures. METHODS: We analyzed 25 adult patients with a quantitative 3-dimensional CT scan of a volar shearing fracture of the distal radius. We identified 3 major fracture patterns: 10 fractures were complete articular fractures (AO C-type; complete dorsal fracture line); 8 fract

Comparative analysis of intramedullary nail fixation versus casting for treatment of distal radius fractures.

PURPOSE: Intramedullary fixation is one treatment option for distal radius fractures. Our purpose was to compare the outcomes of intramedullary nailing to those of casting for these injuries. METHODS: From 2006 to 2009, we reviewed 63 adult patients with isolated distal radius fractures. Thirty-one patients had surgical fixation with an intramedullary device (IMN group) within 4 weeks of the injury, and 32 (cast group) had casting as definitive treatment of the fracture. Clinical outcomes (grip strength; Disabilities of the Arm, Shoulder, and Hand scores; active wrist range of motion; and com…

Trace metal analysis following locked volar plating for unstable fractures of the distal radius.

An increase in the utilization of metallic devices for orthopaedic interventions from joint replacement to fracture fixation has raised concern over local metal ion release and possible systemic sequelae due to dissemination of these ions. Our purpose was to determine whether serum titanium concentrations were elevated in patients who had previously received a locked volar distal radius plate. Our hypothesis was that the simple presence of titanium alone in a relatively fixed implant was not enough to raise serum titanium levels. Twenty-two potential subjects who had received a volar locked di

A comparative study of fragment-specific versus volar plate fixation of distal radius fractures.

BACKGROUND: There are many plating systems available for treating distal radius fractures, and deciding which to use can be difficult. This prospective cohort study compared outcomes of two commonly used fixation systems: fragment-specific fixation and a fixed-angle volar locking plate system. METHODS: Consecutive distal radius fractures were prospectively evaluated in a fragment-specific fixation cohort and a volar locking plate system cohort. Radiographic, functional, and patient-rated outcomes were collected immediately postoperatively and at 6 and 12 months postoperatively. Complications …

[Stabilisation of distal radius fractures by a novel endomedullary, fixed-angle plate: first experience].

BACKGROUND: Based on the fact that not every extra-articular distal radius fracture is stable, we asked ourselves whether there would be a place for other methods for stable fracture management besides on the one hand the plaster cast or pin osteosynthesis, respectively, external fixateur and, on the other hand, the traditional plate fixation (from a palmar or dorsal direction). PATIENTS AND METHODS: In this paper, we report our first experience with a novel plate that has been in use in Europe since April 2005. It is a fixed-angle, intrafocal nail plate. We started using it in April 2005 and
