Selectivity and sensitivity of urine fentanyl test strips to detect fentanyl analogues in illicit drugs.

BACKGROUND: Urine fentanyl test strips have been employed to check street drugs for fentanyl and fentanyl analogue contamination, but there is limited evidence for the applicability of fentanyl strips for this purpose. We examined the ability of four commercially-available fentanyl test strips to detect fentanyl and a range of fentanyl analogues currently on the recreational drug market. METHODS: Four brands of fentanyl test strips (Rapid Response, One Step, Nal van Minden, and Rapid Self Test) were examined using single-component drug solutions containing fentanyl, 28 fentanyl analogues, fou

Validity evidence and clinical utility of the Oviedo Leisure Activities Scale (OLAS-70) for measuring substance-free and substance-related reinforcement.

Substance use disorders (SUDs) can be explained in part by the availability and amount of alternative substance-free reinforcers, which are recognized as the main target in treatment of SUDs. Most questionnaires examining this area assess activities in teens or young adults from the general population and, have not been assessed in the context of treatment of SUDs, and do not address the wide range that is needed in order to plan recreational activities that are incompatible with substance use in clinical contexts, as well as to identify activities that could pose a risk for relapse. Pooled Sampling Technique to Improve the Monitoring of Medication Use in the Racehorse Industry.

All anti-doping programmes face financial constraints and monitoring trends in medication use or abuse in a population of racehorses can be … new COVID variants. Moreover, pooled samples of urine and wastewater have been used to monitor for recreational drug use and for the presence of new psychoactive substances in cities and large events. This approach has been credited with providing timely insight in the trends of illicit drugs use. To be effective, an anti-doping programme should not be predictable to …

Associations between cannabis policies and state-level specialty cannabis use disorder treatment in the United States, 2004-2019.

BACKGROUND: Cannabis use disorder (CUD) treatment prevalence decreased in the US between 2002 and 2019, yet structural mechanisms for this decrease are poorly understood. We tested associations between cannabis laws becoming effective and self-reported CUD treatment. METHODS: Restricted-use 2004-2019 National Surveys on Drug Use and Health included people ages 12+ classified as needing CUD treatment (i.e., past-year DSM-5-proxy CUD or last/current specialty treatment for cannabis). Time-varying indicators of medical cannabis laws (MCL) with/without cannabis dispensary provisions differentiate

Baclofen: To Screen or Not to Screen in Postmortem Blood?

Baclofen (BLF) has been prescribed in the UK since 1972 for the alleviation of spasticity. However, evidence suggests BLF is also recreationally misused. It has been associated with ethanol, gamma-hydroxybutyric acid (GHB), pregabalin (PGL) and gabapentin (GBP) use/abuse, and deaths have been reported. With current postmortem (PM) toxicological screening approaches, BLF is not routinely included in the general drugs screen and is only screened for if specifically mentioned in the case documents. The extent of BLF misuse is thus unclear. This study was carried out to determine the prevalence an

Relation between unconjugated bilirubin and RDW, neutrophil to lymphocyte ratio, platelet to lymphocyte ratio in Gilbert's syndrome.

In this Part II psychiatric disorders coexisting with organic diseases are discussed. "Comorbidity phenomenon" defines the not univocal interrelation between medical illnesses and psychiatric disorders, each other negatively influencing morbidity and mortality. Most severe psychiatric disorders, such as schizophrenia, bipolar disorder and depression, show increased prevalence of cardiovascular disease, related to poverty, use of psychotropic medication, and higher rate of preventable risk factors such as smoking, addiction, poor diet and lack of exercise. Moreover, psychiatric and organic diso

Advancement of humanized mice and leading applications in immunological disease models.

Disease models are used to evaluate drug candidates, and compounds that are highly effective in vivo models have traditionally been prioritized for development. While conventional 'gold standard' animal models have been central to autoimmune drug discovery, there is increasing recognition that addressing unmet medical needs requires models capable of capturing patient pathophysiology beyond the scope of these classical systems. Accordingly, models that reflect human disease mechanisms not reproducible in conventional animals are becoming increasingly important. Humanized mice are immunodeficie

"Ecstasy" associated deaths: what is a fatal concentration ? Analysis of a case series.

Amphetamine derivative drugs, particularly 3,4-methylenedioxymethamphetamine (MDMA), commonly known as ecstasy, are popular recreational drugs. MDMA is associated with causing death by a number of mechanisms, including hyperpyrexia, cardiac arrhythmia water intoxication and liver failure. Seventy-seven deaths where MDMA was detected in body fluids/organs were reviewed. Of these cases 59 deaths had MDMA present in blood. In 13 cases death was attributable to the toxic effects of MDMA alone with a range of 0.478-53.9 mg/l, mean 8.43 mg/l, median 3.49 mg/l. In 22 cases death was due to polydrug u

Tianeptine: enantiomeric separations, structural assignment, and biological interactions.

Tianeptine is an atypical tricyclic antidepressant that has gained attention due to its unique mechanism of action, distinguishing it from other such medications. The drug has proliferated in recreational markets as "gas station heroin" since it produces opioid-like effects and euphoric highs as a result of binding to mu opioid receptors. The unrestricted availability of tianeptine has resulted in many irreversible adverse effects and fatal overdoses. Tianeptine is a chiral molecule and has not yet been widely investigated for the therapeutic/toxic effects of its single enantiomers. Such studi

N-acetylcysteine in the Treatment of Internet Gaming Disorder.

Internet gaming disorder (IGD) is the persistent and recurrent use of the internet to engage in video gaming through a single or multiplayer interface that can lead to significant impairment or distress. With technological advancements in the last decade via portable handheld devices, along with their global availability, video games have found a new medium in which they can provide instantaneous access for casual and enthusiastic users alike. Unfortunately, this exponentially increases the possibility of addiction. IGD shares a similar pathophysiological etiology to addiction as drugs or gamb

Risk at work under pressure with medication- what do we know?

Medication has become an integral part of modern life, as well as in people working in hyperbaric conditions. However, our understanding of how drugs interact with pressure variations, gas compositions, physical exertion, and physiological changes in a hyperbaric environment is very limited. Firstly, the medical condition for which a medication is being taken must be evaluated in the context of fitness for occupational diving. Secondly, the desired or adverse effect of the medication needs to be evaluated in the context of occupational diving. Some potential adverse effects include changes in

Distinct neural mechanisms for the prosocial and rewarding properties of MDMA.

The extensively abused recreational drug (±)3,4-methylenedioxymethamphetamine (MDMA) has shown promise as an adjunct to psychotherapy for treatment-resistant psychiatric disease. It is unknown, however, whether the mechanisms underlying its prosocial therapeutic effects and abuse potential are distinct. We modeled both the prosocial and nonsocial drug reward of MDMA in mice and investigated the mechanism of these processes using brain region-specific pharmacology, transgenic manipulations, electrophysiology, and in vivo calcium imaging. We demonstrate in mice that MDMA acting at the serotonin

Limb-Threatening Ischemia in a Young Man with Cathinone "Bath Salt" Intoxication: A Case Report.

"Bath salts" are synthetic designer drugs that have stimulant properties and are a growing medical concern. The chemical compounds in the mixtures have an affinity for receptors in the brain resulting in a stimulant effect similar to that seen with methamphetamines and cocaine. Although illegal in Canada, these drugs are widely available online with over 20 synthetic drugs marketed as "bath salts" and used increasingly among recreational drug users. Much of the medical literature regarding these drugs comes from emergency medicine case reports, which outline the acute, severe medical, and psyc…

Review of the pharmacology and clinical pharmacology of 3,4-methylenedioxymethamphetamine (MDMA or "Ecstasy").

3,4-Methylenedioxymethamphetamine (MDMA or "Ecstasy") was first synthesised 80 years ago, but has recently received prominence as an illegally synthesised recreational drug of abuse. There is a widely held belief among misusers that it is safe. In the last 2-3 years there have been a number of reports of the drug producing severe acute toxicity and death and there are concerns that it may cause long term toxic damage to 5-hydroxytryptamine (5-HT) nerve terminals. There is a considerable literature on the acute pharmacological effects of MDMA in experimental animals, and this is reviewed. The d

Cannabis and Its Secondary Metabolites: Their Use as Therapeutic Drugs, Toxicological Aspects, and Analytical Determination.

Although the medicinal properties of Cannabis species have been known for centuries, the interest on its main active secondary metabolites as therapeutic alternatives for several pathologies has grown in recent years. This potential use has been a revolution worldwide concerning public health, production, use and sale of cannabis, and has led inclusively to legislation changes in some countries. The scientific advances and concerns of the scientific community have allowed a better understanding of cannabis derivatives as pharmacological options in several conditions, such as appetite stimulati

Drug abuse in Asia.

The article focuses on countries and areas of South-East Asia, which are seriously affected by drug abuse and the problems associated with it. Opium has traditionally been used for treating illnesses and alleviating physical and mental stress, as well as for recreational and social purposes. The prohibition of the sale and use of opium in Burma, Hong Kong, Malaysia, Singapore and Thailand forced many habitual opium users to switch to heroin. Over the past two decades there has been an increasing trend towards drug use, often involving experimentation with more than one substance, among youth i

Comparison of the Neurotoxic and Seizure-Inducing Effects of Synthetic and Endogenous Cannabinoids with Δ9-Tetrahydrocannabinol.

Introduction: Synthetic cannabinoids (SCs) are commonly found in preparations used as recreational drugs. Although severe adverse health effects are not generally associated with cannabis use, a rising number of studies document seizures and even death after SC use. In this study, a mouse model is used to investigate the hypothesis that SCs are more toxic than Δ9-tetrahydrocannabinol (THC), the principal psychoactive constituent of cannabis. Materials and Methods: Beginning with the SCs, JWH-073 and AM-2201, dose-response curves were generated to find the dose of each drug that was similarly e

Liquid ecstasy - a significant drug problem.

INTRODUCTION: Gamma-hydroxybutyric acid (GHB, "liquid ecstasy") and its legal pro-drugs gamma-butyrolactone and 1,4-butanediol are gaining in importance as recreational drugs in Germany. The effects of these substances are comparable with those of alcohol or benzodiazepines. Because of the wide availability of GHB physicians are increasingly being confronted with cases of intoxication. METHODS: This review is based on a selective literature search as well as on the authors' own experience and on information provided by the GIZ-Nord Poisons Centre, Göttingen, Germany. RESULTS: Consumption of
