CASE: We report a case of flexor pollicis longus (FPL) tendon rupture and carpal tunnel syndrome due to scaphoid nonunion advanced collapse deformity. Intraoperative findings showed disruption of the palmar joint capsule and a sharp proximal bone fragment protruding into the carpal tunnel. Removal of this proximal fragment and tendon grafting were performed. At the postoperative 2-year follow-up, the patient had no wrist pain, finger numbness, or restriction of thumb motion.
CONCLUSION: Our results suggest that minimally invasive surgical procedures, such as proximal pole or osteophyte resect

Perilunate dislocations and fracture dislocations represent uncommon and unusual injuries that are often missed at initial presentation and diagnosed late in up to 25% of cases. Prompt open reduction, carpal stabilisation and ligamentous repair is required to reduce the risk of complications. We report a case of an established scaphoid non-union in an operatively managed perilunate fracture dislocation that spontaneously united almost 2 years after the initial injury, just before a planned revision scaphoid fixation with bone grafting. This case highlights the importance of initial clinical as

Diagnosis and proper initial management of acute fractures of the carpal bones is critical because of the limited blood supply of many bones of the wrist and the role of the carpus in optimizing hand function. Pathology is correctly diagnosed by a focused history and examination. Injuries may be missed with a cursory examination and routine wrist radiographs. Together, fractures of the scaphoid and triquetrum make up nearly 90% of carpal bone fractures. Relative frequency, mechanism of injury, diagnosis, and management principles are covered for each of the bones of the wrist.

Bisphosphonates can directly inhibit osteoclasts, which may lead to increased bone density, reduced blood flow, and osteonecrosis of the jaw. Bisphosphonate-related osteonecrosis is usually observed in the jaw bone. In this article, we report a patient with bisphosphonate-related osteonecrosis of the jaw (BRONJ) complicated with wrist scaphoid osteomyelitis. Furthermore, we introduce the pathogenesis, treatment, and prevention of BRONJ.

The Herbert Bone Screw has a thread at both ends and was originally developed for internal fixation of fractured scaphoid bones. The two main advantages of compression and no head, make it most suitable for internal splinting of the small joints of the hand. The authors have used this device on thirty-nine joints in ten different sites of the hand, and it has proved to be an extremely successful method.

Background Wrist injury is a common presentation to the Emergency Department in the United Kingdom. Among these injuries, the scaphoid is the most common fractured carpal bone. However, given the limited ability of conventional radiography to accurately diagnose a suspected scaphoid fracture on presentation, its diagnosis and management remain challenging. Despite the vast clinical evidence supporting the superior accuracy of magnetic resonance imaging, there is little to no evidence around the real-world clinical and economic impact of immediate magnetic resonance imaging in the management of

Background: Scaphoid non-union is a challenging condition that can lead to chronic wrist pain, decreased range of motion (ROM) and functional impairment. One promising approach involves the use of costo-osteochondral rib autografts, offering a non-vascularised and structurally supportive environment for bone regeneration. This systematic review aims to evaluate the current evidence regarding the management of non-salvageable scaphoid proximal poles and non-union using rib costo-osteochondral autografts. Methods: A comprehensive search of electronic databases was conducted. Data extraction was

The scaphoid is the most common carpal bone fractured in adults. In children the fracture is distinctly uncommon. We present our experience with 23 children treated between 1987-1998, 19 of them with fractures in the distal third of the wrist; there were 11 avulsions, 4 waist fractures, and 12 were undisplaced. The mechanism of injury was a direct blow or a fall onto the outstretched hand. Immobilization in a plaster cast led to union in all except 1 case. Based on our experience and study of the literature, it is evident that scaphoid fractures, although uncommon, do occur. Thorough clinical

OBJECTIVE: To investigate the effectiveness of Kirschner wire fixation in scapholunate joint, capitolunate joint, and lunotriquetral joint combined with bone anchor repair for the treatment of acute perilunate injury.
METHODS: Between January 2007 and January 2012, 35 cases of acute perilunate injury were treated. There were 30 males and 5 females with an average age of 45.5 years (range, 32-56 years). Fractures were caused by falling from height in 18 cases, by traffic accident in 11 cases, and by fall injury in 4 cases. The time from injury to operation was 2-6 days (mean, 3.4 days). There

BACKGROUND: Currently, there is no consensus on the optimal surgical technique for scaphoid nonunion (arthroscopic vs open surgery). Open bone grafting is likely to result in better carpal alignment than arthroscopic bone grafting, but the clinical relevance of this issue is unclear. The aim of this retrospective study is to compare the clinical and radiological outcomes of arthroscopic versus open surgery for scaphoid nonunion.
METHODS: A retrospective case-control study was conducted, assessing patients who underwent surgery for scaphoid nonunion between October 2015 and March 2023. Inclusi

A case of post-traumatic carpal proximal instability which was surgically treated two months after the initial trauma is reported. A 41 years old man presented a volar subluxation of the scaphoid associated with a fracture of the volar rim of the distal radius and an increased scapho-lunate gap. This subluxation was initially missed because of the discretion of the radiographic signs and was confirmed by CT scan. A volar rim osteotomy with bone grafting was performed for a buttress support which allowed the reduction of the volar subluxation of the external part of the carpus. Fourteen months

Dual construct fixation has been increasingly used in complex peri-articular or peri-prosthetic long bone fractures, those with poor bone quality and in revision situations. We describe the utilisation of a screw-plate construct in the setting of a juxta-articular distal pole scaphoid fracture, review the literature and provide recommendations for future use. Level of Evidence: Level V (Therapeutic).

Perilunate dislocation is a rare carpal dislocation, which are predominantly trans-styloid and trans-scaphoid. This type of fracture dislocation is seen due to axial loading of a hyperextended ulnar-deviated wrist. This can result in the disruption of the scapholunate, lunocapitate, and lunotriquetral ligaments. The lunate may dislocate volar or dorsal to the other carpal bones. Herein, we have reported a case of trans-styloid, trans-scaphoid perilunate fracture dislocation with carpal tunnel syndrome, which has never been reported in the literature. A 38-year-old construction worker presented

INTRODUCTION: Perilunate dislocations represent one of the most devastating injuries to the carpus. Fortunately, these injuries are relatively rare, constituting approximately 10% of all carpal injuries. One of the problems associated with this injury is the difficulty of its accurate and early recognition.
PRESENTATION OF CASE: In this study, an uncommon case of bilateral dorsal trans-scaphoid perilunate fracture-dislocation following trauma has been reported. The injury was missed initially and the patient was subsequently operated after two weeks. Anatomic reduction was achieved by closed r

The purpose of this study was to describe the agreement between two hospitals on either side of the Atlantic Ocean in reading first day plain radiographs of suspected scaphoid fractures. Two groups of observers, one North American and one European, consisting of observers at various levels of training were compared. Kappa statistics were used to determine inter- and intra-observer agreement. The receiver-operating characteristics (ROC) curves and area under this curve (AUC) for each observer were calculated to determine test performance. Scaphoid radiographs of 80 consecutive patients seen wit

AIMS: The association between scaphoid fractures and ligament injuries is uncertain, and the clinical relevance is not well established. The aim of this study was to undertake a secondary analysis of data from the Scaphoid Waist Internal Fixation for Fractures Trial (SWIFFT) to evaluate the incidence of concurrent ligament injuries in patients with a scaphoid waist fracture displaced by ≤ 2 mm, explore the radiological changes between one and five years after the injury, and assess whether there is an association between the characteristics of the fracture and ligament injury.
METHODS: All 43
