Factors influencing self-harm thoughts and behaviours over the first year of the COVID-19 pandemic in the UK: longitudinal analysis of 49 324 adults.

BACKGROUND: There is concern that the COVID-19 pandemic and its aftermath will result in excess suicides by increasing known risk factors such as self-harm, but evidence on how pandemic-related risk factors contribute to changes in these outcomes is lacking. AIMS: To examine how different COVID-19-related experiences of and worries about adversity contribute to changes in self-harm thoughts and behaviours. METHOD: Data from 49 324 UK adults in the University College London COVID-19 Social Study were analysed (1 April 2020 to 17 May 2021). Fixed-effects regressions explored associations betwe

Polygenic risk scores and help-seeking behaviour in young people with recent onset of mood and psychotic disorders.

OBJECTIVES: We examined associations between polygenic risk scores (PRS) for depression (PRS-MDD), psychosis (PRS-SCZ), bipolar disorders (PRS-BD) and neuroticism (PRS-NEU) and (i) help-seeking, and (ii) new onset cases of full-threshold mood or psychotic disorders in youth. METHODS: Help-seeking for mental health problems was assessed by self-report and mood and psychotic disorders were identified using the Composite International Diagnostic Interview. A principal component analysis of the four selected PRS identified two dimensions (BD-SCZ; MDD-NEU) that accounted for 69.9 % of the explaine

Self-management education programs in chronic disease: a systematic review and methodological critique of the literature.

BACKGROUND: Self-management programs have been widely reported to help patients manage symptoms and contain utilization of health care resources for several chronic conditions, but to date no systematic review across multiple chronic diseases has been reported. We evaluated the efficacy of patient self-management educational programs for chronic diseases and critically reviewed their methodology. METHODS: We searched MEDLINE and HealthSTAR for the period January 1, 1964, through January 31, 1999, then hand searched the reference section of each article for other relevant publications. We incl

Internet-based self-help treatment for panic disorder: a randomized controlled trial comparing mandatory versus optional complementary psychological support.

Panic disorder (PD) is one of the most common psychiatric disorders. Web-based self-help treatments for PD have had promising results. These online treatments seem to have larger effect sizes (ESs) when professional support is added. However, the amount of support or how it should be administered is not yet clear. The aim of this trial was to study two ways of administering psychological support provided by phone as a part of Internet-based self-help treatment for PD based on cognitive behavioral therapy. Seventy-seven participants diagnosed with PD were randomly assigned to one of three exper

Speculum self-insertion: an alternative method for gynaecological examination?

BACKGROUND: Speculum examination is an intrusive practice in the clinical care of women. It requires privacy and patients may experience discomfort or anxiety related to the procedure, which can result in delays or avoidance of necessary healthcare. Speculum self-insertion originated in the United States in the 1970s as part of the self-help movement. However, this clinical practice is largely unknown among healthcare providers and has rarely been assessed. AIM: This study investigates the women's views and healthcare providers' experiences of the self-insertion method. METHOD: A qualitative

"It feels like a safe container": Experiences of finding solid ground groups for adults with dissociative disorders.

OBJECTIVE: Dissociative disorders (DDs) are trauma-related conditions marked by disruptions in memory, identity, and consciousness. Despite high prevalence, DDs are underdiagnosed and undertreated. This study explores the experiences of adults with DD participating in a 31-week Finding Solid Ground psychoeducational group. METHOD: Participants (N = 72) were mostly women (81%) who experienced a DD or posttraumatic stress disorder dissociative subtype, along with multiple mental health comorbidities. Eight groups (seven virtual, one in-person) completed 31 weekly 90-min sessions based on the Fi

Efficacy of the web-based PaarBalance program on relationship satisfaction, depression and anxiety - A randomized controlled trial.

OBJECTIVE: Although relationship distress is strongly associated with mental health problems, poorer social functioning and lower quality of life, only a minority of distressed couples engage in effective couples therapy. Common barriers are the financial burden, fear of being stigmatized, long waitlists and logistical concerns, such as the difficulty in scheduling appointments. Therefore, more accessible help for relationship distress is needed, such as internet-based interventions. METHOD: This study evaluates the efficacy of the German web-based PaarBalance program, an 18-sessions online p

Posttraumatic stress symptom profiles of battered women: a comparison of survivors in two settings.

This study sought to develop a profile of posttraumatic stress symptoms experienced by battered women and to explore differences among subsamples. Two groups of survivors from five states were compared: 159 who had obtained help at domestic violence programs (DVP) and 33 who had obtained help at other types of programs (NDVP). They completed three self-report measures of posttraumatic stress and a fear questionnaire. Sixty percent of the women in the DVP group and 62% in the NDVP group met criteria for a diagnosis of posttraumatic stress disorder (PTSD). The most common symptoms were: nightmar

A Behavioral Measure of Costly Helping: Replicating and Extending the Association with Callous Unemotional Traits in Male Adolescents.

BACKGROUND: Some conduct-disordered youths have high levels of callous unemotional traits and meet the DSM-5's "with limited prosocial emotions" (LPE) specifier. These youths often do aggressive, self-benefitting acts that cost others. We previously developed a task, the AlAn's game, which asks participants to repeatedly decide whether to accept or reject offers in which they will receive money but a planned charity donation will be reduced. In our prior work, more "costly helping" (i.e., rejecting the offered money and protecting the donation) was associated with lower callous unemotional tra

Association Between Digital Front Doors and Social Care Use for Community-Dwelling Adults in England: Cross-Sectional Study.

BACKGROUND: Requests for public social care support can be made through an online portal. These digital "front doors" can help people navigate complex social care systems and access services. These systems can be set up in different ways, but there is little evidence about the impact of alternative arrangements. Digital front-door systems should help people better access services, particularly low-intensity services (high-intensity care is likely to require a full in-person assessment). OBJECTIVE: This study aimed to investigate the association between 2 primary digital front door arrangement

Observation of the effect of one-to-one education on high-risk cases of diabetic foot.

BACKGROUND: Diabetes is a common chronic disease, and its global incidence is on the rise. The disease is directly attributed to insufficient insulin efficacy/secretion, and patients are often accompanied by multiple complications. Diabetic foot is one of the most common complications of diabetes. Diabetic feet have ulcers and infections, which can eventually lead to amputation. Basic nursing care, such as lowering blood pressure and preventing foot skin infections in clinical nursing work, has positive significance for the prevention and control of diabetic feet. AIM: To explore the positive
