Using an Emergency Department Syndromic Surveillance System to Evaluate Reporting of Potential Rabies Exposures, Illinois, 2013-2015.

OBJECTIVES: Mandatory reporting of potential rabies exposures and initiation of postexposure prophylaxis (PEP) allow local health authorities to monitor PEP administration for errors. Our objectives were to use an emergency department (ED) syndromic surveillance system to (1) estimate reporting compliance for exposure to rabies in suburban Cook County, Illinois, and (2) initiate interventions to improve reporting and reassess compliance. METHODS: We queried ED records from 45 acute care hospitals in Cook County and surrounding areas from January 1, 2013, through June 30, 2015, for chief compl

Increasing use and accessibility of anti parasitic drugs for migrants with neglected diseases at a time of migratory emergency.

This work focused on Schistosoma spp. as a potential agent of gross haematuria in non endemic areas. This change in epidemiology is mainly due to recent migratory flows. Moreover it emphasized the needfor cultural action (aimed at Urologists, Dermatologists, General Practitioners and Emergency Medicine doctors) to provide the elements for a correct and timely diagnosis. But the most important issue raised by this paper is the call for a fast track supply of drugs (usually not available in Italy) to field operators for treating tropical diseases.

Progress towards the elimination of onchocerciasis as a public-health problem in Uganda: opportunities, challenges and the way forward.

The national onchocerciasis-control programme in Uganda successfully eliminated Simulium neavei s.s. from the Itwara focus in 1997, by monthly ground spraying with larvicidal temophos (Abate). Since then, no vectors have been caught in the main Itwara focus or two secondary foci in the same area. After 4 years of intervention, S. neavei s.s. has also been nearly eliminated from the Mpamba-Nkusi focus, and the elimination of this vector from two more foci (West Nile and Wambabya-Rwamarongo) appears quite feasible. There are, however, four isolated foci in Uganda (Budongo, Kashoya-Kitomi, Mount

Maintenance fluids in prehospital care: crystalloid versus dextrose solutions--is there a difference?

D5W is the maintenance fluid often used in prehospital care when transporting patients with cardiac or central nervous system processes. However, there is evidence that dextrose solutions are potentially harmful, and that suggests isotonic crystalloid solutions are the preferred maintenance fluid in treating emergent patients regardless of their underlying disease. Dextrose solutions may exacerbate cellular ischemic damage and they cannot be used to resuscitate hypotensive patients. Crystalloids do not cause fluid overload when used at maintenance rates and are effective resuscitative agents i

Microbial signatures of the vaginal microbiome associated with pregnancy outcome in central Anatolian Merino sheep.

Reproductive efficiency is a key determinant of sustainability and profitability in sheep farming systems; however, the role of the vaginal microbiome in shaping pregnancy outcomes remains insufficiently understood. This study aimed to characterise the vaginal microbial communities of Central Anatolian Merino sheep using 16S rRNA gene amplicon sequencing and to explore their potential association with reproductive success. Vaginal samples were collected from 20 healthy ewes (10 non-pregnant (G1), that failed to conceive and returned to oestrus and 10 pregnant (G2), conceived at first service)

Economic analysis of bovine brucellosis control in the Rondônia state, Brazil.

Bovine brucellosis, mainly caused by Brucella abortus, is a worldwide distribution anthropozoonosis that causes great economic losses. In 2001, Brazil launched the National Program for the Control and Eradication of Brucellosis and Tuberculosis (PNCEBT). Contemporaneously, a great effort to characterize the epidemiology of the disease in Brazilian states was started. In the state of Rondônia, a first epidemiological study was carried out in 2004, revealing a prevalence of 35.2% of infected herds and 6.22% of seropositive females. In 2014, after a successful heifer vaccination program with stra

Effects of feeding a source of omega-3 fatty acid during the early postpartum period on the endocannabinoid system in the bovine endometrium.

A total of sixteen Holstein dairy cows (35 ± 1.1 kg/d of milk yield) were randomly assigned to consider the effects of feeding omega-3 sources on endometrial endocannabinoids system (ECS) genes expression to understand the effect mechanism of omega-3 on reproductive performances during the early postpartum period to evaluate to what extent can be intervened in reproduction, e.g. via nutrition to improve fertility. Experimental diets were 1) either protected palm oil (control) or 2) extruded linseed (linseed). Cows (n = 16) were fed from calving date to 70 days in milk (DIM). There was no diffe

Tetrodotoxin suppresses thermal hyperalgesia and mechanical allodynia in a rat full thickness thermal injury pain model.

Burn injuries have been identified as the primary cause of injury in 5% of U.S. military personnel evacuated from Operations Iraqi Freedom and Enduring Freedom. Severe burn-associated pain is typically treated with opioids such as fentanyl, morphine, and methadone. Side effects of opioids include respiratory depression, cardiac depression, decrease in motor and cognitive function, as well as the development of hyperalgesia, tolerance and dependence. These effects have led us to search for novel analgesics for the treatment of burn-associated pain in wounded combat service members. Tetrodotoxin

Multi-tissue omics analyses reveal molecular regulatory networks for puberty in composite beef cattle.

Puberty is a complex physiological event by which animals mature into an adult capable of sexual reproduction. In order to enhance our understanding of the genes and regulatory pathways and networks involved in puberty, we characterized the transcriptome of five reproductive tissues (i.e. hypothalamus, pituitary gland, ovary, uterus, and endometrium) as well as tissues known to be relevant to growth and metabolism needed to achieve puberty (i.e., longissimus dorsi muscle, adipose, and liver). These tissues were collected from pre- and post-pubertal Brangus heifers (3/8 Brahman; Bos indicus x 5

Adjusting the light in the limit conditions of consciousness by the means of ascending reticular activating system (ARAS) and of subordinated systems Part 1: Introduction and Aims.

BACKGROUND: In our activity in the Ambulance Service of Bucharest Municipality during March 2002 - March 2003 we studied a casuistry of patients who had fallen in a coma of varying degrees. To aid better understanding of coma, the concept of "pre-coma stage" or diencephalic "0 stage" was introduced. This concept complements the Arseni classification already used in medical practice, because some doctors alternatively use the term of "inaugural coma" for the same condition that we call diencephalic "0 stage". In the median hypothalamus and on the retino-hypothalamic path (SCN - AN) optical wave

Safeguarding global anticoagulant supply and access.

Anticoagulants are essential to health care, yet their global supply is inherently fragile. Reliance on animal-derived heparin creates vulnerability to contamination, animal disease, and logistical disruption, whereas synthetic alternatives like warfarin and direct oral anticoagulants face mounting manufacturing and geopolitical risks. The COVID-19 pandemic exposed how these intersecting threats can converge during a crisis, causing critical shortages. To build resilience, a systemic shift is required: developing nonanimal-derived anticoagulants, diversifying production geographically, establi

Emergency clinicians need more information about offering spectrum of care and solutions for clients with financial limitations.

OBJECTIVE: Owner-reported financial limitations are common in emergency practice, but it is unclear what clinicians do when faced with clients who cannot afford the recommended treatment plan. The objective of this study was to survey emergency veterinarians and ask about what they would recommend when faced with common scenarios that are treatable but where financial limitations exist. METHODS: An anonymous, internet-based survey of small animal veterinarians at emergency hospitals who described treatment options for a series of case-based scenarios where owners had financial limitations. All the Better to Taste You With, My Dear: ED Management of Mammalian Bites.

Mammal bites account for over 5 million visits to Emergency Departments (EDs) annually. Nurse Practitioners (NPs) need to stay abreast of current guidelines, changes to antibiotic regimens that are now most effective, and understand in what circumstances collaboration with other specialists is indicated. It is not enough to care for the wound, itself, but rather understand in what presentations additional care may be needed despite the fact that there is no clear evidence at the time of evaluation of the need for advanced care. Additionally, NPs should understand what resources are available w

Ultrasound-guided sciatic popliteal block performed at the Emergency Department in a patient with a scorpion bite and severe pain.

We report the case of a paediatric patient who presented at the Emergency Department with severe pain in the right lower extremity caused by a scorpion sting. Analgesics were ineffective, so we decided to perform an ultrasound-guided popliteal block, which provided complete analgesia and allowed the patient to be followed up in the outpatient department, with no adverse effects. The sting of the species of scorpion found in Spain is not dangerous to human life; however, it causes self-limiting localised pain that lasts for 24-48h, and can be severe. The first-line treatment is effective analge
