Female athletes are susceptible to traumatic anterior shoulder instability, possibly due to higher levels of ligamentous laxity leading to soft tissue capaciousness. Rates of shoulder instability are near equal between male and female athletes. Rehabilitation is the initial treatment to improve scapular stabilization and rotator cuff strengthening. If conservative treatment fails, surgical treatment includes arthroscopic versus open Bankart repair for anterior shoulder instability. Surgical intervention for multidirectional instability may include open inferior capsular shift or arthroscopic c

OBJECTIVE: To develop and validate MRI-based radiomics models capable of evaluating supraspinatus tendon tears within the shoulder joints by using arthroscopy as the reference standard.
METHODS: A total of 432 patients (332 in the training set and 100 in the external validation set) with intact supraspinatus tendon (n = 202) and supraspinatus tendon tear (n = 230, 130 full-thickness tears and 100 partial-thickness tears) were enrolled. Radiomics features were extracted from fat-saturated T2-weighted coronal images. Two radiomics signature models for detecting supraspinatus tendon abnormalitie

Glenohumeral joint motion results from a complex interplay between static and dynamic stabilizers that require intricate balance and synchronicity. Instability of the shoulder is a commonly encountered problem in active populations, especially young athletes. The underlying pathoanatomy predisposing to further episodes and the needs of individual athletes must be considered in determining the most appropriate treatment.

Selective capsular repair for anterior instability allows the surgical technique to address the pathology encountered. Capsular tensioning may be performed on either the medial (glenoid) or lateral (humeral) side of the joint depending upon the presence of a Bankart lesion. An initial transverse capsular incision or an enlarged rotator interval defect allows visualization of the joint and flexibility in determining the site of the repair. The arm position is instrumental in determining the appropriate tension necessary to restore stability. Overtightening must be avoided to prevent loss of mot

Pediatric upper extremity injuries, acute and chronic due to overuse, are commonly encountered in the clinical setting. However, interpretation of imaging studies is often challenging in pediatric patients because of changing postnatal skeletal development and the broad spectrum of pathology that can potentially occur. This article discusses normal skeletal development and the pathomechanics of skeletal injuries in pediatric patients. It also outlines a framework for imaging as it pertains to the pediatric upper extremity, with specific emphasis on acute and chronic injuries to the shoulder, e

Trauma to the upper extremity presents difficulties in diagnosis because of the complexity of the anatomy and of the structures involved. These injuries are common, representing more that 45% of all injuries coming to a walk-in emergency center. Commonly missed or misdiagnosed injuries are listed below with clinical examples. Posterior dislocation of the shoulder or perilunar dislocations of the carpus are still commonly missed today in spite of ample information in all medically oriented media. This series of articles is tended to warn emergency room practitioners of the these traps for the u

Snow skiing has been called "the world's oldest sport," "the fastest non-motorized sport on Earth," and "the riskiest sport undertaken by adults on a routine basis." This article discusses the common mechanisms for orthopedic injuries occurring in two of the most popular winter sports in the world that have a reputation for an inherently high risk of injury: alpine skiing and snowboarding. The emphasis herein is on magnetic resonance imaging of characteristic injuries in skiers (knee, thumb, shoulder) and in snowboarders (wrist, forearm, ankle). Spine injuries in snowboarders and skiers are al

Synchronized swimming is a low-injury competitive aquatic sport for all ages. It requires flexibility, kinesthetic awareness, and aerobic conditioning, with the ability to function anaerobically as well. Few acute injuries occur during participation in the sport, but overuse injuries are becoming more common as synchronized swimmers participate in longer, more strenuous workout programs. Most common among these injuries are knee pain associated with the eggbeater kick and shoulder pain associated with sculling. Synchronized swimmers are also prone to otitis externa ("swimmer's ear") and chemic

In 10 cadaver shoulders, a posterior subspinous dislocation of the humeral head was provoked. All specimens showed total rupture of the posterior capsule and teres minor muscle, in most cases together with partial lesion in the infraspinatus muscle. In the majority of the specimens, lesions were also seen in the lower part of the subscapular muscle and in the proximal part of the anterior capsule.

Water polo is a team sport that combines swimming with overhead throwing and wrestling. This places water polo players at risk for a unique group of illnesses and injuries. In addition to the medical problems and injuries seen in competitive swimmers, water polo players are at risk for a variety of traumatic injuries, including concussions, eye injuries, tympanic membrane perforation, fractures, dislocations, and lacerations. Repetitive overhead throwing also places these athletes at risk for related injuries, such as shoulder problems, including rotator cuff strain and impingement, and elbow

The overhead throwing motion is a complex activity that is achieved through activation of the kinetic chain. The kinetic chain refers to the linkage of multiple segments of the body that allows for transfer of forces and motion. The lower extremities and core provide a base of support, generating energy that is transferred eventually through the throwing arm and hand, resulting in release of the ball. The kinetic chain requires optimal anatomy, physiology, and mechanics and is involved in all 6 phases of overhead throwing: windup, stride, arm cocking, acceleration, deceleration, and follow-thr

Shoulder sonography has been shown to be an effective noninvasive means of detecting rotator cuff tears and biceps tendon abnormalities. The examination requires careful attention to technique and a thorough knowledge of normal gross and sonographic anatomy. Rotator cuff tears generally cause a diffuse or focal decrease in the space between the humeral head and the deltoid muscle. Therefore, tears are easiest to detect by noting abnormally close apposition of the deltoid and humeral head.

The anatomical basis of the application of ultrasonography to the examination of the shoulder joint and the diagnostic information gained by the technical possibilities of it are discussed. The author claims that by means of ultrasonic examination it is possible to judge the fact and localization of the rotatory cuff, the lesions of the tendon of the long head of the biceps muscle and inflammation of the joint, as well as certain lesions of the soft parts. In addition, it is possible to form by means of ultrasonography an objective judgement of the result of reconstructive joint operations. Th

The functional and anatomical consideration of the shoulder joint in relation to the clinical aspects can differentiate the general term of "scapulohumeral periarthritis". The most frequently pathological findings are seen in the rotary cuff and the long head of the biceps muscle. Significant causes are further arthrosis of the acromioclavicular joint, the instability of the shoulder joint and the frozen shoulder. The exact diagnosis of the disease makes on operative therapy possible.

INTRODUCTION: Postoperative positioning devices, such as shoulder splints, are commonly used to support and stabilize the upper limb following surgery. However, improper placement and prolonged pressure from these devices can lead to iatrogenic nerve injuries.
CASE PRESENTATION: We present a case of severe axonal injury to the median and ulnar nerves at the proximal hand in a patient following shoulder surgery. This was attributed to palm pressure from a postoperative shoulder splint with a built-in palm stress ball designed to improve circulation and healing, along with impaired pain sensati

Bilateral clavicle fractures represent one of the rarest patterns of shoulder girdle trauma, occurring predominantly after high-energy mechanisms and posing unique challenges in diagnosis and management. Their simultaneous disruption compromises both clavicular struts, resulting in impaired shoulder biomechanics and necessitating timely stabilization for optimal functional recovery. A 25-year-old male sustained high-velocity trauma following a motorcycle accident and presented with severe bilateral shoulder pain, deformity, swelling, and markedly restricted arm elevation.
