Severe hypervitaminosis A in siblings: evidence of variable tolerance to retinol intake.

A 2-year-old boy had signs and symptoms of chronic hypervitaminosis A. A course of increasing severity led to eventual death. A younger brother later had similar clinical features. Chicken liver spread containing up to 420 IU/g vitamin A was the likely source of intoxication. Markedly elevated circulating retinyl ester levels have persisted in the surviving sibling for 3 subsequent years despite severe restriction of vitamin A intake. A therapeutic trial of the carbohydrate-derived complexing agent 2-hydroxypropyl-beta-cyclodextrin was initiated. Circulating retinyl esters transiently increase

Effect of gluten supplementation in healthy siblings of children with celiac disease.

The clinical, functional, and histopathological effects of 18 g additional gluten intake daily for 4 wk was studied in 13 healthy siblings of Spanish children with celiac disease, including two pairs of discordant monozygotic twins. Five of the 13 children were HLA-DR identical to the celiac sibling, 5 shared only one HLA-DR antigen with the celiac sibling, and 3 had completely different HLA-DR antigens than their respective celiac brothers or sisters. Clinical evaluation and functional tests (routine blood, xylose absorption, and fecal fat excretion studies) were performed before, during, and…

[Familial cerebral aneurysms in three families, with two sibling pairs].

The authors describe two pairs of siblings and a mother-son with cerebral aneurysms and the characteristics of familial intracranial aneurysms are briefly discussed. Family 1: A 54-year-old hypertensive woman (case 1) developed subarachnoid hemorrhage and a saccular aneurysm at the proximal portion of the left anterior cerebral artery was demonstrated on the angiogram. An azygos anterior cerebral artery was found as an associated anomaly. A 53-year-old hypertensive woman (case 2), a younger sister of case 1, suffered from subarachnoid hemorrhage and a saccular aneurysm at the distal portion of…

Kidney graft survival in transfused and nontransfused sibling beagle dogs.

In 6 pairs of sibling Beagle dogs, 1 kidney was exchanged between pairs, and contralateral nephrectomy was done. Previously, one dog of each pair was given blood transfusions from the donor of its allograft. All dogs were given azathioprine and prednisone postoperatively for immunosuppression. Four of 6 dogs given pretransplantation transfusions were healthy 1 year after surgical manipulation was done, and 2 died for reasons other than graft rejection. Of the 6 dogs that were not given pretransplantation transfusions, 3 were healthy after 1 years, but 2 were euthanatized because of graft rejec

Attachment network, interparental conflict, and older siblings' behavior predicting toddler behavior problems.

Secure infant-father and infant-mother attachments are associated with fewer internalizing and externalizing behavior problems in childhood. … uld predict toddler behavior problems when other family risk factors (interparental conflict, older siblings' behavior problems) were added as predictors. Participants were 184 second-born, 12-month olds (55.9% girls) who participated in the strange situation procedure with fathers and mothers and were classified into four attachment configurations: both s …

A novel CEP78 variant and rod-cone dystrophy in non-consanguineous siblings.

INTRODUCTION: To report a case of two siblings with a novel CEP78 mutation, presenting with Rod-Cone Dystrophy (RCD). METHODS: This is an observational case report of two Chinese male siblings, born to non-consanguineous parents, presenting with severe rod-cone dystrophy seen on clinical examination, multimodal imaging, and electroretinogram. Both patients underwent targeted gene sequencing. The parents of both patients underwent targeted gene sequencing of requested variants to determine inheritance. RESULTS: Patient 1 and 2 presented with symptoms of RCD at ages 7 and 42 years old respecti…

Blended Phenotypes in Siblings: Dual Diagnoses of Nicolaides-Baraitser and Craniosynostosis Syndromes.

INTRODUCTION: Neurodevelopmental and multiple malformation disorders spanning large phenotypic series can often lead to obscure diagnoses in the clinic. Blended phenotypes from multiple etiologies can compound this issue. We present a rare familial case of two siblings with Nicolaides-Baraitser syndrome (NCBRS) complicated by an initial diagnosis of syndromic craniosynostosis in one of the patients. CASE PRESENTATION: Whole exome sequencing (WES) was performed for the affected siblings using the Agilent SureSelect kit and Illumina HiSeq 2500 system, followed by GATK variant calling and in-hou…

Combined central and peripheral demyelination in two siblings, immune mediated or genetic?

We report unusual cases of combined central and peripheral demyelination in two siblings related to pregnancy, each presenting with progressive tetraparesis and cranial nerve palsies. The elder sister had a relapsing-remitting course with optic nerve dysfunction and died during a relapse from respiratory insufficiency. The younger sister presented with disorientation and acute-onset limb and facial weakness. She responded well to corticosteroid therapy. Their clinical presentation, response to immunomodulatory therapy, nerve conduction studies, cerebrospinal fluid and histology supported an ac…

Primary congenital glaucoma in two siblings with different compound heterozygous CYP1B1 genotypes.

OBJECTIVE: To describe the inheritance pattern and clinical variability of primary congenital glaucoma (PCG) in a family with two affected siblings. MATERIALS AND METHODS: Two sisters diagnosed at birth with bilateral PCG, whose father had bilateral PCG and mother had bilateral microphthalmus, were subjected to a familial genetic study and ophthalmologic follow-up including intraocular pressure (IOP) measurement, and collection of biometric and cup-to-disc ratio data. RESULTS: The inheritance pattern was autosomal recessive in compound heterozygosis. The sisters were found to be carriers of …

Novel PIBF1 Pathogenic Variant in Three Siblings with Joubert Syndrome Type 33.

BACKGROUND: Joubert syndrome type 33 (JBTS33) is a rare autosomal recessive disorder characterized by developmental delay, severe renal disease, hypotonia/ataxia, cerebellar vermian hypoplasia/aplasia, optic nerve atrophy, renal atrophy, and the "molar tooth sign" on imaging. Nearly 40 genes associated with Joubert syndrome have been identified, including CEP290, TMEM216, TMEM67, AHI1, and CC2D2A. METHODS: We report a consanguineous family with JBTS33 diagnosed through whole-exome sequencing. A novel biallelic homozygous nonsense mutation (ENST00000326291.11: c.1231C>T; p.Arg411Ter) in proges

Subsequent development of Kawasaki disease following acute human adenovirus infection among siblings.

We report a middle-childhood girl presented with high-grade fever and headache for 4 days. Following this, the child developed mucocutaneous … rkers and echocardiographic changes, diagnosed as incomplete KD.The subsequent development of KD in siblings, both showing initial viral symptoms and a family history of autoimmune disease, led to the suspicion of a potential viral trigger. This was confirmed through viral PCR studies for human adenovirus (type 3). These cases highlight an unusual occurren …
