Gender and Age Determinants of Psychogenic Movement Disorders: A Clinical Profile of 73 Patients.

BACKGROUND: Psychogenic movement disorders (PMD) is a group of disorders that cannot be attributed to any structural or biochemical abnormality, but has an underlying psychiatric illness. The profile of PMD varies according to country and socioeconomic factors. METHODS: The present study reports the clinical profile of patients with PMD from India. Seventy-three patients with documented or clinically established PMD were seen over a period of 14 years with detailed neurological and psychiatric examinations. RESULTS: The mean age at presentation was 29.1±15.1 years (women, 51%). Approximately

A disembodied man: A case of somatopsychic depersonalization in schizotypal disorder.

In the general concept of self-disturbances in schizophrenia and schizophrenia spectrum disorders, somatopsychic depersonalization (SPD) occupies a special place as it constitutes a syndrome that comprises feelings of detachment from one's own body and mental processes. However, apart from clinical descriptions, to date the pathophysiology of SPD is not fully understood due to the rareness of the syndrome and a lack of experimental studies. In a case study of one patient with schizotypal disorder, we applied a multimodal approach to understanding the SPD phenomena. The patient's clinical profi

Smoking and panic attacks, panic disorder, and agoraphobia: a review of the empirical literature.

The empirical literature regarding panic-spectrum problems (i.e., panic attacks, panic disorder, and agoraphobia) and cigarette smoking is reviewed. In the first section of the paper, empirical studies that document the prevalence of smoking and panic-related problems are presented and discussed. In the second section of the paper, studies pertaining to the role cigarette smoking may play in the onset and maintenance of panic-related problems are critically reviewed. In the third section of the paper, studies related to the association between panic vulnerability factors and the nature of smok

A new approach to assess illness behaviour.

OBJECTIVE: The aim of this study was to develop a scale to assess the different aspects of illness behaviour and to evaluate the measure in depressive and somatising patients. METHODS: The psychometric properties of the scale were assessed in the following diagnostic groups: patients with major depression (n=36), major depressives with an additional somatisation syndrome (n=40), patients with somatisation syndrome (n=37), and a control group (n=37). After multiple test construction procedures and factor analysis, we developed the Scale for the Assessment of Illness Behaviour (SAIB). RESULTS:

The correlation of depression and gastric dysrhythmia in functional dyspepsia.

Psychologic factors in functional dyspepsia have been discussed in many previous articles. However, the relationship between depression and functional dyspepsia is still obscure. We investigated the impact of depression on clinical symptoms and gastric dysrhythmia in functional dyspepsia. Thirty-nine patients with functional dyspepsia and 18 healthy subjects were included. Patients were investigated with clinical symptoms assessment, Zung's self-rating depression scale, and electrogastrography. Patients with functional dyspepsia were divided into two groups: 21 patients with depression and 18

[Subthreshold psychological disorders].

Subthreshold psychic disorders are mild, masked, atypical, or intense but brief psychopathological syndromes below the threshold of standardized diagnoses. They indicate beginning, intermittent, or residual states of well-known psychic disorders or ("comorbid") syndromes associated to other psychic or somatic disorders or possibly partially morbid states by themselves. At least the subthreshold depressions and anxiety disorders are more than twice as frequent as specified diagnosable psychic disorders and have serious consequences with regard to both individual suffering and cost. [Organic neuroses as psychosomatic problem].

To study a role of psychogenic and somatogenic factors in the development of organic neuroses, 302 patients were examined: 199 with cardioneurosis and 103 with irritable-bowel syndrome (IBS) with mental and subclinical somatic pathology or without it. Two types of organic neuroses were distinguished: a conversive one--somatized hysteria (psychogenically determined subsyndromal hysterohypochondriac phobic or affective reactions with clear difference of algesic and autonomic disorders from symptoms of the somatic pathology, with polymorphism of the involved organic systems); and autonomic neuros

Psychogenic factors in the aetiology of stomatopyrosis.

This research has been conducted on the basis of the association between a psychogenic factor and stomatopyrosis as its consequence. Stomatopyrosis is characterized as a burning sensation or as pain in the mouth cavity with clinically normal oral mucosa. It typically occurs with elderly female population, aged on average 67. Burning mouth as a symptom occurs primarily on the lips, although it may be located on some other sites on the oral mucosa. There are various etiological factors influencing the emergence of stomatopyrosis. They are divided into local factors, related to candidiasis, dysfu

Psychologic factors involved in the decision to undergo reconstructive surgery after burn injury.

Burn injuries often result in permanent changes in physical appearance and function. Although reconstructive surgery is often considered to improve function or physical appearance, or a combination, variables that may predict use of surgery are relatively unknown. Burn survivors (N = 46; 48% male) were assessed at an evaluation for potential reconstructive surgery. Measures of adjustment, distress, and personality were administered. Several psychologic and demographic variables differed among those who did versus those who did not subsequently undergo surgery. Social, sexual, and family relati

Challenges to identifying and treating women with depression in rural primary care.

Many women seeking help from rural primary care providers are experiencing major or other types of depression. Women typically do not discuss their depression with their primary care providers although those who did reported finding it very helpful. Feeling understood and advice were particularly viewed as helpful. Major barriers to discussing depression with primary care providers include stigma about depression, lack of time, and perceptions that the primary care providers were not interested in these concerns. Depressed women tend to seek help for certain somatic problems, especially headac

[Panic attacks and avoidance behavior].

In a crossectional investigation based on a group of 122 patients with panic attacks, compiled from a variety of hospitals and therapy institutions, 97 patients were found to be suffering from panic disorder as a relevant medical problem. The disease, however, is often identified in an unsatisfactory manner. Avoidance behaviour (extensive or limited) associated with a severe form of panic disorder, was evident in more than one-half of the group (60%). In these patients the incidence of panic attacks is higher, anxiety is more marked, and there are more often additional anxiety disorders (gener

A study of so-called hypochondriasis.

Twenty-four patients with unexplained somatic complaints were subjected to a thorough somatic examination. Only when the examination proved negative was the patient entered into the study. The patients were clinically appraised according to criteria given in DSM-III. Generalized anxiety disorder (GAD) was diagnosed in 12, somatization disorder (SD) in 8, and hypochondriasis in 4 patients. Seventeen of the 24 patients agreed to participate in biochemical investigations including a TRH load, a dexamethasone test, and a determination of the monoamine metabolites 5-HIAA and HVA in cerebrospinal fl

[Early detection of anxiety disorders].

Anxiety disorders represent a widespread illness. Those affected with initial symptoms usually seek the help of their family doctor. In many cases, considerable time passes before the diagnosis has been established and specialised treatment applied, with the result that chronification is furthered. Physical symptoms of an anxiety disorder, and fears of contracting somatic disease are almost always the first to be described. In patients abusing alcohol and/or hypnotics consideration must always be given to an anxiety disorder. Previously existing symptoms almost always include depressive moods

Social phobia with sudden onset--post-panic social phobia?

Overlap between social phobia (SP) and panic disorder (PD) has been observed in epidemiological, family, and challenge studies. One possible explanation is that some cases of SP develop as a consequence of a panic attack in a social situation. By definition, these cases of SP have sudden onset. It is hypothesized that patients with SP with sudden onset are more similar to patients with comorbid SP and PD than to patients with SP without sudden onset regarding age of onset, extraversion, and prevalence of anxiety symptoms. One hundred and eighty-two patients with a lifetime diagnosis of PD and/

[Anxiety-ridden patients in the doctor's office].

Initially, patients with anxiety syndromes consult the general practitioner complaining of physical symptoms. After an appropriate differential diagnostic work-up, the family doctor can therefore check whether the patient might possibly be suffering from an anxiety disorder. For the treatment of such disorders, psychotherapeutic and psychopharmacological strategies all have a role to play. The general practitioner can initiate supportive medication before referring the patient to a specialist. Early detection and treatment of an anxiety disorder can prevent the condition from becoming chronic

Changing the psychiatric knowledge of primary care physicians. The effects of a brief intervention on clinical diagnosis and treatment.

This research examined the malleability of physicians' knowledge about psychiatric disorders (and their recommended treatments) based on a brief educational intervention. Primary care physicians were randomly assigned to a single-session intervention dealing with selected DSM-IIIR affective and anxiety disorders or to a wait-list control group. Posttest (vs. pretest) measures indicated that experimental physicians showed greater increases than controls in diagnostic accuracy on relevant disorders, specifically, major depression, dysthymic disorder, and agoraphobia with panic attacks. Telephone-supported computerised cognitive-behavioural therapy: REEACT-2 large-scale pragmatic randomised controlled trial.

BackgroundComputerised cognitive-behavioural therapy (cCBT) for depression has the potential to be efficient therapy but engagement is poor in primary care trials.AimsWe tested the benefits of adding telephone support to cCBT.MethodWe compared telephone-facilitated cCBT (MoodGYM) (n = 187) to minimally supported cCBT (MoodGYM) (n = 182) in a pragmatic randomised trial (trial registration: ISRCTN55310481). Outcomes were depression severity (Patient Health Questionnaire (PHQ)-9), anxiety (Generalized Anxiety Disorder Questionnaire (GAD)-7) and somatoform complaints (PHQ-15) at 4 and 12 months.Re

Evidence-based data on pain relief with antidepressants.

This structured review addresses the issue of whether antidepressants have an antinociceptive (analgesic) effect for chronic pain independent of their antidepressant effect. In order to answer this question, human acute pain studies, individual placebo-controlled studies for the treatment of specific chronic pain syndromes, and metaanalytic studies were reviewed and placed into table format. Analysis of this evidence led to the following conclusions: The evidence was consistent in indicating that overall antidepressants may have an antinociceptive effect in chronic pain, and that these drugs w
