Hydatid disease (Echinococcosis) is a common parasitic infection caused by Echinococcus granulosus mainly in sheep-raising areas of the world. Liver, lungs and brain are the predominantly involved organs. However, 0.5-1% of the hydatid disease involves the spine and in 90% of the cases it is confined to the bone and the epidural space. Although intramedullary involvement is extremely rare, in this report, we present a 55-year-old female patient who was diagnosed with a cervical intramedullary hydatid cyst during magnetic resonance imaging of the cervical vertebrae. Accordingly, we imply that p…

OBJECT: Comorbidities in patients undergoing spine surgery may reasonably be factors that increase health care costs. To verify this hypothesis, the authors conducted the following study.
METHODS: Major comorbidities and age-adjusted Charlson Comorbidity Index scores were retrospectively analyzed for 816 patients who underwent spine surgery at the authors' institutions between 2005 and 2008, and treatment costs (hospital charges) were assessed with the help of statistical software. The sample was collected by a nonmedical staff (hired at the beginning of 2006). Patients underwent one of the t…

Cervical spine injury can be excluded by clinical examination, without the need for radiographic study, in many patients. For those who require study, computed tomography of the cervical spine with sagittal and coronal reconstruction is the best modality for both screening and diagnosing cervical spine injury. Optimal evaluation of the obtunded patient remains controversial.

Spinal osteoblastomas are considered benign tumors but can be locally aggressive. Patients usually present with pain and undergo radiologic and histologic work-up to establish a diagnosis. Osteoblastomas have discordant appearances on CT scans and MRI because of the inflammatory response seen on MRI that characterizes these tumors - the "flare" phenomenon. Solely using MRI can lead to over-resection, as signal abnormality may include areas devoid of tumor. There are a few reports of this phenomenon in the radiology literature, but to our knowledge, there are none in neurosurgery journals. We r

Techniques of percutaneous spinal instrumentation have in the meantime become standard methods in many hospitals. While several indications have been established that are excellently suited to this technique, uncertainty prevails for other indications. This contribution intends to clarify the technical prerequisites for performing percutaneous instrumentation in the region of the thoracic and lumbar spine in addition to describing customary indications and various techniques of percutaneous instrumentation. This is combined with a critical assessment of what intrinsically cannot or cannot yet …

Posterior decompression of the cervical spine is an accepted treatment for patients with cervical canal disease, but some patients experience postoperative axial pain and C5 or C6 palsy that affect their quality of life. Here we describe selective posterior decompression using a spinous process-splitting approach to prevent these complications performed in 17 patients with myelopathy treated at median 2.4 levels by selective posterior decompression via the transspinous approach. Clinical symptoms, axial pain, and C5 or C6 palsy were compared before and after treatment. The range of motion of t…

Merkel cell carcinoma is a rare neuroendocrine malignancy, primarily affecting sun-exposed skin. The mean age at presentation is 75 years. To our knowledge there have been only four patients with spinal metastasis reported previously, with one patient alone having a marked neurological deficit requiring surgical decompression. We report a 73-year-old male who presented with rapidly progressive paraplegia 6 months following excision of a primary cutaneous Merkel cell lesion. He had a metastatic deposit at T6 comprising a canal filling epidural component with local aggressive bone destruction. H

STUDY DESIGN: A retrospective cohort study of the relationship between the structures that form the lumbar spine in humans.
OBJECTIVE: To investigate the relationship between the segmental wedging of the vertebral bodies and that of the intervertebral discs, and between the overall lordosis angle and each of the 5 lumbar segments.
SUMMARY OF BACKGROUND DATA: Little attention has been paid to the internal relationship between the structures that form the lumbar spine. Understanding these relationships is instrumental to our ability to restore and rehabilitate the lordotic curvature.
METHODS:…

Calcium pyrophosphate dihydrate crystal-deposits (CPPD) at the spine are rare but the lesions detected with CT scans or MRI are often interpreted as a spondylodiscitis or osteitis. CPPD is a disease of the elderly without major sex predominance. The diagnosis of CPPD requires typical manifestations on a radiograph and/or detection of positively birefringent crystals in the synovial fluid of (peripheral) joints by compensated polarized light microscopy. CPPD crystal deposition at the spine has been associated with clinical manifestations, typically spine stiffness, and is sometimes associated w…

Acute spinal injuries are fortunately rare in pediatric sports but can be catastrophic. Imaging is integral to the diagnosis and care of spinal trauma. Plain radiographs and CT are critical for detecting vertebral fracture, and MR imaging is an essential adjunct for evaluating muscular, ligamentous, and spinal cord injury. Back pain is a common complaint among athletes of all ages. The growing spine has unique weaknesses that result in a higher rate of detectable radiologic abnormalities. Disk pathology is less common in children, and is often uniquely associated with fracture of the ring apop…

There are a number of reports on Smith-Petersen osteotomy (SPO), pedicle subtraction osteotomy (PSO) and vertebral column resection (VCR). However, there are few systematic reviews of all three kinds of osteotomies. Literature review and author's experience of SPO, PSO and VCR osteotomy will be described. Various surgical techniques can be applied according to the disease entity and magnitude of the deformity. The most appropriate methods for deformity correction should be chosen and the potential complications should be considered. Before attempting an osteotomy of the spine for a spinal defo…

Manipulative therapy of the cervical spine is associated with a considerable risk of stroke. We evaluated all cases with the diagnosis of arterial dissection submitted between 1996 and 2005 to the Schlichtungsstelle für Arzthaftpflichtfragen der Norddeutschen Arztekammern for assessment of the accusations brought against the therapists who conducted the manipulation. Neither in the 7 carotid nor in the 9 vertebral artery cases could a causal link be made between the dissection and the manipulation. However, in 5 of the 7 carotid and 7 of the 9 vertebral artery dissections there was clear evide…

In the current article we review the available English literature by pubmed search engine on the topic of osteochondromas and their location in the cervical spine. The focus is to investigate the location of the lesions in cervical spine and to analyze clinical presentations and symptoms of patients, the aetiology and histopathology examination of the masses. In addition we report a rare illustrative case of a 62 year-old man with an osteochondroma of the cervical spine. The mass developed at a very slow rate for many years and produced no clinical symptoms. The location of the extradural mass…

Imaging procedures are an important diagnostic cornerstone for the examination and disorders of the musculoskeletal system. Thanks to the development made in the last 100 years, this kind of diagnosis has made an enormous progress and will continue to do so. Computer technology allows not only to improve and speed up the imaging process but also to make technically outstanding reconstructions which can be necessary for specific operative indications but provoke a development that does not meet the current requirements of cost effectiveness and expendiency. Thus, every user of this technique sh

European spine journal : official publication of the European Spine Society, the European Spinal Deformity Society, and the European Section of the Cervical Spine Research Society2009eng
The morbidity of surgical procedures for spine tumors can be expected to be worse than for other conditions. This is particularly true of en bloc resections, the most technically demanding procedures. A retrospective review of prospective data from a large series of en bloc resections may help to identify risk factors, and therefore to reduce the rate of complications and to improve outcome. A retrospective study of 1,035 patients affected by spine tumors-treated from 1990 to 2007 by the same team-identified 134 patients (53.0% males, age 44 +/- 18 years) who had undergone en bloc resection fo…

STUDY DESIGN: Retrospective case review of children with eosinophilic granuloma of the cervical spine.
OBJECTIVE: To present the clinical features, radiographic findings, management, and results of 7 pediatric patients with eosinophilic granuloma of the cervical spine.
SUMMARY OF BACKGROUND DATA: Management of the eosinophilic granuloma of the pediatric cervical spine is challenging, and must be adapted according to the patient, as no level I evidences have been produced. The therapeutic goals in these children are spinal stability, preservation of neurologic function, and relief of pain, al…
