Traumatic Rhabdomyolysis: Crush Syndrome, Compartment Syndrome, and the 'Found Down' Patient.

Traumatic rhabdomyolysis is a systemic manifestation of muscle injury and can occur from various traumatic etiologies, including crush syndrome (CrshS) and compartment syndrome (CS). Although historically described in natural disaster survivors trapped under collapsed structures, the frequency of CrshS has increased in the setting of 'found down' patients from opioid overdoses. Signs and symptoms of the injured limb in CrshS may range from pressure-induced skin changes to neurologic deficits and paralysis. Although its pathophysiology differs from CS, severe injuries may lead to an associated

Neurological recovery after early decompression for dorsal Pott's spine.

INTRODUCTION: Tuberculosis spondylitis or Pott's disease is the most common destructive form of skeletal tuberculosis. The most commonly affected site is the thoracolumbar vertebra. Once invading the adjacent structures of the vertebrae and intervertebral discs ultimately form an abscess causing spinal cord compression, vertebral collapse, and severe kyphotic deformity. PRESENTATION OF CASE: We present the first-ever case done in King Fahd Military Medical Complex in Dhahran, Saudi Arabia, of an 18-year-old male diagnosed with tuberculosis spondylitis who was complaining of progressive upper

Tongue Function: An Underrecognized Component in the Treatment of Obstructive Sleep Apnea with Mandibular Repositioning Appliance.

Obstructive sleep apnea (OSA) is a common but still underrecognized disorder. A mandibular repositioning appliance (MRA) is used to treat OSA by advancing the mandible and thereby reducing the collapsibility of the upper airway. It has been found that an MRA increases the volume of the upper airway, especially the velopharyngeal area, in OSA patients. We hypothesize that this increase in the velopharyngeal volume is associated with an anterior displacement of the tongue, but likely not with a stretching of the soft tissue connecting the soft palate, lateral pharynx, palatopharyngeal arch, and

Increased Levels of Brain Adrenomedullin in the Neuropathology of Alzheimer's Disease.

Alzheimer's disease (AD) is characterized by the loss of synaptic contacts caused in part by cytoskeleton disruption. Adrenomedullin (AM) is involved in physiological functions such as vasodilation, hormone secretion, antimicrobial activity, cellular growth, and angiogenesis. In neurons, AM and related peptides are associated with some structural and functional cytoskeletal proteins, causing microtubule destabilization. Here, we describe the relationships between AM and other signs of AD in clinical specimens. Frontal cortex from AD patients and controls were studied for AM, acetylated tubulin

Gene expression analysis of an epidermolysis bullosa simplex Dowling-Meara cell line by subtractive hybridization: recapitulation of cellular differentiation, migration and wound healing.

An intact keratin 5/keratin 14 intermediate filament cytoskeleton is vital for the integrity of basal keratinocytes and for the development and maintenance of epidermal structures. In patients with epidermolysis bullosa simplex Dowling-Meara (EBS-DM), heterozygous mutations in the keratin 14 gene in keratinocytes cause a cytoskeletal collapse leading to fragile cells susceptible to cellular stress. The primary aim of this work was to extend analysis of differentially expressed genes in an EBS-DM model cell line to obtain insights into the molecular consequences resulting from the keratin 14 mu

Core decompression and labral support for the treatment of juvenile osteonecrosis.

The final outcome of Legg-Calvé-Perthes disease is variable and depends mostly on the age of the patient at onset and the amount of femoral head involvement. The goals of treatment are to achieve a round femoral head with a congruous hip joint and to contain the femoral head to avoid extrusion. We will term this condition in the adolescent age, "juvenile osteonecrosis," as we understand that the osteonecrosis pattern responds differently to that in younger patients who develop Perthes and it is similar to adult osteonecrosis. Most of the deformity comes from the collapse and enlargement in siz

[Conventional and semi-open kyphoplasty].

Kyphoplasty is a young method which was developed for the minimally invasive augmentation of osteoporotic vertebral fractures. In contrast to vertebroplasty, the kyphoplasty technique allows an age-dependent fracture reduction through the inflation of a special balloon in the fractured cancellous bone of the vertebral body. The cancellous bone of the fracture zone is compressed by the balloon, so that a cavity remains in the vertebral body after removing the balloon, which is filled with highly viscous augmentation material. The reduced risk of serious complications, for example epidural leaka

A morphological model of vertebral trabecular bone.

In their micro-structures, typical natural cellular materials such as vertebral trabecular bone have a network of doubly tapered struts, thickening near the strut joints. However, past analytical models for vertebral trabecular bone do not take account of the effect of strut taper on the mechanical properties. This paper presents an analytical cell model comprised of doubly tapered struts to predict the global mechanical properties of vertebral trabecular bone. The predicted results for male, female, and both sexes fit the experimental data well. By considering several strut taper geometries,

Impact of compounded financial collapse and armed conflict on access to radiation therapy in Lebanon: lessons for building resilient cancer care systems in fragile settings.

BACKGROUND: Lebanon's radiation therapy (RT) system has been exposed to successive, overlapping crises since 2019, including financial collapse, the Beirut Port explosion, and armed conflict in 2024. This study analyzes the impact of compounded crises on RT access and practice patterns using longitudinal institutional data from Mount Lebanon Hospital (MLH) as a sentinel cohort. METHODS: A retrospective observational study was conducted at MLH (2018-2025). Annual treated patient volumes, treatment technique distributions (3D-CRT, IMRT, SBRT, SRS), and hypofractionation adoption patterns were a

Outcomes and Complications of First Tarsometatarsal Plantarflexion Bone Block Arthrodesis With a Precontoured Allograft Wedge.

BACKGROUND: Instability and collapse of the medial longitudinal arch and structural incompetence of the first ray have been linked to various conditions of the foot. These include progressive collapsing foot deformity (PCFD), hallux valgus (HV), and midfoot osteoarthritis (OA). More recently, an innovative approach combining the biomechanical advantages of the modified Lapidus procedure and Cotton osteotomy were described in the literature. The aim of this study was to determine the midterm outcomes following first-tarsometatarsal plantarflexion bone block arthrodesis with a precontoured allog

Functional reorganization and systemic toxicity induced by microplastics and climate-relevant stressors in Tribolium castaneum: Are we simulating tomorrow's toxic reality?

Microplastic (MP) pollution and the intensification of global climate change raise concerns regarding potential interactions between these stressors and their ecotoxicological effects on terrestrial biota. In this study, we investigated the ecotoxicological effects of polyethylene MPs (PE-MPs) under different climate scenarios in Tribolium castaneum. Adults were exposed for 21 days to two environmentally relevant concentrations of PE-MPs (0.005 % and 0.1 %) under three contrasting temperature and CO2 scenarios (27 °C/1000 ppm, 28.5 °C/1500 ppm, and 30 °C/2000 ppm). Experimental study of a 3D-printing technique combined with biphasic calcium phosphates to treat osteonecrosis of the femoral head in a canine model.

OBJECTIVE: This study was aimed to use a digital design of 3D-printing technology to create a surgical navigation template. At the same time, biphasic calcium phosphate (BCP) was applied to treat osteonecrosis of the femoral head (ONFH) in animal models, based on accurate positioning of necrotic lesions in the navigation templates and observation of its therapeutic effect. METHODS: Fifteen healthy adult male and female beagle dogs weighing 20 + 2 kg were randomly divided into three groups (n = 5) after establishing a model of ONFH using the liquid nitrogen freezing method. Each model underwen

Beyond the Common Femoral Artery: The Overlooked Risk of External Pudendal Artery Injury.

Femoral arterial access is a cornerstone of numerous interventional procedures including percutaneous coronary intervention (PCI), structural valve and neurovascular interventions. Although common femoral artery (CFA) injuries are well-recognised, branch-vessel injuries, such as the external pudendal artery, remain underreported, despite the potential for significant bleeding and haemodynamic compromise. We present a 91-year-old lady who developed a rapidly expanding groin haematoma following PCI via femoral access, despite optimal technique using a micropuncture access kit under ultrasound gu

Tracheal intubation in ICU: Past, present, and future.

Tracheal intubation in the intensive care unit is a high-risk procedure, with hypoxemia, cardiovascular collapse, and cardiac arrest remaining frequent despite substantial progress. This narrative review outlines the evolution of ICU intubation from a primarily technical intervention to a physiologically complex procedure requiring systematic anticipation, optimization, and teamwork. Current evidence supports individualized respiratory preparation, including noninvasive ventilation and high-flow nasal oxygen in selected hypoxemic patients, head-up positioning, and cautious peri-induction venti

Mechanistic origin of the absence of pressure-induced amorphization in NH4F vs ice: The role of an intermediate rotator phase.

The absence of pressure-induced amorphization in NH4F stands in stark contrast to the behavior of ice, despite their isostructural characteristics. In this study, we developed a deep potential model for NH4F and constructed its phase diagram. Our molecular dynamics simulations reveal that the transitions from Phases Ih and Ic to Phase II proceed via an intermediate Phase IV (NaCl-type). Within this Phase IV lattice, NH4+ ions undergo active rotation, enabling a transient reconfiguration of the hydrogen-bond (HB) network. This intermediate rotator phase acts as a structural buffer, effectively

Correlative Multimodal Framework Reveals Supramolecular Chirality Loss Preceding Fibrillar Rarefaction in Dermal Collagen.

Early-stage remodeling of collagen-rich tissues is commonly inferred from fibrillar thinning and loss of connectivity, yet the structural change that initiates this process has remained unclear. Here, we show that early collagen remodeling is first expressed as a loss of supramolecular chirality coherence, rather than a reduction in collagen density or network integrity. By correlating molecular-scale chiral order with mesoscale fibrillar organization across three independent adult skin sample donors (Samples 1-3, referred to as S1-S3), we identify an abrupt collapse of chirality coherence des

Energy-Transfer-Enabled Skeletal Rearrangement of Cyclic Ketones into Strained Cyclopropane-Fused Medium Rings.

Cyclopropane-fused medium rings (CFMR) are attractive structural motifs that combine high strain with unique three dimensionality, however their efficient and modular synthesis remains a formidable challenge. Herein, we present a general photocatalytic skeletal editing strategy that enables a direct topological leap-a single-step reorganization that simultaneously alters both ring size and fusion topology-from readily available five- to eight-membered cyclic ketones into diverse seven- to ten-membered cyclopropane-fused medium rings. This transformation proceeds through a novel energy-transfer

N-halamine modified cotton dressing with thermosensitive antibacterial-antifouling switchable surface for wound healing.

Surface biofouling of cotton gauze dressings greatly promotes biofilm formation and delays wound healing. Herein a reversibly thermoresponsive switchable biointerface strategy is proposed to regulate the antibacterial and antifouling activity of cotton dressings for promoting wound healing. Poly(ethylene glycol) (PEG) brushes are covalently grafted onto cotton fibers to form a durable antifouling underlayer, followed by in situ polymerization of poly(N-isopropylacrylamide) (PNI) and subsequent chlorination to generate N-halamine antibacterial groups of N-Cl. In the initial antibacterial state,

Atypical and less-common imaging presentations of chronic nonbacterial osteitis in children: a pictorial review.

Chronic nonbacterial osteitis (CNO), including its multifocal form, chronic recurrent multifocal osteomyelitis (CRMO), is an autoinflammatory sterile bone disorder with protean imaging features across the appendicular and axial skeletons in children and adolescents. Magnetic resonance imaging (MRI)-particularly whole-body MRI (WBMRI)-is pivotal for diagnostic imaging, mapping total inflammatory burden, demonstrating multifocality, and monitoring treatment response. Radiographs are frequently normal early in the disease. Beyond the classic metaphyseal pattern, we demonstrate atypical presentati
