Single-Stage Fixation of Bilateral Open Medial Hoffa Fractures Associated with Bilateral Patella Fracture and Bilateral Distal Radius Fractures: A Case Report.

INTRODUCTION: Posterior cruciate ligament (PCL) avulsion fractures, lateral collateral ligament (LCL)-biceps femoris complex avulsion injuries are relatively uncommon injuries, and associated diaphyseal tibia fracture further is the rarest combination. It is challenging to diagnose such complex injuries, and missed injuries, particularly at the knee can lead to significant instability, increasing patients' morbidity. CASE REPORT: A 25-year-old male patient was diagnosed with PCL avulsion fracture, LCL - biceps femoris complex injury at the fibular side with Type I open mid-shaft both-bone leg

Interprosthetic Femoral Sleeves in Revision Arthroplasty: A 20-Year Experience.

BACKGROUND: Interprosthetic femoral fractures in patients with ipsilateral stemmed total hip arthroplasty (THA) and total knee arthroplasty (TKA) can be technically demanding to treat surgically. Nonunion and implant failure are among the main complications following fixation of interprosthetic femoral (IF) fractures. Total femoral arthroplasty (TFA) is associated with a high incidence of infection and instability. IF sleeves have been designed to avoid the disadvantages of these techniques and to provide a stable construct. The aim of this study was to present the results with this device fro

Use of cylindrical titanium mesh and locking plates in anterior cervical fusion. Technical note.

After performing anterior cervical corpectomy or discectomy for cervical spondolytic myelopathy or radiculopathy, iliac crest bone graft and fibular auto- or allograft is often used to achieve arthrodesis in the cervical spine. The purpose of this study was to evaluate the use of a cylindrical titanium mesh and locking plate system as an alternative technique in achieving anterior cervical fusion and maintaining lordosis. Hospital records and radiographs of 38 patients who underwent anterior cervical discectomies (28 patients) or corpectomies (10 patients) from 1995 to 1997 were reviewed retro

Knee arthrodesis with modular megaprosthesis as salvage procedure for the limb following in a patient with an infected knee tumor prosthesis: A case report.

INTRODUCTION: The management of periprosthetic knee infections is a complex event, especially in the context of megaprostheses. An accepted option in these cases is knee arthrodesis, aiming to salvage the limb with acceptable and pain-free functionality. We present a successful case of knee arthrodesis using a hybrid model of modular nail-prosthesis in a patient with massive bone stock loss following the removal of an infected tumor-type prosthesis. CASE PRESENTATION: A 65-year-old male patient with a chronic periprosthetic infection related to a resection prosthesis implanted 5 years earlier

Single-Stage Arthroscopic Anterior and Posterior Cruciate Ligament Repairs and Open Medial Collateral Ligament Repair for Acute Knee Dislocation.

Till date, there are no clear guidelines regarding the treatment of multiple ligament knee injuries. Ligament repair is advantageous as it preserves proprioception and does not involve grafting. Many studies have reported the use of open repair and reconstruction for multiple ligament knee injuries; however, reports on arthroscopic-combined single-stage anterior cruciate ligament (ACL) and posterior cruciate ligament (PCL) repairs are scarce. In this report, we describe a case of type III knee dislocation (ACL, PCL, and medial collateral ligament (MCL) injuries) in a 43-year-old man, caused by

Presence of mandibular third molars during bilateral sagittal split osteotomy increases the possibility of bad split but not the risk of other post-operative complications.

Timing of third molar removal in relation to bilateral sagittal split osteotomy (BSSO) is controversial, especially with regard to post-operative complications. We investigated the influence of mandibular third molar presence on complications after BSSO with sagittal splitters and separators, by a retrospective record review of 251 patients (502 surgical sites). Mandibular third molars were present during surgery at 169 sites and removed at least 6 months preoperatively in 333 sites. Bad splits occurred at 3.0 % (5/169) and 1.5% (5/333) of the respective sites. Presence of mandibular third mol

Treatment and differential diagnosis of aneurysmal bone cyst based on our own experience.

BACKGROUND: Aneurysmal bone cyst was first described by Jaffe and Lichtenstein in 1942. Schajowicz in 1972 in his Histopathological Classification of Primary Bone Tumors (later modified by WHO in 1993) placed it in group IX - tumor-like lesions. Its etiology and pathogenesis is not completely clear. Aneurysmal bone cysts are usually found in older children and young adults, rarely before the age of 5 or after 30. The aim of the study is to evaluate aneurysmal cyst treatment outcomes in patients we have treated and present diagnostic difficulties, especially considering the giant cell tumor in

The use of titanium surgical mesh-bone graft composite in the anterior thoracic or lumbar spine after complete or partial corpectomy.

Various conditions such as fracture, dislocation, tumor, or infection adversely affect the vertebral body and lead to instability. Restoration of a stable anterior column is essential for normal spinal biomechanics. Various biological and mechanical spacers have been used to reconstruct the anterior column after corpectomy. In this retrospective review, the authors evaluated clinical charts and radiographs of 13 patients receiving titanium surgical mesh (TSM)-bone graft composite to reconstruct the anterior spinal column in the thoracic or lumbar region. The objective of this review was to eva

Effect of screw tunnels on proximal femur strength after screw removal: A finite element analysis.

BACKGROUND: The presence of screw tunnels in the femoral neck is a problem for patients with proximal femoral fractures after removal of internal fixation. The question of how much does the existence of the screw tunnels affect the strength of the femur and whether the patient needs to be protected with an adjunctive device has been controversial. The objective of this finite element analysis was to determine (1) whether the screw tunnels affects normal weight bearing after removal of internal fixation of a proximal femur fracture, (2) which screw tunnels parameters affect the weight bearing c

Innovative development of robot reduction system in geriatric pelvic fractures: A single-center case series in Beijing, China.

Displaced fragility fractures of the pelvis (FFP) pose significant challenges in orthopaedic trauma, owing to patient comorbidities, deteriorating bone quality, and surgical complexities. Despite technological advancements, no robotic methods have been documented for displaced FFP management. To address this, we introduced an advanced robot-assisted fracture reduction system, comprising a tracking device, path planning software, and robotic arms. This study evaluated fifteen consecutive patients with displaced FFP (average age 80.4 ± 9.1 years), who underwent robot-assisted reduction and inter

A Gustilo Type 3B Open Tibial Fracture Treated with a Proximal Flexor Hallucis Longus Flap: A Case Report.

INTRODUCTION: In the treatment of Gustilo Type 3B open tibial fractures, it is important to perform soft tissue reconstruction and bone reconstruction simultaneously. Gastrocnemius muscle and soleus muscle flaps are generally used as rotational flaps for the tibia. The distal third of the tibia can often not be covered with the gastrocnemius muscle and soleus muscle flaps. Treatment distal to the distal third of the tibia is difficult because fewer flap options are available. In the present report, we describe our experience with a Gustilo Type 3B open tibial fracture treated by gastrocnemius
