Prospects for recombinant vaccines against Babesia bovis and related parasites.

Babesial parasites infect cattle in tropical and temperate regions of the world and cause significant morbidity and mortality. Discovery of protective antigens that could be used in a killed vaccine has been slow and to date there are few promising vaccine candidates for cattle Babesia. This review describes mechanisms of protective innate and adaptive immune responses to babesial parasites and different strategies to identify potentially protective protein antigens of B. bovis, B. bigemina, and B. divergens. Successful parasites often cause persistent infection, and this paper also discusses

Pharmacotherapy for atrial fibrillation: is rhythm control achievable?

As the most common sustained cardiac arrhythmia, the health burden presented by atrial fibrillation (AF) continues to grow. One of the current dilemmas in managing AF lies in the decision of whether to pursue a rate or rhythm control strategy. Current antiarrhythmic drugs have many limitations in terms of efficacy and adverse effects, and the availability of better antiarrhythmic drugs that are effective and safe would probably improve outcomes in AF. Enthusiasm for pulmonary vein ablation procedures has been tempered by the relatively high frequency of recurrences of AF, which are often asymp

False and non-localizing signs in neuro-ophthalmology.

The absolute reliance on abnormal neuro-ophthalmologic findings as signposts for particular lesions of the neuraxis must be tempered by a working knowledge of false and nonlocalizing signs and symptoms. Transient monocular visual loss or complex visual hallucinations may lead to neuroanatomic ambiguity, and elevated intracranial pressure is not the most common cause of swollen optic disks in late life. Sixth nerve palsy associated with increased intracranial pressure is possibly the best-known false localizing sign, but other ocular motility disturbances (divergence paresis, convergence insuff

Short-term effect of fresh disk allograft on the osteoarthrotic sheep temporomandibular joint.

UNLABELLED: The aim of this experimental study is to evaluate the effect of fresh disk allograft on the osteoarthrotic temporomandibular joints in four sheep. STUDY DESIGN: Bilateral osteoarthrosis was induced in the sheep's temporomandibular joint. Three months later, unilateral diskectomy and fresh disk allograft repair were performed, with the contralateral joint being left as a control. At 6 months after the initial procedure, 3 months after the repair, the four sheep were sacrificed. RESULTS: It was found that the operated joint was fibrously repaired without ankylosis, whereas the cont

A controlled study of methylphenidate in the treatment of attention deficit disorder, residual type, in adults.

Thirty-seven adult patients meeting the Utah criteria for attention deficit disorder, residual type, were entered into a double-blind crossover trial of methylphenidate and placebo. A moderate-to-marked therapeutic response occurred in 21 (57%) of the patients while receiving methylphenidate and in four (11%) while receiving placebo, a highly significant difference statistically and clinically. The responding patients showed significant improvement in the following areas: attentional difficulty, motor overactivity, affective lability, and impulsivity. The diagnosis of attention deficit disorde

Acute respiratory disease in University of the Philippines and University of Wisconsin students. A comparative study.

In a comparison of acute respiratory disease patterns and incidence in students in a semi-tropical climate at the University of the Philippines with those in students in a temperate climate at the University of Wisconsin, USA, it was found that, while respiratory infections were the commonest cause of infirmary admissions in both institutions, yet, contrary to expectations, their incidence and relative importance were actually greater in the Philippine students than in the Wisconsin students. Peak rates occurred during the rainy season in the Philippines and during the coldest months in Wiscon

Microsporidium Keratitis After Hurricane Francine: A Case Report.

Microsporidial ocular infections are being increasingly reported, especially in temperate climates. In this case report, we describe five patients who presented to a private ophthalmology practice in Southeastern Louisiana in 2024, all within 3 months of Hurricane Francine landing. The patients' ages ranged from 5 to 23 and included two female and three male patients. All patients except one had exposure to contaminated water, and microsporidia were detected via PCR in all patients. Microsporidial keratoconjunctivitis was diagnosed based on typical clinical features. The patients were all effe

10 years in lupus - progress, but not enough.

Systemic lupus erythematosus (SLE, lupus) remains an enigmatic diagnosis with a poor prognosis. SLE is characterised by complex biology and heterogeneous clinical manifestations that create a major hurdle to the approval of new medicines and contribute to multiple trial failures. While the pace of research has accelerated, drugs currently in development target a limited spectrum of mechanisms, tempering hope that any will be a panacea. The pace of translation lags behind advances in basic science, and this continues to cost patients dearly. The unacceptable metabolic adverse effects of glucoco

Glucagon-Like Peptide-1 Receptor Agonists for Arthritis and Osteoarthritis.

Glucagon-like peptide-1 receptor agonists (GLP-1RAs) play a pivotal role in managing type 2 diabetes mellitus (T2DM), obesity, and other conditions, with proven benefits including, but not limited to, weight loss, glycemic control, and cardiovascular protection. However, their implications for arthritis are still debatable. GLP-1RAs may reduce systemic inflammation, alleviate joint stress through overall weight loss, and exhibit chondroprotective effects. Nevertheless, concerns such as augmented gout flares, joint pain, and indefinite long-term outcomes temper their promise. This editorial hig

Organ Donor Transmission of Rickettsia typhi to Kidney Transplant Recipients, Texas, USA, 2024.

Murine typhus, a fleaborne disease caused by the bacterium Rickettsia typhi, is found throughout temperate and tropical regions of the world. Transmission of R. typhi to humans involves several species of fleas, and most infections result from direct inoculation of R. typhi-infected flea feces into abrasions in the skin. We describe the transmission of R. typhi from an organ donor in Texas, USA, to 2 kidney transplant recipients. The donor and 1 recipient died from the infection. The occurrence of R. typhi transmission via transplantation is a harbinger for the reemergence of murine typhus in

What Is the Psychosocial Impact of Providing Genetic and Genomic Health Information to Individuals? An Overview of Systematic Reviews.

Optimistic predictions that genetic and genomic testing will provide health benefits have been tempered by the concern that individuals who receive their results may experience negative psychosocial outcomes. This potential ethical and clinical concern has prompted extensive conversations between policy-makers, health researchers, ethicists, and the general public. Fortunately, the psychosocial consequences of such testing are subject to empirical investigation, and over the past quarter century, research that clarifies some of the types, likelihood, and severity of potential harms from learni

Overwintering of West Nile Virus in the United States.

The establishment of a tropical virus such as West Nile (WNV; Flaviviridae: Flavivirus) within the temperate latitudes of the continental United States was unexpected and perhaps contingent, in part, upon the ability of this invasive virus to persist during winter when temperatures become too cold for replication and vector mosquito gonotrophic activity. Our Forum article reviews research examining possible overwintering mechanisms that include consistent reintroduction and local persistence in vector mosquitoes and avian hosts, mostly using examples from research conducted in California. We c

[Leptospirosis in a family after whitewater rafting in Thailand].

Leptospirosis is a zoonosis found worldwide, with an incidence that is approximately 10 times higher in the tropics than in temperate regions. The main reservoir of leptospirosis is the rat and human infection usually results from exposure to infected animal urine or tissues. Only 10% of cases are symptomatic. We present here two confirmed and two probable cases of leptospirosis in a family returning from whitewater rafting in Thailand, illustrating the wide variety of the clinical manifestations of this infection. Two of the patients were hospitalized and presented a probable Jarisch-Herxheim

Flavivirus NS1 structures reveal surfaces for associations with membranes and the immune system.

Flaviviruses, the human pathogens responsible for dengue fever, West Nile fever, tick-borne encephalitis, and yellow fever, are endemic in tropical and temperate parts of the world. The flavivirus nonstructural protein 1 (NS1) functions in genome replication as an intracellular dimer and in immune system evasion as a secreted hexamer. We report crystal structures for full-length, glycosylated NS1 from West Nile and dengue viruses. The NS1 hexamer in crystal structures is similar to a solution hexamer visualized by single-particle electron microscopy. Recombinant NS1 binds to lipid bilayers and

The eponymous Dr James Marion Sims MD, LLD (1813-1883).

Dr James Marion Sims was born in 1813 in Lancaster County, South Carolina. It was while pioneering numerous surgical procedures in Alabama that in 1849 he achieved the outstanding landmark in medical history of successfully, and consistently, repairing vesicovaginal fistulae. Sims soon developed a reputation as a fine surgeon, with new operations and techniques, using novel surgical instruments and his innovative approaches frequently published. Moving to New York City in 1853, he further established hospitals devoted entirely to women's health. Sims was controversial, with flamboyant descript

Intracranial Aspergillus granuloma.

Intracranial fungal granulomas are rare and of the histologically verified granulomas, Aspergillus spp. is the commonest causative fungal pathogen. Most of the reported large series of aspergillus granulomas are from countries with temperate climate like India, Pakistan, Sudan, and Saudi Arabia. In contrast to disseminated aspergillosis that occurs in immunosuppressed individuals, most of the intracranial aspergillus granulomas are reported in immunocompetent individuals. The temperature, humidity, high spore content in the atmosphere during ploughing, and occupation as agricultural worker are

Radioscapholunate arthrodesis.

Although RSL fusion is a viable option for isolated radiocarpal arthritis, the enthusiasm for this procedure should be tempered with the reality that kinematics of the wrist is not entirely suited for independent midcarpal flexion and extension [10]. Limited wrist flexion and extension is expected following a successful RSL arthrodesis. The effects of imposed abnormal kinematics are further shown by the high incidence of RSL nonunions, occurrence of scaphoid fractures, and postoperative deterioration of the midcarpal joint [15,22]. In a young patient with posttraumatic arthritis or rheumatoid

Babesia sp.: emerging intracellular parasites in Europe.

The emergence of Lyme borreliosis as the most prevalent arthropod disease of humans in the temperate northern hemisphere has resulted in renewed interest in human babesiosis, transmitted by the same tick vectors. The advent of new molecular tools has made possible a reappraisal of the main parasites involved (Babesia divergens in Europe and Babesia microti in the USA). B. divergens is probably restricted to European cattle, though there are several nearly identical species. B. microti occurs as a world-wide species complex rather than as a single species, and although both phenotypic and genot

Superficial fungal infections.

Superficial fungal infections arise from a pathogen that is restricted to the stratum corneum, with little or no tissue reaction. In this Seminar, three types of infection will be covered: tinea versicolor, piedra, and tinea nigra. Tinea versicolor is common worldwide and is caused by Malassezia spp, which are human saprophytes that sometimes switch from yeast to pathogenic mycelial form. Malassezia furfur, Malassezia globosa, and Malassezia sympodialis are most closely linked to tinea versicolor. White and black piedra are both common in tropical regions of the world; white piedra is also end

Clinical review 51: Management of hypercalcemia.

Hypercalcemia is a very common electrolyte abnormality that is most often seen as a mild elevation in the setting of asymptomatic primary hyperparathyroidism. The hypercalcemia is usually less than 11.2 mg/dL and does not call for specific or vigorous approaches to management. However, more marked elevations in the serum calcium are also seen and call for a more specific approach to management. When the serum calcium is only moderately elevated, the therapeutic approach should be tempered by clinical assessments about the presence of symptoms and the underlying etiology. When the serum calcium
