A Systematic Approach to the Evaluation of Acute Musculoskeletal Injuries in the Emergency Department.

Musculoskeletal (MSK) complaints account for a significant proportion of emergency department (ED) visits and range from minor soft tissue injuries to fractures, tendon ruptures, dislocations, septic arthritis, and compartment syndrome. Despite their frequency, these presentations can be diagnostically challenging because of variable mechanisms of injury, overlapping examination findings, and limitations of plain radiography for detecting soft tissue and occult bony injuries. Missed diagnoses, such as Achilles tendon rupture, scaphoid fracture, Lisfranc injury, and tibial plateau fracture, may

Concomitant intra-articular glenohumeral injuries in displaced fractures of the lateral clavicle.

PURPOSE: To detect concomitant intra-articular glenohumeral injuries, in acute displaced fractures of the lateral clavicle, initially missed due to unfeasible clinical evaluation of the acutely injured shoulder. METHODS: All patients suffering from an acute displaced lateral clavicle fracture with indication to surgical treatment underwent diagnostic shoulder arthroscopy prior to open reduction and internal fixation. In case of therapy-relevant intra-articular glenohumeral injuries, subsequent surgical treatment was performed. RESULTS: Intra-articular injuries were found in 13 of 28 patients

Pectoralis major ruptures.

Injuries of pectoralis major (PM) muscle are among rare muscle injuries. Due to the lack of experience and knowledge in this field, they are often missed or improperly treated. Most often they are caused by an indirect mechanism, during strength sports, typically using the bench press. In clinical terms, they are typically characterized by specific disorder of the anterior axillary fold and a hematoma and, after subsidence of the swelling, disfiguration with medial retraction of the muscle belly. The most beneficial imaging method to examine these injuries is MRI with an emphasis on a properly

Nanostructural and nanomechanical responses of collagen fibrils in the collagenase-induced Achilles tendinitis rat model.

Achilles tendons are vulnerable to acute or chronic injuries that lead to inflammation. We investigated nanostructural and nanomechanical changes in collagen fibrils from rat Achilles tendons over a period of 9 weeks after injury using atomic force microscopy (AFM). To evaluate the nanostructural changes in Achilles tendons, we measured the diameter and D-banding of collagen fibrils by AFM. And the adhesion forces, which were related to cross-linking of collagen, were calculated from the retraction process of a force-distance curve. We successfully observed the time course of changes in collag

Enhanced Achilles tendon healing by fibromodulin gene transfer.

UNLABELLED: Tendon injury is a major musculoskeletal disorder with a high public health impact. We propose a non-viral based strategy of gene therapy for the treatment of tendon injuries using histidylated vectors. Gene delivery of fibromodulin, a proteoglycan involved in collagen assembly was found to promote rat Achilles tendon repair in vivo and in vitro. In vivo liposome-based transfection of fibromodulin led to a better healing after surgical injury, biomechanical properties were better restored compared to untransfected control. These measures were confirmed by histological observations …

Tears of the subscapularis tendon in athletes--diagnosis and repair techniques.

Isolated injury to the subscapularis tendon is an uncommon - but potentially debilitating - injury among athletes. The most frequent injury pattern seen in younger athletes is an acute, traumatic tendon avulsion. In the older athletic population, chronic degenerative tearing of the tendon may precede sports-related rupture, particularly following shoulder dislocation. This article reviews how to diagnose this injury in athletes, and describes both conservative and operative treatment options and their outcomes.

Collagen V haploinsufficiency in female murine patellar tendons results in altered matrix engagement and cellular density, demonstrating decreased healing.

Collagen V (Col5) is a quantitatively minor component of collagen fibrils comprising tendon, however, plays a crucial role in regulation of development and dynamic healing processes. Clinically, patients with COL5a1 haploinsufficiency, known as classic Ehlers-Danlos Syndrome (cEDS), present with hyperextensible skin, joint instability and laxity, with females more likely to be affected. Previous studies in Col5-deficient mice indicated that reduced Col5a1 expression leads to a reduction in stiffness, fibril deposition, and altered fibril structure. Additionally, Col5-deficient male tendons dem

Hand injuries in young children.

Four hundred and fifty five young children (0-6 years old) were treated for hand injuries between 1996 and 2000. Boys (61%) were injured more often and a higher number of injuries occurred during May and September. Fingertip injuries were the most common injuries (37%), and were often caused by jamming in doors at home. Fractures were caused by falls and punches and tendon/nerve injuries by sharp objects. The incidence of hand injuries increased from 20.4/10,000/year in 1996 to 45.3/10,000/year in 2000. Only 4% of the children had complex injuries but these placed a high demand on resources. T

The biomechanics and imaging of soccer injuries.

Soccer is one of the most popular sports worldwide and requires the player to have considerable stamina while superimposing sprinting, jumping, tackling, rapid changes of direction, and kicking. This contact sport carries a significant risk for acute and overuse injuries, with the lower limb the region most frequently injured. This article discusses the biomechanics and current role of imaging in the management of several specific soccer injuries highlighting injuries of the ankle, pelvis, and groin and specific muscle and tendon injuries commonly encountered in this athletic population.

Bilateral anatomical variations in the extensor compartment of forearm and hand.

An unusual variation of the extensor muscles was found during the routine dissection of the posterior compartment of the forearm. The left forearm presented an extensor medii proprius muscle, the tendon of which had an unusual trajectory. It passed through the second extensor compartment between the tendons of the extensor carpi radialis longus and extensor carpi radialis brevis muscles. The right forearm presented two muscles for the index finger: one, the extensor indicis et medius communis, the tendon of which was split into three tendons, one radial and one ulnar for the index finger and a

Hook of the hamate fractures. Diagnosis, treatment, and complications.

Four cases of undiagnosed hook of the hamate fracture are presented. In each case, the initial injury was considered trivial; medical attention was not sought until rupture of the little profundus/superficialis tendon occurred. All were treated with excision of the hook and tendon repair, which usually consisted of an end-to-side or "Y" type repair of the little to the ring profundus. Postoperatively, all patients were pain-free; they returned to their respective preinjury activities, although most had some limitation of motion in the digits with the tendon repair. A review of the English lite

A Unique Presentation of Bilateral Posterolateral Corner Injury With Right Posterior Cruciate Ligament and Left Anterior Cruciate Ligament Tears.

Bilateral posterolateral corner (PLC) injuries of the knees are exceptionally rare, particularly when associated with asymmetric cruciate ligament involvement. This case report presents a 23-year-old man who sustained simultaneous bilateral PLC injuries with a right posterior cruciate ligament (PCL) tear and left anterior cruciate ligament (ACL) tear following a single trauma, during a stampede. Initial examination revealed bilateral varus instability with positive dial tests. Magnetic resonance imaging (MRI) confirmed combined PLC and cruciate ligament injuries with associated fibular collate

A Rare Case of Bilateral Spaghetti Injuries in Children due to Assault by Mother and Analysis of Functional Outcome at Three Years.

Management of child abuse with flexor tendons, neurovascular injuries, and life-threatening conditions is challenging. It needs a multisectoral coordinated and synchronized team effort for successful outcomes. We present a case series of children abused by a parent with a sharp object. The children sustained multiple flexor tendon injuries, neurovascular injuries in upper limbs, and tracheal injury compromising respiration. We performed a tracheostomy to save a child and subsequently repaired numerous flexor tendons, nerves, and arteries. During follow-up, these children required secondary rec…

Relative Motion Extension Orthosis Versus Classic Immobilization in Extensor Tendon Repairs (Zones IV-VI of the Hand): A Randomized Controlled Trial.

BACKGROUND: Traditional treatment for extensor tendon injuries typically involves tenorrhaphy followed by a 4- to 6-week period of immobilization in a plaster orthosis. This study compares the efficacy of relative motion extension (RME) orthoses versus classic immobilization (CI) in promoting recovery after such injuries. METHODS: We conducted a randomized, controlled, blind clinical trial approved by the Galicia ethics committee. Participants were individuals with single tendon injuries in the long fingers. They were randomly assigned to either the RME or the CI treatment group. Outcome meas…

Comparative Efficacy of Surgical Versus Nonsurgical Management for Acute Achilles Tendon Rupture in a Novel Mouse Model.

BACKGROUND: Acute Achilles tendon rupture is a common and serious injury in sports medicine. Clinical studies demonstrate that both surgical and nonsurgical interventions can achieve satisfactory outcomes; however, considerable debate exists regarding the optimal treatment modality for this injury. Currently, most animal experimental studies on acute Achilles tendon rupture lack clinical relevance due to inadequate fixation of the ankle joint. METHODS: This study involved 162 male C57BL/6 mice and 30 Scx-CreER T2 ; Rosa26-tdTomato transgenic mice. The injury+repair groups underwent Achilles t

Quadriceps Tendon Repair Using Double-Row Suture Anchor Fixation.

Quadriceps tendon ruptures compromise the knee extensor mechanism and cause an inability to ambulate and significant functional limitations. Therefore, the vast majority of quadriceps tendon ruptures are indicated for operative intervention to restore patient mobility and function. Although these injuries were traditionally repaired using a transosseous repair technique, recent literature has shown that suture anchor repair may offer biomechanical advantages. Additionally, research in other areas of orthopaedics has found that a double-row suture anchor construct can offer additional biomechan
